The lungs are a port of entry and primary infectious focus of Coxiella burnetii, the obligate intracellular contagium of the worldwide zoonosis Q fever. The infectious process and immune response are characterised by studies in cell culture and animal systems. Following endocytosis, replication exclusively occurs in the phagolysosome. Several potential virulence factors are described.

OBJECTIVE: To study the frequency of Q fever in HIV-infected individuals.
DESIGN: A seroprevalence study.
SETTING: French National Reference Centre for Rickettsial Agents, Marseille, France.
PATIENTS AND METHODS: Five out of the 68 hospitalized cases of Q fever diagnosed in 1987-1989 were also HIV-infected and are described here. We describe six men with a slowly progressive myelopathy characterized by asymmetric, incomplete spinal cord syndrome manifested with a thoracic sensory level, mild spastic paraparesis, and urinary incontinence. The spinal cord lesions were evident by MRI in four of them. An outbreak of Q fever in a university department where sheep placentas were being used for research is described. Of six persons exposed to the sheep, four had positive titers with only one person developing an acute febrile illness and liver disease. This report illustrates the value of the family physician obtaining an occupational history and conducting an outbreak investigation.

A review on Q fever epidemiologic aspects in Roumania from the first case report in 1947 up to the actual cases is presented. It includes the dynamics of the number of cases, features of epidemics, results of surveys on slaughterhouse workers and laboratory-acquired cases, results of serologic surveys in domestic animals, and of wild life studies.
