Purified Exosome Product Enhances Tendon-Bone Healing in a Rat Rotator Cuff Repair Model.

BACKGROUND: Tendon-bone healing after rotator cuff injury is, or remains, a clinical challenge. PURPOSE: To examine the role of intraoperative use of purified exosome products (PEPs) in the treatment of rotator cuff injuries and to assess the effect of PEP on supraspinatus tendon-bone healing after rotator cuff repair in a rat model. STUDY DESIGN: Controlled laboratory study. METHODS: This study used 120 Sprague-Dawley rats that underwent an acute supraspinatus tendon transection from the insertion, followed by repair using a single transosseous suture. They were divided into 3 groups: a co…

Rat rotator cuff tendon-to-bone healing properties are adversely affected by hypercholesterolemia.

BACKGROUND: Rotator cuff tendon tears represent a major component of reported orthopaedic injuries. In addition, more than one quarter of U.S. adults either currently have high cholesterol levels or have reduced their previously high cholesterol levels through the use of pharmaceuticals. Our clinical data have already linked hypercholesterolemia to full-thickness rotator cuff tears, and experimental data from our laboratory have shown effects on native tendon properties in multiple species. The objective of this study was to evaluate healing of supraspinatus tendons in our rat rotator cuff inj

[Paraclinical diagnostic procedures in micro- and macrotraumas of the shoulder. Indications for echography and CT scanning].

A careful physical examination is absolutely necessary as a first evaluation of all disorders of the shoulder before performing any imaging technique that must be chosen according to the kind of suspected pathology. The best indications for performing an arthroscanner are the intraarticular lesions such as injuries of the glenoid labrum and of the articular capsule. Echography is the most performing technique for bicipital and rotator cuff tendinitis. CT scan helps to understand the antero-internal retro-coracoidal impingment syndromes as well as uninterpretable bone lesions at standard radiog

Editorial Commentary: May the Force Be With You-Preoperative Strength Is the Rotator Cuff Retear Predictor We Can Actually Train.

Predicting the clinical course of rotator cuff repairs is one of the most significant and unresolved challenges in shoulder surgery. Most established predictors of retear (e.g., advanced age, fatty infiltration, large tear size, and tendon retraction) are nonmodifiable-already set prior to the time the patient arrives in the operating room. Preoperative shoulder strength, measured as a side-to-side deficit on handheld dynamometry, warrants a larger role in discussions of rotator cuff repair outcomes. It is a practical metric (inexpensive, fast, and clinically accessible), and importantly, ther

Application of musculoskeletal ultrasound in postoperative rehabilitation assessment and monitoring after rotator cuff repair: A systematic review.

BACKGROUND: The development of rehabilitation protocols after rotator cuff repair has long lacked objective benchmarks. Traditional time‑based regimens are limited by considerable inter‑individual variability and an increased risk of re‑tear. Musculoskeletal ultrasound allows dynamic assessment of tendon healing and muscle morphology, yet evidence for directly linking its use to rehabilitation decisions remains scarce. OBJECTIVE: To systematically synthesize the evidence on the use of musculoskeletal ultrasound monitoring to inform rehabilitation decision‑making after rotator cuff repair. ME

Proteomic trajectories in human rotator cuff degeneration: a systematic review of immunohistochemical studies.

BACKGROUND: Rotator cuff (RC) repairs heal unreliably, particularly in chronic disease. The molecular mechanisms underlying failed repair remain poorly understood. While individual studies have examined protein expression in diseased RC tissue, no synthesis has investigated these findings across various disease stages. This systematic review explores the stage-specific protein expression of human RC degeneration using immunohistochemistry. METHODS: Following PRISMA guidelines, MEDLINE, Embase, and Cochrane Library were systematically searched to September 2025 for human studies that use immun

Double Pulley-Triple Row Technique for Arthroscopic Rotator Cuff Repair: A Technical Note.

Arthroscopic rotator cuff repair is the gold standard for treatment, but current techniques have shortcomings, especially for larger tears. Single-row repairs often fail to fully restore the footprint, leading to high retear rates. Although triple-row and double-row repairs show promise, concerns regarding retear persist. Biomechanical studies favor triple-row repair for better coverage and pressure distribution. Techniques such as linked double-row and double-pulley methods enhance strength. Secondary cuff failures near the musculotendinous junction are commonly caused by stress concentration

Editorial Commentary: The Subacromial Balloon Spacer for Massive Irreparable Rotator Cuff Tear Demonstrates Cost-Effectiveness in Some Settings.

The InSpace subacromial balloon spacer is indicated for the treatment of massive irreparable rotator cuff tears. The device is placed in the subacromial space with the aim of restoring shoulder function by limiting painful acromiohumeral contact and recentering the superiorly migrated humeral head. However, controversy exists because 2 randomized controlled trials have produced conflicting findings with regards to efficacy. These findings, along with narrow indications, have led to low use of the balloon among European and Latin American surgeons. The controversy generated by conflicting rando

Autograft Long Head Biceps Tendon Can Be Used as a Scaffold for Biologically Augmenting Rotator Cuff Repairs.

PURPOSE: We create a viable, mechanically expanded autograft long head biceps tendon (LHBT) scaffold for biologically augmenting the repair of torn rotator cuffs. METHODS: The proximal aspect of the tenotomized LHBTs was harvested from patients during rotator cuff repair surgery and was mechanically formed into porous scaffolds using a surgical graft expander. LHBT scaffolds were evaluated for change in area, tensile properties, and tenocyte viability before and after expansion. The ability of endogenous tenocytes derived from the LHBT scaffold to promote tenogenic differentiation of human ad

Editorial Commentary: Is Arthroscopic In Situ Repair Effective for Long-Term Functional Recovery and Pain Relief in Symptomatic Partial Rotator Cuff Tears?

Currently, although most evidence suggests that surgery is effective in treating symptomatic partial-thickness rotator cuff tears in patients with failure of nonoperative management and with a tear of more than 50% of the tendon thickness, there is little consensus on the best method of repair. Some surgeons would advocate completing the tear and repairing it, whereas others would advocate performing in situ repair. In our opinion, it is important to also consider treating the long head of the biceps tendon, which is frequently a source of pain at the time of or after surgery.

Editorial Commentary: The Partial Thickness Rotator Cuff Tear: Is Acromioplasty Without Repair Ever Indicated?

I believe that arthroscopic repair is the treatment of choice for patients with partial thickness rotator cuff tears when nonoperative methods have been exhausted. Excluding overhead athletes and patients in whom long head biceps tendon pathology is the primary concern, I do not believe that a significant role exists for debridement with or without acromioplasty in the majority of patients with partial thickness tears. Regarding the repair technique, I prefer in situ repair for bursal-sided tears because the superior capsule is intact and completion of the tear with repair for articular-sided

Suture-bridge subscapularis tendon repair technique using low anterior portals.

A suture-bridge technique has been introduced to facilitate fixation procedures and to achieve increased holding strength in posterosuperior rotator cuff. Based on biomechanical studies, this technique has been suggested as an effective method that could optimize rotator cuff tendon-footprint contact area and mean pressure, as well as holding strength. In this technique, the suture-bridge creation is adapted for arthroscopic subscapularis repair to attain the ideal cuff integrity and footprint restoration. To obtain enough working portals and space, two accessory portals were made on the anter

Reverse passage of the suture lasso in arthroscopic rotator cuff repair.

Suture passage in arthroscopic rotator cuff repair can be technically difficult. The suture lasso is typically passed antegrade from the bursal side of the rotator cuff. Antegrade passage of the suture lasso can be particularly difficult when visualization is limited. Reverse passage of the suture lasso from the undersurface can be used to place sutures in technically challenging circumstances. The suture lasso is placed retrograde through the undersurface of the rotator cuff and used as a suture shuttle to bring sutures back through the rotator cuff. This technique is easily reproducible and

Reverse rotator interval closure.

We describe a technique for reducing capsular volume arthroscopically by shifting the anterior inferior glenohumeral ligament (AIGHL) and capsule up to the top of the subscapularis. This procedure is performed when laxity exists in the absence of a Bankart lesion. The AIGHL is first released from the capsule. This allows sutures to be placed through the capsule inferiorly so that it can be shifted up superiorly during the repair. The AIGHL and capsule are then released from the underlying subscapularis. Sutures are then passed through the capsule and out of the accessory anterior portal, progr

Editorial Commentary: Rotator Cuff Repairs Fail at an Alarmingly High Rate During Long-Term Follow-Up: Graft Augmentation and Biologics May Improve Future Outcomes.

Rotator cuff repairs (RCR) frequently fail to heal, particularly those with advanced fatty infiltration, supraspinatus and infraspinatus atrophy, narrowed acromiohumeral distance, and large-to-massive tear size. Unfortunately, the longer the follow up, the more sobering the statistics, with some reported retear rates ranging up to 94%. Importantly, recent long-term radiographic assessments after primary RCR reveal direct correlations between failure and patient-reported outcomes, functional deterioration, and ultimately, progression of glenohumeral arthritis and/or frank cuff tear arthropathy.

Chondrogenic differentiation and lubricin expression of caprine infraspinatus tendon cells.

Reparative strategies for the treatment of injuries to tendons, including those of the rotator cuff of the shoulder, need to address the formation of the cartilage which serves as the attachment apparatus to bone and which forms at regions undergoing compressive loading. Moreover, recent work indicates that cells employed for rotator cuff repair may need to synthesize a lubricating glycoprotein, lubricin, which has recently been found to play a role in tendon tribology. The objective of the present study was to investigate the chondrogenic differentiation and lubricin expression of caprine inf
