Seven patients with chronic dislocations of the shoulder were treated by humeral head or total shoulder replacement. All patients had significant humeral head damage. Three dislocations were posterior and four were anterior. All had been unreduced for at least two months. A Neer prosthesis was used in each patient and the version of the humeral component was altered by 30 degrees-50 degrees away from the direction of the dislocation. Only a sling and swathe was used postoperatively, and the patient was allowed early motion in a limited-goals physical therapy program. There were no redislocatio

BACKGROUND: Little information is available for the outcomes of conversion to total shoulder arthroplasty (TSA) of failed hemiarthroplasty (HA) implanted for fractures or fracture-dislocations of the proximal humerus.
MATERIALS AND METHODS: We evaluated the clinical and radiographic results in 16 patients who underwent conversion of HA to TSA due to pain and shoulder disfunction. Patients were a mean age of 63 years at revision, which was occurred a mean of 3.3 years after the HA. The main prerequisites for conversion were forward flexion to at least 60°, no massive cuff tear, or severe resor

We present a case describing a technique for the surgical management and aesthetico-functional reconstruction of a shoulder in an adult with unilateral upper limb phocomelia (ULP).1 A 25 year old male patient was presented to our clinic with upper left limb non-syndromic phocomelia. His main concerns was an aesthetically unpleasant limb and shoulder shape. Few older publications describe available options for pediatric patients, such as clavicle transposition or limb amputation, but nowadays ULP treatment options focus on prosthetic rehabilitation (PR) and targeted muscle reinnervation (TMR).

The authors present the tragic case of an 18-month-old child who was bitten by a dog, causing amputation of the forearm and substantial damage to the cutaneous muscle on his back, shoulder, thorax, and neck. A free latissimus dorsi flap was performed to preserve the humerus from which the periosteum had been torn away. A series of cutaneous expansions were then undertaken to graft skin back onto the back, the armpit, and the shoulder stump, to allow for a mechanical prosthesis. A study of the literature on this subject proves that dog bites are more frequent and serious (sometimes even fatal)

The objective of this study was to define targeted reaching performance without visual information for transhumeral (TH) prosthesis users, establishing baseline information about extended physiological proprioception (EPP) in this population. Subjects completed a seated proprioceptive targeting task under simultaneous motion capture, using their prosthesis and intact limb. Eight male subjects, median age of 58 years (range 29-77 years), were selected from an ongoing screening study to participate. Five subjects had a left-side TH amputation, and three a right-side TH amputation. Median time si

BACKGROUND: The purpose of this study was to compare long-term outcomes, complications, and reoperation rates of primary reverse total shoulder arthroplasty (RTSA) performed at a single institution using 2 implant designs: a Grammont medialized prosthesis (medialized [M] group) and a Frankle glenoid-based lateralized prosthesis (glenoid-lateralized [GL] group).
METHODS: Between 2004 and 2008, 100 consecutive single-institution primary RTSAs were performed by reconstructive shoulder surgeons who were not design consultants, with the aim of obtaining 10-year follow-up: 56 in the M group and 44

BACKGROUND: Treatment for displaced proximal humeral fractures is still under debate. Few studies exist at the highest level of evidence. Although reversed total shoulder prosthesis has gained popularity and showed promising results in the treatment for proximal humeral fractures in the elderly patients, no randomized controlled trials exist to the authors' knowledge.
METHODS/DESIGN: This study is a randomized semi-blinded controlled multicenter trial designed according to the Consort statement and the recommendations given by the Cochrane reviewers for proximal humeral fractures. The study w…

Routine observation of the scapulohumeral line in the true anteroposterior radiographic view of the shoulder will allow a higher level of confidence in ruling out posterior dislocation of the glenohumeral joint. It will also facilitate evaluation of the relationship between the proximal humerus and the scapula (especially the acromion and the glenoid fossa) in (1) abnormal conditions of the subacromial space; (2) fractures and dislocations of the proximal humerus; and (3) proximal humerus prosthesis insertion and assessment.

PURPOSE: When compared to standard-length humeral stem in reverse total shoulder arthroplasty (RTSA), short humeral stems in RTSA require good proximal humeral metaphyseal bone quality to gain proper and secure fixation during prosthetic implantation. Shorter humeral stems potentially carry more risk of misalignment than standard or long humeral stems. The hypothesis was that misalignment of the short humeral stems is influenced by regional bone quality.
METHODS: RTSA with a short curved humeral stem with neck-shaft angle (NSA) default of 132.5° was reviewed. The study group included 35 cases

PURPOSE: Reverse shoulder prostheses are increasingly used for treatment of rotator cuff tear arthropathy and other degenerative shoulder diseases. In recent years, aiming for bone stock preservation has led to the design of metaphyseal humeral components without a stem. The aims of this study were to evaluate the complication and reintervention rates, as well as the clinical and radiographic outcomes in patients who underwent reverse shoulder arthroplasty (RSA) with stemless implants.
METHODS: A systematic review of the literature was completed until May 2020 using PubMed, EMBASE, CINAHL and

Reverse shoulder arthroplasty is a common procedure. While dislocation is a common complication, there are few reports documenting dislocation with associated polyethylene liner dissociation from the humeral tray. There have been 4 instances of this occurrence over 9 years at our institution, and there are specific radiographic clues for diagnosis. Recognizing polyethylene liner dissociation on imaging is important prior to treatment. In a "routine" dislocation, closed reduction may be attempted but when the polyethylene is dissociated, open reduction is the only treatment option, as closed re

BACKGROUND: in locked dislocation of the shoulder, instead of reducing back to the glenoid, the humeral head remains incarcerated on the glenoid in a locked fashion. This clinical situation is fairly uncommon. It is essential to conduct an individual evaluation of each patient to determine the appropriate treatment.
OBJECTIVE: the aim of this study was to evaluate the functional outcomes of reverse total shoulder arthroplasty (rTSA) in the treatment of locked shoulder dislocation.
METHODS: patients with locked shoulder dislocation who underwent reverse shoulder prosthesis surgery and were ad…

We report a case of targeted muscle reinnervation of a muscle free flap for improved prosthetic control in a patient who had an amputation of the left upper extremity at the level of the shoulder after a severe electrical burn. The reinnervated muscle free flap receives signals from the brachial plexus, and these signals are amplified to provide an interface for a myoelectric prosthesis. This allows for more coordinated and efficient control of the artificial limb.

A 52-year-old man incurred a brachial plexus injury with a two-part anterior fracture dislocation of the shoulder. The fracture was converted to a four-part fracture dislocation during attempts to reduce the initial lesion in a closed fashion. The four-part fracture dislocation was treated with a Neer humeral head replacement. Postoperatively, the prosthesis gradually dislocated inferiorly and relocated spontaneously with the recovery of the brachial plexus lesion.

Background: Irreversible brachial plexus injury can leave patients with a painful, insensate, and nonfunctional flail limb, often after failed reconstructive attempts and with limited remaining surgical options. This study describes a modified transhumeral amputation technique incorporating a passive biceps brachii traction sling to improve residual limb stability and prosthetic readiness in this challenging setting. Methods: This retrospective, uncontrolled five-case series assessed the feasibility, perioperative safety, descriptive clinical outcomes, and early prosthetic integration of trans

AIMS: Stemless humeral implants have been developed to overcome stem-related complications in total shoulder arthroplasty (TSA). However, stemless implant designs may hypothetically result in less stable initial fixation, potentially affecting long-term survival. The aim of this study is to investigate early fixation and migration patterns of the stemless humeral component of the Simpliciti Shoulder System and to evaluate clinical outcomes.
METHODS: In this prospective cohort study, radiostereometric analysis (RSA) radiographs were obtained in 24 patients at one day, six weeks, six months, on

BACKGROUND: Acromial fractures after reverse shoulder arthroplasty (RSA) have been reported to occur in up to 7% of patients. Whereas RSA implant parameters can be configured to alter stability, range of motion, and deltoid mechanical advantage, little is known about the effect of these changes on acromial stresses. The purpose of this finite element study, therefore, was to evaluate the effect of RSA humeral and glenoid implant position on acromial stresses.
METHODS: Solid body models of 10 RSA reconstructed cadaveric shoulders (38-mm glenosphere, 155° neck-shaft angle) were input into custo

National shoulder arthroplasty registries are currently used to assess incidence, indication, type of prosthesis and revision, but they seem to lack sufficient information to lead to evidence based decision-making in shoulder surgery. There appears to be a large difference in registered parameters and outcome measurement per country. First we investigated whether existing registries have sufficient common datasets to enable pooling of data. Second, we determined whether known risk factors for prosthetic failure are being recorded. Through a non-systematic literature review studies on registrie
