Arterial occlusive disease has been recognized in association with radiation arteriopathy and, rarely, with spontaneous arterial disruption. This association results from the greater role of radiation therapy in the current management of malignant diseases coupled with longer patient survival and the lengthy latency period between radiation and clinical manifestations of radiation arteriopathy. Experience with six patients having radiation-associated arterial disease was retrospectively reviewed. There were four men and two women, with a mean age of 51 years (range, 36-74). Arteries exposed to…

A number of methods of managing radiation-induced skin reactions have been presented in this article. However, currently no standard of care exists among institutions and physicians in managing these skin reactions. In fact, little nursing research has been performed on managing radiation skin reactions. The Radiation Therapy Special Interest Group (RTSIG) of the Oncology Nursing Society is in the process of developing radiation-induced symptom management research. Until these data are available, it is recommended that nurses caring for patients with radiation-induced skin reactions focus on e…

It is apparent from preclinical and clinical research to date that continued evaluation of new and alternative treatment strategies is required to eliminate the obligate periods of neutropenia and thrombocytopenia after acute high-dose irradiation. Future treatment strategies may involve new combinations of cytokines to affect hematopoietic stem cell proliferation and "engineered" cellular grafts to provide short-term in vivo expansion of neutrophils and platelets in an effort to bridge the cytopenic gap until endogenous or transplanted stem cells regenerate the hematopoietic and immune system…

Radiation therapy of cancers in the pelvic region may lead to radiation proctitis. Radiation injury to the rectal wall eventually causes connective tissue fibrosis and obliterative endarteritis with subsequent local tissue ischemia. Patients with radiation proctitis may be minimally ill and heal spontaneously. However, symptoms of proctitis may persist, and the disease progresses to chronic bleeding and/or stricture and fistula formation. Medical therapy is often unsuccessful, and surgery is eventually required. Because of numerous postoperative complications and no guarantee of success, surge…

The marked radiosensitivity of renal tissue represents a limitation on the total radiotherapeutic dose that safely can be applied to treatment volumes that include the kidneys. Radiation nephropathy is characterized by a progressive reduction in renal hemodynamics associated with a severe anemia. The latter is often normochromic normocytic in character, but can progress to a microangiopathic hemolytic anemia. The pathogenic mechanisms responsible for the development of radiation nephropathy remain ill-defined. Experimental studies which allow serial determinations of functional, morphologic, a…

The clinical features and long-term outcome of seven patients with delayed cerebral radiation necrosis (DCRN) are described. Radiotherapy had been given for pituitary tumour (1), astrocytoma (2), pinealoma (2), craniopharyngioma (1) and parotid carcinoma (1). The mean latency to onset of the first neurological symptoms was 22 months (range 6-40 months), and mean duration of follow-up was 86 months (range 60-126). Three patients died at a mean of 84 months after radiotherapy (range 62-98). A fourth patient probably died from metastatic disease. Three patients remain alive, albeit severely disab…

Solar radiation is a well documented risk factor in melanoma and non melanocytic skin cancer (i.e. basal cell and squamous cell carcinoma). It has been suspected to play a role also for lip cancer and for eye melanoma. Primary prevention should include advises to the public about sensible sun exposure. Results of such efforts will be apparent only in a long term perspective.

Radiation therapy is administered to approximately one third of patients with cancer as part of their treatment plan. Radiation-induced bowel injury is a major cause of morbidity in these patients. The pathophysiology of this condition as well as recommendations for the management of acute and chronic radiation enteritis are discussed. In general, except for patients presenting with signs of an acute abdomen, conservative management yields the best clinical results.

Thyroid cancer was the first solid tumor that showed an increased incidence among the Japanese A-bomb survivors and recently published data indicated an increase of thyroid cancer among children in Belarus. The annual thyroid cancer rate between 1986 and 1989 was 4 cases and 2 years later a 14-fold increase was found. That study has several methodological weaknesses but is nevertheless alarming. The present paper reviews the current epidemiological knowledge on radiation-induced thyroid cancer, and discusses the methodological difficulties. It is concluded that low doses of brief gamma radiati…

Radiation-induced optic neuropathy is a well-known complication of cranial radiotherapy. We describe four cases. In two cases, the diagnosis was made on MRI. After administration of Gadolinium-DTPA, there was an enhanced captation at the level of the optic nerve. In addition we reviewed 47 cases reported in the literature. Visual prognosis is less than 1/10 in 78% of affected eyes. In case of associated brain necrosis, the mortality rate is 62%.

The prevalence of radiation-associated cardiac disease is increasing due to prolonged survival following mediastinal irradiation. Side effects of radiation include pericarditis, accelerated coronary artery disease, myocardial fibrosis and valvular injury. We evaluated the cases of three young patients with evidence of significant valvular disease following mediastinal irradiation. One patient underwent the first reported successful aortic and mitral valve replacement for radiation-associated valvular disease (RAVD) as well as concurrent coronary artery revascularization. A review of the litera…

Thyroid cancer is a well documented late effect of exposure to ionizing radiation. The excess risk begins 5-10 years after exposure and continues until at least 40 years after exposure. Females are roughly three times more susceptible to both radiogenic thyroid cancer and to thyroid cancer of other origins than are males. Therefore, relative risk estimates for radiogenic thyroid cancer do not necessarily differ by sex. The excess risk is higher among children exposed prior to five years of age than in those exposed later. The risk for radiogenic cancer following exposure to 131I appears to be …

Current research indicates a role for several oncogenes in radiation-induced carcinogenesis in vivo and cell transformation in vitro. Certain oncogenes are probably also involved in some cases of human cancer caused by exposure to nonionizing radiation and may play a mechanistic role in the phenomenon of radioresistance seen in later stages of tumor progression. The mechanisms of oncogene activation seen in radiation-induced tumors include point mutations, gene amplification, and changes in gene expression. Genetic factors associated with target species, strain, and tissue type play an importa…

Ultraviolet radiation from high-intensity sources has well-known acute effects on the eye and skin, consisting primarily of photokeratoconjunctivitis and sunburn, which are enhanced in the presence of photosensitizing agents. Long-term elevated exposure to low-level ultraviolet radiation is also responsible for an increased risk of squamous- and basal-cell carcinomas; probably responsible for an increased risk of cortical and posterior subcapsular cataracts, pterygium, and cutaneous melanoma and intraocular melanoma and possibly responsible for immunologic effects of clinical significance.

External irradiation of the thyroid causes the subsequent development of thyroid malignancies at an approximate rate of 5 per rad per million people per year. There is no certain lower threshold, but possibly doses above 3000 rads are less effective. The tumours produced are primarily differentiated malignancies typical of non-irradiated patients, and the clinical course is similar. Most radiation-associated malignancies followed treatment given for enlarged thymic glands, enlargement of the tonsils or adenoids, and acne. Currently patients are seen with radiation-associated thyroid disease fo…
