The impact of vaccination on rhinosinusitis and otitis media.

Routine childhood vaccination has affected frequency and bacteriology of acute otitis media (AOM) and acute bacterial rhinosinusitis (ABRS). Routine influenza vaccination moderately reduces AOM, and the Haemophilus influenzae type b vaccine likely had a minor role in AOM and ABRS. The conjugated pneumococcal vaccine has drastically reduced invasive pneumococcal disease and caused a moderate decrease in AOM and, likely, ABRS. The vaccine serotypes of Streptococcus pneumoniae have been all but eliminated, but other serotypes have emerged as potential causes of invasive disease. Antibiotic resist…

Chronic rhinosinusitis: an education and treatment model.

Research into chronic rhinosinusitis (CRS) has rapidly expanded over the last decade, resulting in a plethora of proposed etiologic and disease-modifying factors. Potentially, advancement of knowledge in this field has developed more than any other disease in otolaryngologic science. However, the teaching and education of this complex and still evolving process has lagged behind. Trainees, students, and residents may find the heterogeneous group of pathophysiologic mechanisms difficult to learn and apply to treatment decision making. Identification of the propagating factor and subsequent micr…

Immunopathology of chronic rhinosinusitis in young children.

OBJECTIVE: Previous investigation demonstrated predominantly lymphocytic inflammation in sinus mucosa of young children with chronic rhinosinusitis (CRS) rather than eosinophilic inflammation typical of adult CRS. Immunohistopathological study was undertaken to define further the cellular response in pediatric CRS. STUDY DESIGN: Maxillary mucosal biopsies from children and adults with CRS were stained for CD3 (T lymphocytes), CD4 (helper T lymphocytes), CD8 (cytotoxic T lymphocytes), CD20 (B lymphocytes), CD68 (monocytes/macrophages), CD56 (natural killer cells), kappa and lambda (plasma cell…

Inflammation in chronic rhinosinusitis and nasal polyposis.

Inflammation is a defining characteristic of chronic rhinosinusitis and nasal polyposis. Research had traditionally focused on the role of eosinophils in the pathogenic mechanisms, but recently more attention has been given to neutrophils and to different T-lymphocyte subtypes. This article summarizes current understanding, and discusses opportunities and potential pitfalls of inflammation-related research.

Analysis of the olfactory mucosa in chronic rhinosinusitis.

The impact of chronic rhinosinusitis (CRS) on the olfactory mucosa (OM) is dramatic. Cellular profiles and epithelial integrity in OM biopsies were evaluated using histological and immunohistochemical methods to define a strategy for future histological studies of CRS. We have examined nasal biopsies of 54 CRS patients (18-63 years old) and have defined specific histopathological patterns of the OM: normal pseudostratified, goblet cell hyperplasia, squamous metaplasia, and erosion. Goblet cell hyperplasia was most similar to a normal pseudostratified OM pattern but with goblet cells intermixed…

Diagnosis and management of chronic rhinosinusitis in adults.

Chronic rhinosinusitis (CRS) is characterized by mucosal inflammation affecting both the nasal cavity and paranasal sinuses; its causes are potentially numerous, disparate, and frequently overlapping. The more common conditions that are associated with CRS are perennial allergic and nonallergic rhinitis, nasal polyps, and anatomical mechanical obstruction (septum/turbinate issues). Other less common etiologies include inflammation (eg, from superantigens), fungal sinusitis or bacterial sinusitis with or without associated biofilm formation, gastroesophageal reflux, smoke and other environmenta…

Invasive fungal sinusitis and complications of rhinosinusitis.

Acute fulminant invasive fungal sinusitis in an immunocompromised host and bacterial rhinosinusitis with intracranial or orbital extension are challenging to manage and constitute true otolaryngic emergencies. In the absence of rapid diagnosis and treatment, these diseases can be fatal. In this article, the complications of bacterial rhinosinusitis are divided into three categories: local, orbital, and intracranial. They vary in their characteristics and severity according to their location, duration, and specific symptoms. This article reviews the pathophysiology, clinical presentation, diagn…

Role of matrix metalloproteinases in chronic rhinosinusitis.

PURPOSE OF REVIEW: In this review, we discuss the role of matrix metalloproteinases and the potential therapeutic inhibition of metalloproteinases in chronic rhinosinusitis. Metalloproteinases control tissue remodelling along with several other physiologic processes. Failures may cause extracellular matrix deposition and sustained inflammation, which are common features in chronic rhinosinusitis. RECENT FINDINGS: Metalloproteinases are rarely studied in chronic rhinosinusitis. Upregulation of certain metalloproteinases (gelatinases, collagenases and matrilysin) is described in the literature.…

A sheep model for the study of biofilms in rhinosinusitis.

BACKGROUND: Bacterial biofllms have been shown in chronic diseases such as cystic fibrosis, cholesteatoma, and otitis media with effusion. Recently, their detection on the mucosal tissue of sinusitis patients has implicated them in the pathogenesis of this condition. We present an animal model using sheep experimentally infected with Staphylococcus aureus to study the possible association between biofilm and sinusitis. METHODS: Twenty-four sheep underwent bilateral endoscopic sinus surgery to identify their frontal ostia. The frontal sinuses were treated in one of the following ways according

Upregulation of surfactant protein A in chronic rhinosinusitis.

OBJECTIVES: Surfactant protein A (SP-A) is protein that appears to play an important role in mammalian first-line host defense. However, the presence of SP-A in the human paranasal sinus mucosa is not well known. The purpose of this study was to investigate the expression of SP-A protein in human paranasal sinus mucosa and to compare the expression of SP-A mRNA between normal paranasal sinus mucosa and paranasal sinus mucosa with chronic rhinosinusitis. METHODS: Paranasal sinus mucosa samples from 10 patients who underwent surgery for chronic rhinosinusitis without polyps and 10 normal contro…

Management of refractory chronic rhinosinusitis in children.

OBJECTIVE: Pediatric chronic rhinosinusitis (CRS) continues to be a difficult problem for the medical community. Traditionally, oral antibiotic therapy has been the cornerstone of treatment, but some patients, who are resistant to multiple trials of oral antibiotics, require alternative therapeutic modalities. One such option, the use of intravenous (IV) antibiotics, has shown a high success rate in a limited study. Our goal is to estimate the success of a therapy consisting of culture directed IV antibiotics, adenoidectomy, and sinus aspiration in ameliorating long-term (>12 months) clinical …

What drives the inflammatory response in rhinosinusitis.

The dilemma of treating chronic sinusitis continues to confuse clinicians. When mucosal thickening does not respond to aggressive antibiotic therapy, other etiologies should be considered. Perhaps the most likely is the inflammatory response. In comparison with asthma, chronic sinusitis exhibits amazing similarities. Atopic dermatitis also exhibits similar inflammatory responses that lead to thickening of the skin in a manner not dissimilar to mucosal thickening. The interactions amid eosinophils, lymphocytes, leukotrienes, interleukins, and epithelial cells serve as a reminder that there is o

Hashimoto's thyroiditis: a rare cause for rhinosinusitis.

Rhinosinusitis is a common disorder with various etiological factors. In our clinical practice allergy seems to be a predominant cause followed by other less common causes like infection, drugs and aspirin hypersensitivity. We present a case of chronic rhinosinusitis secondary to hypothyroidism. Although hormonal causes like hypothyroidism have been proven to cause nasal congestion and rhinosinusitis, this is the first reported case of chronic rhinosinusitis in Hashimoto's thyroiditis in our center. This is a diagnosis of exclusion for chronic rhinosinusitis not responding to optimal medical t…

The new histologic classification of chronic rhinosinusitis.

Two histologic patterns of disease are found in chronic rhinosinusitis. The first is dominated by eosinophilia and polypoid changes. Glandular hyperplasia and hypertrophy characterize the second. We present the evidence supporting the existence of these two patterns of disease and link these histologic patterns to the larger pathophysiologic processes that drive them. This histologic classification should be acknowledged both in the clinical setting and in laboratory research of chronic rhinosinusitis.

The intracranial complications of rhinosinusitis: can they be prevented?

OBJECTIVES/HYPOTHESIS: Reference textbooks on the intracranial complications of rhinosinusitis imply that many of the intracranial complications of rhinosinusitis can be prevented. We sought to examine whether or not this is true. STUDY DESIGN: A retrospective case series. METHODS: The study included 47 consecutive patients presenting with intracranial complications secondary to rhinosinusitis between 1992 to 1999 with a mean follow-up of 5 years and 1 month. RESULTS: The most common presenting symptoms of intracranial involvement were an altered mental state, headache, fever, seizure, vomi…

Peripheral eosinophilia in the diagnosis of chronic rhinosinusitis.

PURPOSE: To determine the relationship between peripheral blood eosinophilia and chronic rhinosinusitis. MATERIALS AND METHODS: A retrospective review was conducted of consecutive operative cases during 1 calendar year. The preoperative complete blood count (CBC) were tabulated for three groups of patients: those undergoing endoscopic sinus surgery, those undergoing septoplasty with turbinate reduction alone, and a nonrhinologic control group. Statistical analysis was performed to determine differences in the components of the CBC among these three groups of patients and to identify significa…

Chronic rhinosinusitis: is the nose really involved?

To determine whether chronic rhinitis usually accompanies chronic sinusitis, and critically examine the concept of chronic rhinosinusitis, paired specimens of nasal septal mucosa and ethmoid sinus mucosa were obtained in a prospective cohort of 42 patients undergoing endoscopic sinus surgery for chronic rhinosinusitis. The histopathological degree of inflammation for each specimen was scored using a five-point rating scale. Cell counts for eosinophils and inflammatory cells per high-power field (HPF) were determined. Mean patient age was 40.5 years and mean Lund score was 9.3. The inflammation…

[The role of Inflasinusans in the treatment of acute rhinosinusitis].

UNLABELLED: The urgency of the problem of acute rhinosinusitis is determined by the scale of its prevalence, the tendency to prolonged course and chronization, and its active role in the formation of severe complications, which necessitates the search for new methods of diagnosis and treatment of this pathology. Assessment of the sense of smell in patients with acute rhinosinusitis is an important diagnostic criterion that allows assessing the severity of inflammation and the effectiveness of therapy. OBJECTIVE: To improve the results of treatment of acute rhinosinusitis through the use of In…
