Interesting findings relating to aspirin-induced asthma recently emerged. In this distinct clinical syndrome, aspirin and most other nonsteroidal anti-inflammatory drugs that inhibit cyclooxygenase precipitate rhinitis and asthma attacks. Aspirin-induced asthma affects 5-10% of adult asthmatics, but remains largely underdiagnosed. The natural history of aspirin-induced asthma has been described, based on an extensive pan-European survey. All over Europe the disease develops according to a pattern in a similar, characteristic way, which might suggest a common underlying principle. Aspirin provo…

Oral antihistamines generally represent the first line of treatment (after allergen avoidance) in mild seasonal allergic rhinitis (SAR), and in perennial allergic rhinitis (PAR) where symptoms are intermittent. They are safe, effective and easily administered. First-generation antihistamines experienced problems mainly with sedation and anticholinergic activity. Second-generation antihistamines are safer and have largely eliminated these effects, although a very small number of patients taking terfenadine or astemizole have developed a characteristic ventricular dysrhythmia, 'torsade de pointe…

Allergic rhinitis involves an early phase, largely mediated through mast cells, and a late phase which involves cellular infiltration and mediator release. In the early phase, mast cells release mediators as a result of antigen cross-linking adjacent immunoglobulin E molecules bound to mast cell surfaces. This results in an accumulation of histamine which gives rise to the characteristic symptoms of rhinitis--sneezing, itching, rhinorrhoea and congestion. The late phase of the allergic response (hours after challenge) involves infiltration of the nasal epithelium by eosinophils, basophils, mon…

Whenever an animal is examined because of chronic rhinitis, the dentition should be carefully examined to rule out the possibility of a primary dental problem. Oral examination under heavy sedation or general anesthesia is necessary to diagnose many of these cases. A periodontal probe is used in detecting deep periodontal pockets that extend into the nasal cavity or maxillary sinus. A dental explorer is used to reveal pulpal exposure secondary to dental fractures or rapid dental wear or attrition. Careful oral examination is used to localize any oronasal or oroantral fistula from a previously …

In the last few years, owing to new pathophysiological and pharmacological notions, the therapy of allergic rhinitis has achieved some remarkable goals. On the basis of personal experience and of the literature, a close examination of drugs used for aspecific treatment of allergic rhinitis is carried out, mainly regarding the most currently useful and effective.

A group of Angus X Holstein cattle were determined to have allergic rhinitis. Clinical signs included nasal discharge, tearing, sneezing, and nasal pruritus. The diagnosis was made on the basis of intradermal skin testing of affected and clinically normal cattle. The affected cattle had positive test results to various tree, grass, weed, and mold allergens. The clinical signs and seasonal occurrence were similar to those features for allergic rhinitis in man. Breeding data indicated an inherited mode of transmission, although only females were affected.

The ultrastructure of the respiratory nasal mucosa of patients suffering from vasomotor rhinitis was studied and compared with the reported ultrastructural findings in cases of chronic perennial allergic rhinitis. The principal ultrastructural differences were the absence of interendothelial gap junctions, the scarcity of eosinophils and plasma cells, and the absence of immunologically stimulated or degranulated mast cells in vasomotor rhinitis. These differences were discussed in the light of the known pathophysiologic facts.

Eosinophilic nonallergic rhinitis is a newly described symptom complex reported to occur in a significant percentage of adult patients who have symptoms of perennial rhinitis. They are characterized by nasal eosinophilia, negative allergy skin tests, and a normal serum immunoglobulin E concentration. Twelve children, aged 6 to 17 years, who represent the syndrome of eosinophilic nonallergic rhinitis, have been identified. All have severe perennial rhinitis and nasal eosinophilia. Physical examination revealed pale, boggy membranes with a clear nasal discharge in ten of 12 patients. All 12 pati…

In a study of 770 patients with allergic rhinitis, anamnestic data were collected by means of a questionnaire. 484 (63%) of the patients were women and 286 (37%) men. 45.5% of the patients had perennial symptoms, 33.5% had seasonal symptoms only, and 21% had predominantly seasonal but also some perennial symptoms. More than 50% of the patients belonged to the age group 21-40 years. The seasonal group included significantly more young patients than the perennial group. More than 50% of the parents and about 45% of the siblings had some atopic disorder. About 20% of the rhinitis patients had a c…

Noninfectious rhinitis and the clinical pharmacology of drugs used in its treatment, including specific treatment recommendations, are reviewed. Characterized by hyperreactivity of the nasal mucosa to a variety of stimuli, noninfectious rhinitis can be classified either as seasonal or perennial allergic rhinitis (when antigens can be identified) or as vasomotor or nonallergic rhinitis (when antigens are not identifiable). However, noninfectious rhinitis is probably better viewed as a continuum between these extremes rather than as definitive categories. Treatment measures include removal of of…

The clinical picture and the course of the acute necrotizing rhinitis in man is described in its different stages. Necrotizing rhinitis seems to be a significant factor in the etiology of some disturbances of the nasal respiratory function and of the ciliary activity of the nose, of atrophic rhinitis and of dry pharyngitis, in some cases of intranasai adhesions and of permanent mucosal defects of the septum. To our knowledge these defects have not been reported in the literature in the past, but are apparently not too rare.

Allergic rhinitis is a common disorder that regularly occurs in children and adolescents. The disease is associated with other allergic diseases, such as asthma, and it carries a heavy burden, with effects on sleep, school performance, and quality of life. Classic symptoms include sneezing, rhinorrhea, nasal obstruction, and nasal itching. When the eyes are involved, the term allergic rhinoconjunctivitis is used. Triggers may include airborne pollens, molds, dust mites, and animals. Skin or blood allergy testing can be a useful diagnostic modality that may guide therapy. Immunotherapy can prev…

INTRODUCTION: Allergic rhinitis is a prevalent condition warranting special aeromedical consideration due to its potential for acute and painful manifestations involving the middle ear or paranasal sinuses during rapid barometric pressure changes. Although second generation antihistamines and intranasal steroids are safe and effective treatments for this common condition, aeromedical management varies.
METHODS: An aeromedical policy review of 14 public access civil and military data repositories was undertaken. Policy within a convenience sample of nine countries (Australia, Canada, Croatia, …
