A 38-year-old male with acute myocardial infraction who had received streptokinase presented with acute painless diminution of vision in the left eye. Examination revealed features of central retinal artery occlusion on the left side with vision of perception of light. Treatment in the form of systemic and local intraocular pressure lowering agents, retrobulbar xanthinol nicotinate and systemic injection of B-complex resulted in improvement of vision to counting fingers up to one meter. In this case thrombolytic therapy itself led to embolism into the left central retinal artery resulting in i…

PURPOSE: To report a case of probable non-uniting double retinal arterial circulation.
METHODS: Case report. A 66-year-old man suffered central retinal artery occlusion.
RESULTS: The medical findings and angiographic workup supported the existence of double central retinal circulation not uniting before entering the globe and independently branching in the retina.
CONCLUSIONS: In previously recognized unusual anatomic structures of the central retinal artery two parallel arteries were recognized, both of which united before entering the globe. We have described a rare anatomic variant in wh…

A 30-year-old woman noticed sudden visual loss in her right eye 3 days after a normal childbirth and without eclampsia during pregnancy. An ophthalmic examination revealed that she had impending central retinal artery occlusion and her right visual acuity was 20/2000, which recovered to 20/30. A systemic examination showed hypercoagulability and hyperlipidemia but no abnormal findings on brain CT scan and echocardiography. Clinical features of this case were very similar to Purtscher's retinopathy.

PURPOSE/METHODS: The blood flow in the posterior ciliary and retinal vascular circulations was studied by fluorescein angiography in a patient with a central retinal vein occlusion and cilioretinal infarction.
RESULTS/CONCLUSIONS: Fluorescein dye appeared in two cilioretinal arteries before the dye appeared in the retinal arterial circulation. This finding supports the concept of a relative hypoperfusion of the cilioretinal arteries secondary to increased retinal venous pressure.

We treated 14 consecutive patients for acute central retinal artery occlusion. Eleven were treated with urokinase introduced through a microcatheter in the proximal segment of the ophthalmic artery. Fibrinolysis was succeeded by heparinization for two to three days. Instead of urokinase, we used tissue plasminogen activator in three patients. Vision improved markedly in four of the 14 patients. Five others had slight improvement of visual acuity, visual field, or both. In five patients, no change occurred. Such a recovery of visual acuity may be anticipated if treatment begins within the first…

A case of bilateral cilio-retinal artery occlusion in a patient with classic migraine is described. It is unlikely that this hitherto unreported occurrence represents altered tone in the dense adrenergic innervation to the posterior ciliary circulation. We advise the continued use of beta-adrenergic blockers as prophylaxis in ocular migraine.

Central retinal artery occlusion (CRAO) is uncommon in those younger than 50. We present a 42-year-old man who complained of sudden loss of vision in the left eye. Fundus examination showed typical findings of CRAO. Extensive medical work up showed no evidence of diabetes or of cardiovascular, hematologic or collagen disease. However, a malignant tumor of the right testis was found, with metastases in the lung, retroperitoneal lymph nodes and brain. The right testis was resected and chemotherapy and brain irradiation resulted in remission during a follow-up of 2.5 years. The differential diagn…

Whereas most ocular complications following cerebral angiography are benign and transient, central retinal artery occlusion following cerebral angiography produces severe permanent visual deficits in nearly 40% of patients. Physicians ordering and performing cerebral angiography should be aware of this complication, since immediate attention to patients with visual disturbance from central retinal artery occlusion may save useful vision. All previously reported cases of central retinal artery occlusion following cerebral angiography have occurred after direct percutaneous carotid angiography. …

The clinical history of a man suffering from recurrent embolisms of the retinal artery is presented, extending over 11 years from the time when the patient was 57. The initial finding was a retinal artery embolism whose course was photographically recorded for an hour, until the emboli disappeared at the periphery. Three days and between three-and-a-half and six-and-a-half years later there were recurrences of the embolism. The patient's visual acuity dropped from 0.5 after the first, photographically recorded embolism, to 0.05-0.1 after the last. The gravity of the event is documented by the …

The third reported case of bilateral central retinal artery occlusion associated with polyarteritis nodosa is presented. An alternative etiology of the frequently observed association of polyarteritis nodosa with optic atrophy is suggested. A plan of therapy is recommended.

Purpose: to assess the tomographic retinal layers' thickness in eyes affected by branch retinal artery occlusion (BRAO) and to compare it to those of patients affected by primary open angle glaucoma (POAG). Methods: retrospective review of 27 patients; 16 with BRAO (16 eyes) and 11 with POAG (20 eyes) were identified among those who received SD-OCT scans, including analysis of macular retinal nerve fiber layer (mRNFL), ganglion cell layer (GCL), inner plexiform layer (IPL), inner nuclear layer (INL), neuroretinal rim (NRR), circumpapillary RNFL at 3.5 mm and hemisphere asymmetry (HA). Results:…

PURPOSE: To report the results of scanning laser polarimetry (NFA/GDx; Laser Diagnostic Technologies, San Diego, CA).
DESIGN: Prospective, consecutive observational case series.
PARTICIPANTS: Ten consecutive patients with central retinal artery occlusion (CRAO).
METHODS: Neuro-ophthalmic examination and scanning laser polarimetry of the retinal nerve fiber layer (RNFL) of 10 patients with CRAO.
MAIN OUTCOME MEASURES: Duration of visual loss, visual acuity, funduscopy, and scanning laser polarimetry of the RNFL in 10 eyes of 10 patients with CRAO.
RESULTS: The duration of visual loss befor…

BACKGROUND: Little is known about retinal vascular lesions underlying hemorrhage in the acute phase of branch retinal vein occlusion (BRVO). … eeding. However, in the early phase of indocyanine green (ICG) angiography, ICG dye leaked from the retinal artery at a point proximal to the first bifurcation. In the late phase of ICG angiography, the dye pooled along the retinal artery in a fusiform fashion. One year after laser photocoagulation was performed in the area of the BRVO, ICG dye leakage from the …

The authors report a patient who was observed to have a localized retinal detachment with combined central retinal artery and vein occlusion after cataract surgery performed with retrobulbar anesthesia. The authors propose that this condition resulted from injection of the anesthetic mixture into the optic nerve. No acute neurologic symptoms occurred, but visual loss was severe and permanent. This case adds to the previously reported spectrum of complications from retrobulbar anesthesia.
