Sepsis is a clinical syndrome encompassing physiologic and biologic abnormalities caused by a dysregulated host response to infection. If sepsis progresses into septic shock it is associated with a high mortality, therefore early identification and treatment is critical. Interventional radiologists often perform treatments which may cause or temporarily exacerbate sepsis. Therefore, it is important for the interventional radiologist to recognize early signs of sepsis and understand sepsis treatment protocols. These protocols include source control of infections, administration of systemic broa

PURPOSE: To establish Portuguese Diagnostic Reference Levels (DRLs), for six body fluoroscopy guided interventional procedures (FGIP).
METHOD: A retrospective study was conducted in five interventional departments most representative of Interventional Radiology (IR) practice. Dose values, in terms of air kerma area product (PKA in Gy.cm2), air kerma at the patient entrance reference point (Ka,r in mGy), and exposure parameters (fluoroscopy time (FT) and number of cine runs) were collected. Examinations were selected per procedure (at least 20), according to the antero-posterior and lateral di

Image guidance is becoming standard of practice for cerebrospinal fluid sampling in children to improve success rates and avoid complications. This article discusses various imaging guidance techniques available in the pediatric age group. For neonates and infants, imaging guidance using ultrasound is the technique of choice, and for older children, fluoroscopy or even cone beam computed tomography can be used when anatomy is complex.

Radiodermatitis, predominantly over the right scapula, is a well-known complication of complex percutaneous coronary interventions (PCI). To reduce focal radiation exposure, we analyzed an inversed X-ray beam direction using a reversed C-arm position. On phantom experiment, we found that 130° right anterior oblique projection reduced skin dose over the right scapula by 98.2 % (P < 0.001) compared with conventional 50° left anterior oblique projection. A 73-year-old man with history of bypass surgery, multiple PCI and chronic radiodermatitis over the right scapula presented with recurrent chest

Budd-Chiari syndrome (BCS) is characterized by obstruction of the hepatic venous outflow tract. Therapeutic options for BCS are limited. We report a case of a 21-year-old woman with protein S and C deficiency with gross ascites. Treatment with transjugular intrahepatic portosystemic shunt (TIPS) was attempted, which revealed occluded hepatic veins, so transcaval TIPS was performed. No serious procedure-related complication occurred. After successful shunt creation, the patient's symptoms subsided and she was discharged and followed up for 6 months.

PURPOSE: To describe the percutaneous nephrostomy technique used at our centre in the treatment of supravesical urinary tract obstruction and to analyse the results obtained.
MATERIALS AND METHODS: Three-hundred and seventy-three patients underwent 412 percutaneous nephrostomies, most of which (78%) in an emergency setting, performed with a newly devised "mixed" technique. The procedure combines the positive elements of the two techniques employed to date in the management of upper urinary tract obstruction: the Seldinger angiographic technique and the Goodwin technique. Background This study aims to identify the reliability of kidney-ureter-bladder (KUB) radiography as a triage tool in acute ureteral colic (AUC). Moreover, this article correlates between KUB and non-contrast computerized tomography (NCCT) in view of stone characteristics and clinical outcomes. Methodology A retrospective cohort study recruited patients who had proven ureteric stones on NCCT. A blinded review of KUB and NCCT was performed to identify the following variables in both tests: site, ureteric stone maximum diameter, and stone density. Correlation between KUB radiography and NCCT has

An 84-year-old woman with pleural effusion developed anterior chest pain during thoracentesis, and post-procedural chest radiography revealed hydropneumothorax, initially suggesting iatrogenic pneumothorax. However, comparison of pre- and post-procedure imaging showed ipsilateral hemithoracic volume loss with tracheal deviation toward the procedure side, supporting pneumothorax ex vacuo, managed conservatively without intervention.

Hematopoietic stem cell transplantation (HSCT) is an increasingly available treatment option for patients with various oncologic, hematologic, and immunologic diseases. Although HSCT can be curative for some diseases, complications associated with this treatment limit its success and applicability. Gastrointestinal graft-versus-host disease (GVHD) and hepatic veno-occlusive disease are unique and deadly complications of HSCT. These diseases can mimic other HSCT complications, such as infection, hemorrhage, and hepatotoxicity with cholestasis, but GVHD and veno-occlusive disease require specifi

Pancreatic cancer often invades the duodenum and causes obstruction, but rarely causes massive duodenal bleeding. A 68-year-old male was admitted to our hospital because of vomiting. Enhanced abdominal CT showed a hypovascular tumor with air bubbles in the uncinate process of the pancreas. The tumor invaded the duodenum and metastasized to the liver and peritoneum. The main trunk of the superior mesenteric artery (SMA) was circumferentially involved. After admission, he had hematemesis and melena. Emergency gastroduodenoscopy revealed pulsating vessels in the third portion of the duodenum and

Variant anatomy of the superior thyroid artery is important during surgical procedures, interpretation of angiograms, and interventional radiography in the neck. Pattern of the variations shows population differences but there is no data from the Kenyan population. This study therefore investigated the variations in origin of the superior thyroid artery in a Kenyan population. Forty six necks (36 males and 10 females) from 46 cadavers of black Kenyans in Department of Human Anatomy University of Nairobi, Kenya were bilaterally dissected to expose the origin of the superior thyroid artery. Splenic injury is a rare but potentially life-threatening complication of colonoscopy procedures. We present the case of a 64-year-old female patient who developed an acute splenic laceration following an elective screening colonoscopy. The procedure was anatomically challenging due to colonic tortuosity but otherwise uneventful. While in the post-anesthesia care unit, the patient became hypotensive with abdominal distention and tenderness. Initial abdominal radiography was unrevealing for acute abnormalities. However, CT with intravenous contrast demonstrated hemoperitoneum secondary to a sma

Transcatheter pulmonary valve implantation (TPVI) is indicated for use in the management of failing pulmonary valves in humans. We report here the long-term follow-up of the first documented transcatheter pulmonary valve implanted in a client-owned dog. A one-year-old Beagle dog with severe congenital type A valvular pulmonic stenosis first underwent percutaneous balloon pulmonary valvuloplasty, leading two years later to severe pulmonary regurgitation. A TPVI using a Melody™ bioprosthetic valve was then successfully performed, with normalization of the right heart cavities. Repeated two- and

Rotator cuff calcific tendinopathy (RCCT) is a very common condition, characterized by calcium deposition over fibrocartilaginous metaplasia of tenocytes, mainly occurring in the supraspinatus tendon. RCCT has a typical imaging presentation: in most cases, calcific deposits appear as a dense opacity around the humeral head on conventional radiography, as hyperechoic foci with or without acoustic shadow at ultrasound and as a signal void at magnetic resonance imaging. However, radiologists have to keep in mind the possible unusual presentations of RCCT and the key imaging features to correctly

As use of imaging in the evaluation of pregnant and lactating patients continues to increase, misperceptions of radiation and safety risks have proliferated, which has led to often unwarranted concerns among patients and clinicians. When radiologic examinations are appropriately used, the benefits derived from the information gained usually outweigh the risks. This review describes appropriateness and safety issues, estimated doses for imaging examinations that use iodizing radiation (ie, radiography, computed tomography, nuclear scintigraphy, and fluoroscopically guided interventional radiolo
