We describe the case of a 61-year-old woman diagnosed with primary Sjögren's syndrome (SS) after an 8-year history of IgA nephropathy and a 3-year history of recurrent purpuric rashes. Her two daughters had previously been diagnosed with other autoimmune diseases. One daughter had Graves' disease and the other had Hashimoto's disease and systemic lupus erythematosus. Twelve of 25 patients with mixed connective tissue disease complained of xerostomia and/or ocular symptoms of keratoconjunctivitis sicca. In addition to the clinical features of mixed connective tissue disease, all 12 patients had high titers of antibody to the ribonuclease-sensitive component of the extractable nuclear antigen. Eight patients had both clinical xerostomia and keratoconjunctivitis sicca, one had keratoconjunctivitis sicca and salivary gland enlargement, while there had xerostomia but no ocular complaints. A case is presented showing the first reported use of a small, digitally-guided, intra-oral ultrasound probe to image a calculus in a submandibular salivary duct. This use of intra-oral ultrasound is discussed in the context of the overall current status of intra-oral and extra-oral ultrasound imaging. It is proposed that using a combination of intra-oral and extra-oral probes, the whole of the salivary apparatus may now be examined sonographically for the presence of calculi.

Xerostomia is the main oral symptom and clinical sign in Sjögren's syndrome (SS) but there are many other causes of a dry mouth. Salivary gland enlargement is a further feature but may be episodic. Xerostomia is a subjective symptom which can be non-invasively assessed by sialometry but this is too variable, insensitive and non-specific to be diagnostic of SS. Sialochemistry, though conceptually appealing is nondiscriminatory and of little diagnostic value. An unusual sialoith within Wharton's duct was associated with an abscess of the submandibular space. Previously reported sialoiths of considerable size are reviewed, and etiology, symptomatology, and surgical removal are discussed. Emphasis is placed on the need for thorough examination of patients who have symptoms of sialoithiasis in an attempt to locate the sialolith before it assumes unusual dimensions.

A series of 73 lesions presenting in the parapharyngeal space is presented. There were 53 (73 per cent) patients with deep lobe parotid benign or malignant tumours, five (7 per cent) patients with tumours of neurogenous origin, seven (9 per cent) patients with abscesses, five (7 per cent) patients with enlarged lymph nodes, two with branchial cysts, and one with a lipoma.
