A free bone block operation for recurrent anterior dislocation of the shoulder joint.

An operation to prevent recurrent anterior dislocation of the shoulder by insertion of a free iliac bone graft into an anterior glenoid osteotomy is described and has been performed on 16 shoulders in 15 patients. No additional fixation of the graft was used and the divided subscapularis tendon was not sutured. The average follow-up was 7 years and 8 months, no serious complications occurred, there were no recurrences, there was no evidence of degenerative osteoarthrosis and only 4 patients had any detectable loss of power. Only 2 patients had noticed any restriction of movement. There was som

Shoulder joint arthroplasty in young patients: Analysis of 8742 patients from the Australian Orthopaedic Association National Joint Replacement Registry.

BACKGROUND: Shoulder replacement is a reliable treatment for the relief of pain and improvement of function in patients with glenohumeral arthritis, rotator cuff arthropathy, osteonecrosis and fracture. Limited data is available comparing revision rates for the different types of shoulder replacement when used in younger patients. This study aims to compare the survivorship of hemi resurfacing, stemmed hemiarthroplasty, total shoulder arthroplasty and reverse total shoulder arthroplasty in younger patients using data from a large national arthroplasty registry. METHODS: Data from the Australi

Simultaneous involvement of pigmented villonodular synovitis in the left shoulder glenohumeral joint and ankle joint: a rare case report.

BACKGROUND: Pigmented Villonodular Synovitis, although uncommon, can lead to significant joint destruction if not diagnosed and treated early. Concurrent polyarticular presentation in adults is exceedingly rare, appearing in less than 1% of cases. CASE PRESENTATION: This case report presents a unique and rare occurrence of Pigmented Villonodular Synovitis involving both the ankle and shoulder joints simultaneously in an adult patient. The patient initially developed a diffuse form Pigmented Villonodular Synovitis in the left ankle, which was managed with total synovectomy at another tertiary …

The colonisation of the glenohumeral joint by Propionibacterium acnes is not associated with frozen shoulder but is more likely to occur after an injection into the joint.

AIMS: Our aim was to investigate the prevalence of Propionibacterium (P.) acnes in the subcutaneous fat and capsule of patients undergoing shoulder surgery for frozen shoulder or instability. PATIENTS AND METHODS: A total of 46 patients undergoing either an arthroscopic capsular release or stabilisation had biopsies taken from the subcutaneous fat and capsule of the shoulder at the time of surgery. These samples were sent for culture in enrichment, and also for Nucleic Acid Amplification testing. The prevalence of P. acnes and other microbes was recorded. Fisher's exact test of binary variabl

Perioperative shingles diagnosis does not influence prosthetic joint infection rates after shoulder arthroplasty.

PURPOSE: Shoulder arthroplasty (SA) utilization has increased substantially, with a corresponding rise in revision procedures, many driven by periprosthetic joint infection (PJI). Herpes zoster (HZ), or shingles, is increasingly prevalent among older and immunocompromised patients and shares overlapping risk factors with PJI. However, the association between shingles and postoperative complications following SA is unknown. This study evaluated the relationship between peri-operative shingles and postoperative complications after SA. METHODS: Patient-level claims from adults ≥ 18 years undergo

Assessment of shoulder joint and muscle characteristics side-asymmetry in professional padel players.

Padel is a worldwide racket sport that is growing each year. The unilateral upper limb repetitive movements of this sport may lead to shoulder structural and functional adaptations, which may alter muscle mechanics, however evidence is limited. We aimed to investigate if there is morphological, mechanical and functional shoulder symmetry between upper limbs of professional padel players. Fourthteen (7♂ and 7♀) professional Brazilian padel players were evaluated at supraspinatus muscle thickness (MT), stiffness, shoulder rotation range of motion (ROM), shoulder internal and external rotation pe

Relationship between SLAP Lesions and Shoulder Joint Capsule Thickness: An MR Arthrographic Study.

Background and Objectives: This study aimed to evaluate the relationship between SLAP lesions and the shoulder joint capsule thickness via MR arthrography. Understanding the relationship between SLAP lesions and the joint capsule thickness is important because an increased capsule thickness may indicate chronic inflammation and contribute to persistent pain and dysfunction. These findings have significant clinical implications for the diagnosis, management, and treatment strategies of shoulder joint pathologies. Materials and Methods: We retrospectively analyzed the MR arthrography results of …

Septic arthritis of shoulder joint following a COVID-19 vaccination: A case report.

INTRODUCTION: Septic arthritis of the shoulder joint is a serious condition which requires early diagnosis and treatment. This condition usually presents with shoulder pain, limited range of motion and/or fever, and is diagnosed by shoulder arthrocentesis with significant synovial fluid leukocytosis or positive synovial fluid gram stain or culture. There are some risk factors for septic arthritis, notably bacteremia, pre-existing joint pathology and an immunocompromised host such as poorly controlled type II diabetes mellitus. CASE PRESENTATION: A 45-year-old Thai female developed left should…

A case of unilateral shoulder joint hydrarthrosis with wild-type amyloidogenic transthyretin amyloidosis.

Wild-type amyloidogenic transthyretin (ATTR) amyloidosis, known as systemic senile amyloidosis (SSA), is an age-related nonhereditary amyloidosis, which is known to cause cardiomyopathy and carpal tunnel syndrome (CTS). Herein, we report a case of unilateral hydrarthrosis with arthritis of the right shoulder joint in an 82-year-old Japanese housewife who has a seven year history of polyneuropathy due to an unknown aetiology. At first, her joint pain was thought to be caused by overuse of her right upper arm. Despite treatment with non-steroidal anti-inflammatory drugs (NSAIDs) and repeated art…

Late prosthetic shoulder joint infection due to Actinomyces neuii in an adult man.

A 72-year-old man with a history of right reverse shoulder arthroplasty presented with a 1-month history of erythema, pain and drainage from the right shoulder. Arthrocentesis was performed and synovial fluid gram stain revealed gram-positive rods. Clinical diagnosis of prosthetic shoulder joint infection was made. Orthopaedic surgeons performed irrigation and debridement with resection of the right shoulder prothesis and implantation of an antimicrobial spacer. Operative cultures grew Actinomyces neuii The patient was treated with 6 weeks of ceftriaxone with improvement in both clinical sympt…

Clavicle Elevation or Shoulder Girdle Depression in Acromioclavicular Joint Dislocation: A Radiological Investigation.

BACKGROUND: The side-comparative coracoclavicular (CC) distance is used to describe the vertical instability component of acute acromioclavicular (AC) joint dislocations. Elevation of the clavicle or a depression of the shoulder girdle can lead to an increased CC distance. The dislocation direction has not yet been investigated and is not included in common classification systems. HYPOTHESIS: Clavicle elevation is primarily responsible for vertical dislocation in AC joint separation. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Weighted and nonweighted bilateral plain

Modulation of shoulder muscle and joint function using a powered upper-limb exoskeleton.

Robotic-assistive exoskeletons can enable frequent repetitive movements without the presence of a full-time therapist; however, human-machine interaction and the capacity of powered exoskeletons to attenuate shoulder muscle and joint loading is poorly understood. This study aimed to quantify shoulder muscle and joint force during assisted activities of daily living using a powered robotic upper limb exoskeleton (ArmeoPower, Hocoma). Six healthy male subjects performed abduction, flexion, horizontal flexion, reaching and nose touching activities. These tasks were repeated under two conditions:

Glenohumeral joint motion after subscapularis tendon repair: an analysis of cadaver shoulder models.

BACKGROUND: As for the surgical treatment of the rotator cuff tears, the subscapularis tendon tears have recently received much attention for the mini-open or arthroscopic repair. The results of surgical repair for the subscapularis tendon tear are satisfactory, but the range of external rotation is reported to be restricted after the repair. The purpose of this study was to evaluate the range of glenohumeral joint motion after repairs of various sizes of subscapularis tendon tears. METHODS: Using eight fresh frozen human cadaveric shoulders (mean age at death, 81.5 years), three sizes of sub
