IMPORTANCE: Historical structural racism may be associated with racial, ethnic, and geographic disparities in breast cancer outcomes, but few studies have investigated these potential relationships.
OBJECTIVE: To test associations among historical mortgage lending discrimination (using 1930s Home Owners' Loan Corporation [HOLC] redlining data), race and ethnicity, tumor clinicopathologic features, and survival among women recently diagnosed with breast cancer.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study used a population-based, state cancer registry to analyze breast tumor clinicopa

Overdose in California is highly racialized, with Black Californians experiencing excessive mortality. People experiencing homelessness (PEH) report high rates of fentanyl-stimulant co-use and drug-related harm. There is limited research examining structural racism and intersectional violence of policing, homelessness, and overdose throughout the lifespan of Black people. POLY HOME is a qualitative study among PEH who fentanyl and stimulants in San Francisco, California, examining how housing status shapes drug-related harm and engagement with overdose prevention, services, and treatment. Indigenous people in Canada continue to experience systemic racism, disrespect, and loss of autonomy within the healthcare system, particularly in the context of pregnancy and childbirth. Despite national commitments to reconciliation and the recognition of Indigenous rights through frameworks such as the Truth and Reconciliation Commission (TRC) and the United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP), the realities of maternity care for Indigenous people are shameful. This study draws on data from a national survey examining respect, mistreatment and autonomy in pregna

PURPOSE OF REVIEW: In the last two decades, empirical research has significantly advanced our understanding of the link between discrimination and cardiovascular disease (CVD). This integrated scoping and narrative literature review delineates the extant peer-reviewed research on discrimination and clinical and subclinical CVD in samples that include Black adults, using a multilevel conceptualization of race-related discrimination and racism. We also identify potential intermediary mechanisms in the racism-CVD relationship and propose a comprehensive future research agenda.

OBJECTIVE: Experiences of racism have been linked to increased race-based traumatic stress symptoms. However, studies in this area have not focused solely on Chinese individuals in North America, such as those in the United States and Canada. Furthermore, little is known about protective factors that may buffer the adverse impact of racism on race-based traumatic stress symptoms. Addressing these limitations, this study examined (a) the relationship between racism and race-based traumatic stress symptoms and (b) the protective role of both individual and collective resilience in the associatio

Young women of color frequently face discrimination, reflecting the intersecting societal influences of sexism and racism. Although friendships play a significant role in women's lives, there is a lack of research on the role of friendships in navigating exposure to gendered racial discrimination (in-person and social media) and associated mental health. This study investigated the extent to which the content of friendship conversations (i.e., co-rumination against gendered racism, socializing messages related to gendered racial pride and empowerment and oppression awareness) and perceived fri

Black gay, bisexual, and other men who have sex with men (BMSM) experience the highest rates of HIV acquisition annually out of any population in the United States, and young BMSM (YBMSM) are heavily impacted by this inequity as they enter adulthood. Despite a high annual HIV incidence, extant literature has found BMSM to engage in fewer sexual risk behaviors than White and Hispanic/Latino men who have sex with men, resulting in a gap between risk behaviors and the inequity of HIV infection. Structural factors, such as racism and homophobia, are thus being examined in order to understand this

PURPOSE: Structural racism and racial discrimination may increase adverse sexual health outcomes in Black adolescent girls. However, the influence of racism on sexual health has not been well-described in qualitative studies. The purpose of this study was to explore definitions of structural racism and determine how experiences of racial discrimination impact sexual health and decision-making in a sample of adolescent Black girls.
METHODS: Black girls aged 13-19 years were recruited from an ongoing longitudinal study. Virtual focus groups and individual interviews were facilitated, inquiring

HIV prevalence and engagement in sexual behaviors associated with HIV transmission are high among transgender people of color. Per intersectionality, this disproportionate burden may be related to both interpersonal and structural racism and transphobia. The goal of this study was to estimate the association between interpersonal and structural discrimination and sexual behaviors among transgender and gender diverse (TGD) U.S. young adults. We used logit models with robust standard errors to estimate the individual and combined association between interpersonal and structural racism and transp

UNLABELLED: Although numerous studies have documented an association between racial discrimination and internalizing psychopathology symptoms, there is a lack of empirical work that establishes cognitive and emotional mechanisms through which racial discrimination is associated with specific transdiagnostic mental health outcomes (i.e., anxious arousal and low positive affect) among Black Americans.
OBJECTIVE: The goal of this study was to test a new etiological model of how racial discrimination is associated with anxious arousal and low positive affect. The overarching model posits that rac

BACKGROUND: Experiences with lesbian, gay, bisexual, transgender, or queer (LGBTQ) discrimination and racism have both been associated with mental health problems and illicit drug use. However, the cumulative effects of both forms of discrimination--and resulting internalized oppression--on illicit drug use in LGBTQ people of color (POC) has not been examined in the research literature.
METHODS: Using online questionnaires, this study collected self-report data from 200 LGBTQ POC about their experiences with racism, LGBTQ discrimination, internalized racism, internalized LGBTQ discrimination,

Our objective was to examine the role of structural racism and economic disadvantage in perinatal health inequities using the Environmental influences on Child Health Outcomes Cohort. Participants' addresses were linked to area-level measures of life expectancy, education, unemployment, health insurance, jail rate, segregation, and housing cost burden. We created absolute measures to represent economic disadvantage and relative measures comparing values for Black or Latinx people to White people in the same area to represent structural racism. We used quantile G-computation to estimate the eff

OBJECTIVES: Parental racial socialization refers to the messages children receive about race from their parents. Little is known about parental racial socialization's long-term impact on problematic substance use or whether it can protect against adversities commonly experienced by Black Americans. We examined whether parental racial socialization reduces the risk of problematic substance use and buffers the impact of neighborhood disadvantage and racial discrimination on problematic substance use, with attention to the types of messaging.
METHOD: Data are from a sample (n = 825) of the Woodl

BACKGROUND: Since the COVID-19 pandemic began, a dramatic rise in anti-Asian sentiment as well as cissemxism and heterosexism (hereafter, cisheterosexism) has been documented, particularly affecting Asian American (AA) sexual and gender minority (SGM) adolescents who face intersecting oppressions.
PURPOSE: We investigated the associations between state-level structural anti-Asian racism, structural cisheterosexism, and psychological, behavioral, and sleep health outcomes among AA SGM adolescents.
METHODS: This study analyzed a subsample of AA SGM adolescents (N = 980) from a US cross-section

INTRODUCTION: Racial and ethnic disparities in tobacco use and related health consequences persist. Experiences with racial discrimination have been associated with higher likelihood of smoking and worse cessation outcomes. The purpose of this study is to examine associations between perceived discrimination with serious quit attempts, smoking relapse, and changes in motivation to quit smoking, during the COVID-19 pandemic.
METHODS: Data came from an October-November 2020 US nationally representative cross-sectional survey of adults who currently smoked cigarettes (n=1223). Participants repor

Quantifying the effect of structural racism on health is a subject of sustained interest. To measure structural racism, analysts often calculate contrasts between marginalized and privileged groups on measures of social determinants in health. These contrasts are predominantly calculated on the relative scale, rather than the absolute scale. In this paper, we investigated the rank ordering of 38 US states in terms of the relative and absolute difference in education and incarceration between Black and White individuals using 2015-2019 data from the American Community Survey and the National Pr

BACKGROUND: Structural racism is how society maintains and promotes racial hierarchy and discrimination through established and interconnected systems. Structural racism is theorized to promote alcohol and tobacco use, which are risk factors for adverse health and cancer-health outcomes. The current study assesses the association between measures of state-level structural racism and alcohol and tobacco use among a national sample of 1,946 Black Americans.
METHODS: An existing composite index of state-level structural racism including five dimensions (subscales; i.e., residential segregation a

Although Black girls use substances at lower rates than boys and girls from various other racial groups, they tend to have worse health outcomes associated with substance use that can also impact their sexual health. The association between substance use and sexual risk behaviors is usually attributed to lack of access to quality health care and lack of culturally specific prevention programming and treatment options tailored to this group. Accordingly, the theoretical frameworks for health promotion for Black girls often focus on addressing deficits, ignoring the powerful and intersecting soc

Structural racism is increasingly recognized as a key driver of health inequities and other adverse outcomes. This paper focuses on structural racism as an "upstream" institutionalized process, how it creates health inequities and how structural racism persists in spite of generations of efforts to end it. So far, "downstream" efforts to reduce these health inequities have had little success in eliminating them. Here, we attempt to increase public health awareness of structural racism and its institutionalization and sociopolitical supports so that research and action can address them. This pa
