Bilateral changes in somatosensory sensibility after unilateral below-knee amputation.

OBJECTIVE: To evaluate possible alteration in proprioceptive and cutaneous sensibility in the nonamputated leg of unilateral transtibial amp … on and touch-pressure perception at the knee and foot levels. RESULTS: In the traumatic group, the sensory thresholds of the nonamputated leg were significantly higher than the control values in the 2 modalities tested. The movement detection was reduced at the knee and ankle levels, whereas a decrease in touch-pressure sensibility was observed only at the plantar …

Artemin induced functional recovery and reinnervation after partial nerve injury.

Systemic artemin promotes regeneration of dorsal roots to the spinal cord after crush injury. However, it is unclear whether systemic artemin can also promote peripheral nerve regeneration, and functional recovery after partial lesions distal to the dorsal root ganglion (DRG) remains unknown. In the present investigation, male Sprague Dawley rats received axotomy, ligation, or crush of the L5 spinal nerve or sham surgery. Starting the day of injury, animals received intermittent subcutaneous artemin or vehicle across 2weeks. Sensory thresholds to tactile or thermal stimuli were monitored for 6…

Pain measurement with evoked potentials: combination of subjective ratings, randomized intensities, and long interstimulus intervals produces a P300-like confound.

Evoked potentials in response to painful stimuli have been studied as objective measures of pain. Bromm has advocated experimental conditions in which, (1) stimulus intensities are randomized, and (2) subjects rate each stimulus. However, a cognitive, i.e. information processing, 'late positive component' (LPC), e.g. the P300, may be elicited by these same conditions, whether or not the stimuli are painful. The LPC may overlap, and interfere with the measurement of, responses that are only seen with painful stimuli. We compared the LPC in two experimental protocols using ten subjects and elect

Somatosensory processing in a German family with PINK1 mutations: its potential role in Parkinson disease.

BACKGROUND: It is unclear whether sensory symptoms in Parkinson disease (PD) are of primary or of secondary origin attributable to motor symptoms such as rigidity and bradykinesia. OBJECTIVE: The aim of this study was to elucidate whether sensory abnormalities are present and may precede motor symptoms in familial parkinsonism by characterizing sensory function in symptomatic and asymptomatic PINK1 mutation carriers. METHODS: Fourteen family members with PINK1 mutation and 14 healthy controls were examined clinically, with nerve conduction studies and quantitative sensory testing (QST). RES

Psychophysical sensory examination in individuals with a history of methylmercury exposure.

Paresthesias are the first symptom that people report following toxic doses of methylmercury. The authors conducted a psychophysical study of tactile sensation to evaluate the somatosensory abilities of subjects living in a methylmercury-polluted area around Minamata City, Japan. The authors examined control subjects and methylmercury-exposed subjects with and without numbness. A history of methylmercury exposure was taken and a neurological examination performed. Aluminum-oxide abrasive papers were used as stimuli in a psychophysical sensory examination of fine-surface-texture discrimination.

Thenar eminence quantitative sensory testing in the diagnosis of proximal median nerve compression.

Diagnosis of proximal median nerve compression (PMNC) remains a clinical challenge. The authors hypothesized that measurement of the sensibility of the thenar eminence might identify PMNC by demonstrating abnormal function in the palmar cutaneous branch of the median nerve. This hypothesis was evaluated by means of quantitative sensory testing of the thenar eminence in 33 healthy volunteers, 14 patients with carpal tunnel syndrome, and 35 patients with PMNC. The cutaneous pressure thresholds for one-point static touch (1PS) and two-point static touch (2PS) were measured with the Pressure-speci

Effect of anodal high-definition transcranial direct current stimulation on the pain sensitivity in a healthy population: a double-blind, sham-controlled study.

High-definition transcranial direct current stimulation (HD-tDCS) of brain areas related to pain processing may provide analgesic effects ev … erall, the active stimulation protocols were not able to induce significant modulation of the somatosensory thresholds in this healthy population compared with sham-tDCS. Unrelated to the HD-tDCS protocol, a decreased sensitivity was found after the first intervention, indicating a placebo effect or possible habituation to the quantitative sensory testing assessment …

Responses to gastric distension in functional dyspepsia.

BACKGROUND: Studies have shown that an altered visceral perception threshold plays a role in the pathogenesis of upper gastrointestinal tract symptoms in dyspeptic patients. However, it is not clear whether the compliance and adaptive relaxation of the proximal stomach contribute to the symptoms. AIMS: To investigate whether abnormal relaxation or adaptation of the proximal stomach during the interdigestive state and the postprandial phase could explain the symptoms of functional dyspepsia. SUBJECTS: Twelve volunteers and 12 patients with dysmotility-like functional dyspepsia were included i

Oesophageal sensation assessed by electrical stimuli and brain evoked potentials--a new model for visceral nociception.

Sensory thresholds and brain evoked potentials were determined in 12 healthy volunteers using electrical stimulation of the oesophagus 28 and 38 cm from the nares. The peaks of the evoked potentials were designated N for negative deflections and P for positive. Continuous electrical stimulation (40 Hz) at the 38 cm position resembled heartburn (five of 12 subjects) while non-specific ('electrical') sensations were provoked at 28 cm (10 of 12). Thresholds of sensation and of pain were lower at the initial than the second determination, but did not differ with respect to stimulation site. The pa…

[Experience with electrostimulation of the acoustic nerve before cochlea implantation].

The electrical stimulation of the acoustic nerve plays an essential role in the selection of patients for cochlea implantation. The performance of a deaf ear under the conditions of electrical stimulation can be assessed by means of the pre-operative promontory test using a transtympanic needle electrode. Five components of this test were applied to 48 patients: the determination of perception threshold and upper comfortable level for pulse and burst stimulation, the pitch test, the temporal difference test and the gap detection test. The mean values emerging from these examinations may be use

Depth-dependent noise interference reveals scene-structure constraints on contour completion.

Contour completion allows perception of whole objects despite fragmented input, yet the mechanisms that support this process remain debated. Purely local, bottom-up models posit that oriented edge signals propagate across gaps according to two-dimensional (2-D) principles of good continuation, predicting that interference from luminance noise should depend only on local image geometry. In contrast, scene-based frameworks - including accounts that incorporate 3-D surface layout - propose that completion depends on a global analysis of depth, occlusion, and surface assignment, predicting stronge

Making Corneal Sensation Testing Accessible: Bench Validation of a Low-Cost Adjustable Monofilament Aesthesiometer.

PURPOSE: Corneal sensation testing is essential for diagnosing and monitoring neurotrophic keratopathy, yet the gold-standard Cochet-Bonnet (COBO) aesthesiometer (>£1000) is unavailable in most ophthalmology departments. The cotton wisp alternative is qualitative and insensitive to early sensory loss. This study evaluated whether a generic adjustable monofilament corneal aesthesiometer (GAMCA) fabricated from commodity materials could replicate COBO performance. METHODS: The GAMCA comprised a 0.12 mm nylon monofilament of adjustable length on a millimetre-scaled sliding carrier, constructed f

Efficient Spatial Estimation of Perceptual Thresholds for Retinal Implants via Gaussian Process Regression.

Retinal prostheses restore vision by electrically stimulating surviving neurons, but calibrating perceptual thresholds (i.e., the minimum stimulus intensity required for perception) remains a time-intensive challenge, especially for high-electrode-count devices. Since neighboring electrodes exhibit spatial correlations, we propose a Gaussian Process Regression (GPR) framework to predict thresholds at unsampled locations while leveraging uncertainty estimates to guide adaptive sampling. Using perceptual threshold data from four Argus II users, we show that GPR with a Matérn kernel provides more

Factors influencing olfactory function in an adult general population sample: the CHRIS study.

The sense of smell allows for the assessment of the chemical composition of volatiles in our environment. Different factors are associated with reduced olfactory function, including age, sex, as well as health and lifestyle conditions. However, most studies that aimed at identifying the variables that drive olfactory function in the population suffered from methodological weaknesses in study designs and participant selection, such as the inclusion of convenience sample or only of certain age groups, or recruitment biases. We aimed to overcome these issues by investigating the Cooperative Healt

Screening for the loss of protective sensation in people without a history of diabetic foot ulceration: Validation of two simple tests in India.

The ability of the Ipswich touch test (IpTT) and VibratipTM to detect loss of protective sensation (LOPS) was tested against a neurothesiometer in an outpatient diabetic population without a history for ulceration. Our results support the use of the IpTT as a screening tool for LOPS, but not of VibratipTM.

Test-retest and time dependent variation and diagnostic values of vibratory sensation determined by biothesiometer and the Rydel-Seiffer tuning fork.

BACKGROUND AND AIMS: Polyneuropathy is a common neurological disorder with many potential causes. An essential part in screening, diagnosis, and follow-up evaluation of polyneuropathy is testing of the sensory function including vibratory sensation. The graduated Rydel-Seiffer tuning fork and the biothesiometer have been developed to quantify vibratory sensation through detection thresholds. The aim of this study is to compare the vibration detection thresholds determined by the two instruments regarding intraindividual temporal changes, interindividual variation in healthy subjects and compar

Responsiveness of quantitative sensory testing-derived sensory phenotype to disease-modifying intervention in patients with entrapment neuropathy: a longitudinal study.

The German Research Network on Neuropathic Pain (DFNS) quantitative sensory testing (QST) method for sensory phenotyping is used to stratify patients by mechanism-associated sensory phenotype, theorised to be predictive of intervention efficacy. We hypothesised that change in pain and sensory dysfunction would relate to change in sensory phenotype. We investigated the responsiveness of sensory phenotype to surgery in patients with an entrapment neuropathy. With ethical approval and consent, this observational study recruited patients with neurophysiologically confirmed carpal tunnel syndrome.
