Magnetic resonance imaging (MRI) provides the most detailed evaluation of the sacroiliac (SI) joint and surrounding soft tissue. Therefore, this technique represents the most sensitive and specific method in early diagnosis of infectious sacroiliitis. Among three patients diagnosed as having infectious sacroiliitis in our hospital, the short tau inversion recovery sequence (STIR) was found to be more effective than the T1 contrast-enhanced sequence, particularly in delineating all findings of the SI joint inflammation and allowing for the early detection of septic sacroiliitis.

AIM: To determine which signs are the most accurate in the diagnosis of sacroiliitis with MRI.
MATERIAL AND METHODS: 40 consecutive patients with inflammatory low back pain underwent MRI at 1.5 T with FSE T2 and SE T1 weighted-images before and after Gadolinium-DOTA injection. 22 patients were suffering from spondylarthropathy while the other 18 patients constituted the control group. Each examination was interpreted by two independent observers who analysed 11 different signs.
RESULTS: Intra and inter observer reproducibility were high (respectively 76% and 70%). Inter observer reproducibil…

PURPOSE OF THE STUDY: Sacroiliitis in children is a rare disease. Since 1878, only 100 cases have been reported in the international literature.
MATERIALS AND METHODS: We reviewed a series of 11 cases (mean age of 7.75 years) with a delay of 45 months. All the patients had limp, and hip pain.
RESULTS: The clinical examination found each time F.A.B.E.R.E. and Glaensen signs. The hip mobility was limited most of time. There was a non specific inflammatory syndrome. Roentgenogram signs were delayed, and associated widening of the joint, geodes, condensation, and at an ultimate stage, fusion of …

Rickets can manifest with a wide variety of rheumatic symptoms. In this paper, a fifteen year old female patient with hypophosphatemic rickets presenting with symptoms suggesting sacroiliitis at disease onset is reported. The sacroiliac joint involvement in this case was attributed to the subchondral bone fractures due to the secondary hyperparathyroidism. Her symptoms resolved completely after treatment with calcitriol and phosphate solution for hypophosphatemic rickets.

Pyogenic sacroiliitis is a relatively rare condition in children. Vigilance and careful clinical examination of the joint could minimize the delay in diagnosis. In areas endemic for brucellosis, negative serology, raised erythrocyte sedimentation rate, positive magnetic resonance imaging, isotope bone scan and bacteriological culture confirm diagnosis. Appropriate adequate antibiotic therapy for four to six weeks cures the infection and prevents morbidity.

We report the occurrence of bilateral sacroiliitis in 2 cases of biopsy-proven sarcoidosis. Tuberculosis was excluded by tuberculin testing and bronchoalveolar lavage in both cases. In the literature, 5 cases of sacroiliitis and sarcoidosis have thus far been described, but in all tuberculosis was not excluded (tuberculin testing was not performed or revealed a positive test result). In 1 of these cases tuberculosis was even simultaneously suspected to be present. In all previous cases, antituberculous or other antimicrobial agents were given. Previously reported cases of sacroiliitis in sarco…

Rheumatological manifestations may develop as paraneoplastic syndromes in patients with malignancy. Sacroiliitis and spondyloarthropathy have not, however, been previously associated with cancer. The case is described of a patient with a stage IV diffuse well differentiated lymphocytic lymphoma who developed concomitant sacroiliitis and enthesopathies with rapid progression following the diagnosis of malignancy.

We report the unique occurrence of a unilateral sacroiliitis in a patient with active sarcoidosis accompanied by pulmonary tuberculosis. Convincing (clinical) evidence of sarcoidosis as the extremely rare cause of this articular involvement is presented. Discussion is focused on comparison of sarcoidosis and tuberculosis, particularly with respect to their articular involvement, and the literature of previously reported cases of sarcoid sacroiliitis is briefly reviewed.

Six patients (4 male and 2 female) presenting with an original, predominantly osseous disease are reported. The disease is characterized by a chronic inflammatory process affecting the thorax, the spine, the femur in 1 case and in all patients only one of the sacro-iliac joints. The condition had begun 11 to 28 years previously and had been active for 6 to 24 years. One case started with regressive polyarthritis. There was no visceral involvement. Radiograms showed bone condensation with hyperostosis, sometimes hypertrophic, unrelated to any articular space. Depending on each patient, the lesi

To evaluate clinical usefulness of quantitative sacroiliac scintigraphy (QSS) in detecting sacroiliitis, we used a modified, pixel by pixel technique for calculating sacroiliac joint/sacrum uptake ratios (sacroiliac joint index - SII). We studied 90 controls, 18 selected patients with active sacroiliitis, 2 ankylosing spondylitis patients with completely ankylosed sacroiliac joints, 14 patients with nonspecific low back pain and 5 patients with rheumatoid arthritis. In the controls, we found that the SII decreases with increasing age (P less than 0.001) and is higher in males than in females (…

Fifty-seven women who were treated for severe acute salpingitis between June, 1974, and June, 1976, were examined in 1978, 24 to 48 months after their first hospitalization. At the same time, 31 healthy women matched for age and parity were examined. All patients and control subjects were examined independently by a gynecologist and a rheumatologist. Forty-two of the patients and all control subjects were examined by quantitative 99mTc-pertechnetate scanning of the sacroiliac joints. These same joints also were examined by x-ray in the 57 patients. Sacroiliitis was documented in 39 of the 57 s…

The presence of sacroiliitis was studied in 100 consecutive psoriatic patients, 62 males and 38 females. In this selected population 14 patients (14%) had grade 2 to 4 sacroiliitis, in seven associated with clinical findings of ankylosing spondylitis (AS). Sacroiliac joint involvement was significantly related to more severe cutaneous and ungual psoriatic disease. Sacroiliitis was observed in nine males and five females but clinical AS predominated in the males, six males and one female. No difference in distribution was observed by sex, onset age, or duration of psoriasis when patients with a…

MRI is used for early detection of inflammation of sacroiliac joints as it shows active lesions of sacroiliitis long before radiographs show damage to the sacroiliac joints. Early diagnosis of arthritis allows early treatment of inflammation and can help delay disease progression and prevent irreversible damage. Also, early identification of axial involvement in juvenile spondyloarthropathy is crucial, as treatment options are different than for peripheral juvenile spondyloarthropathy. In general, standard sequences used in adults are also applied to children. However, interpreting MR images o…

We report for the first time a case of atypical unilateral sacroiliitis secondary to mechanical stress injury. Unilateral sacroiliitis can be caused by a variety of etiologies. The first diagnosis to rule out is infection since it requires urgent treatment to avoid its serious consequences. Spondyloarthritis can be manifested by unilateral sacroiliitis in its early stage. Sacral fractures should always be looked for on the imaging modalities performed. In our case, no signs of infection or systemic disease were found. No fracture was seen on the imaging examinations and we had negative culture…
