Vestibular Disability/Handicap in Fibromyalgia: A Questionnaire Study.

Fibromyalgia (FM) is a poorly understood, central pain processing disorder characterized by a broad range of symptoms, such as chronic pain, sleep disruption, chronic fatigue, and psychosomatic symptoms. In addition, recent studies have shown that FM patients also experience dizziness. We aimed to establish a prevalence rate of vestibular symptoms in a population of FM patients through a battery of questionnaires investigating socio-demographic, clinical and psychological characteristics, combined with the Dizziness Handicap Inventory (DHI) and the Situational Vertigo Questionnaire (SVQ). A to

Spectral responses of the human circadian system depend on the irradiance and duration of exposure to light.

In humans, modulation of circadian rhythms by light is thought to be mediated primarily by melanopsin-containing retinal ganglion cells, not rods or cones. Melanopsin cells are intrinsically blue light-sensitive but also receive input from visual photoreceptors. We therefore tested in humans whether cone photoreceptors contribute to the regulation of circadian and neuroendocrine light responses. Dose-response curves for melatonin suppression and circadian phase resetting were constructed in subjects exposed to blue (460 nm) or green (555 nm) light near the onset of nocturnal melatonin secretio

Baihe Ganmai Dazao Decoction ameliorates depression-like and sleep-disturbance phenotypes: potential involvement of IL-17A/IL-17RA-related inflammatory signaling and microglial polarization.

ETHNOPHARMACOLOGICAL RELEVANCE: Originally documented in Zhang Zhongjing's Synopsis of Prescriptions of the Golden Chamber of the Eastern Han Dynasty, Ganmai Dazao Decoction (GMDZ) is composed of three medicinal materials: Glycyrrhizae Radix et Rhizoma (GRR), Tritici Levis Fructus (TLF), and Jujubae Fructus (JF). Traditionally, it has been used to treat zangzao, a syndrome characterized by emotional disturbance and sleep disorder, and is now widely prescribed for depression, anxiety, and insomnia. Nevertheless, the effective material foundation and intrinsic therapeutic mechanisms of GMDZ are

Neuropsychiatric features of Parkinson's disease in the era prior to the use of dopaminergic therapies.

BACKGROUND: Psychosis in Parkinson's disease includes hallucinations and delusions. Other non-psychotic neuropsychiatric features include depression, anxiety and apathy. There is currently controversy over whether psychosis in Parkinson's is an intrinsic part of the disorder or the result of dopaminergic medications. This study aimed to examine a historical cohort of individuals with Parkinson's prior to the use of dopaminergic therapy to assess the prevalence of psychotic and other neuropsychiatric features. METHODS: The case notes of patients with Parkinson's disease admitted to the Nationa

Mitigating Blue-Light Risk in Display-Based Digital Therapeutics: A Practical Framework to Support Clinical Efficacy.

Display-driven optical stimuli underpin a major class of clinically validated digital therapeutics (DTx) now expanding from neuropsychiatric disorders to chronic diseases. The display's optical characteristics-spectral power distribution, luminance, contrast, and temporal modulation-therefore define the delivered dose of these software-based interventions. In this context, blue-rich emission in the 450-480 nm band, particularly with evening exposure, can suppress melatonin via melanopsin-mediated intrinsically photo-sensitive retinal ganglion cell (ipRGC) pathways and perturb circadian timing,

GABAB - and GABAA -receptor-mediated regulation of Up and Down states across development.

Slow oscillations, the hallmark of non-REM sleep, and their cellular counterpart, Up and Down states (UDSs), are considered a signature of cortical dynamics that reflect the intrinsic network organization. Although previous studies have explored the role of inhibition in regulating UDSs, little is known about whether this role changes with maturation. This is surprising since both slow oscillations and UDSs exhibit significant age-dependent alterations. To elucidate the developmental impact of GABAB and GABAA receptors on UDS activity, we conducted simultaneous local field potentials and intra

Impact of circuit configuration on the mechanical performance of CPAP therapy devices: an experimental study.

INTRODUCTION: CPAP therapy treats various respiratory disorders. The overall performance of therapy delivery can be affected by the adopted circuit configuration. Recently, parallel to the canonical open configuration (OC), closed configurations (CC) have been proposed with potential advantages in terms of oxygen consumption, noise, airway dryness and contamination. However, the mechanical performance of CPAP devices in CC has been marginally investigated. The aim of this study is to clarify whether CPAP therapy delivered in CC configuration retains mechanical performances equivalent to that a

Quality of life tools among patients on dialysis: A systematic review.

INTRODUCTION: The universal increase in obesity and diabetes has increased the chronic kidney disease (CKD) rate. In 2017, almost 800 million individuals suffered from CKD worldwide. Kidney dialysis becomes necessary as the disease progresses. Dialysis negatively impacts CKD patients' quality of life (QoL). It causes several complications that affect patients' physical, social, psychological, and spiritual aspects of life. This systematic review aims to identify condition-specific tools used to assess CKD patients' quality of life on dialysis. MATERIAL AND METHODS: A systematic literature sea

Complementary home mechanical ventilation techniques. SEPAR Year 2014.

This is a review of the different complementary techniques that are useful for optimizing home mechanical ventilation (HMV). Airway clearance is very important in patients with HMV and many patients, particularly those with reduced peak cough flow, require airway clearance (manual or assisted) or assisted cough techniques (manual or mechanical) and suctioning procedures, in addition to ventilation. In the case of invasive HMV, good tracheostomy cannula management is essential for success. HMV patients may have sleep disturbances that must be taken into account. Sleep studies including complete

Diurnal Regulation and Gene-Specific Vulnerability of Oxidative Alcohol-Metabolizing Enzymes to Circadian Disruption.

Oxidative alcohol metabolism in the liver relies on sequential enzymatic reactions involving alcohol dehydrogenase (ADH), cytochrome P450 2E1 (CYP2E1), and aldehyde dehydrogenase (ALDH) isozymes. However, the circadian regulation of these enzymes, their susceptibility to genetic, environmental, and metabolic disruption, and their functional implications toward alcohol-mediated tissue injury remain incompletely defined. To address this gap, we performed a comprehensive integrative analysis of the publicly available circadian transcriptome datasets spanning genetic clock disruption, acute sleep

Chronic intermittent hypoxia alters genioglossus motor unit discharge patterns in the anaesthetized rat.

The respiratory control system is subject to diverse and considerable plasticity in health and disease. Intermittent hypoxia elicits expression of intrinsic plasticity within sensory and motor pathways involved in the control of breathing with potentially adaptive and maladaptive consequences for respiratory homeostasis. We and others have shown that chronic intermittent hypoxia (CIH) - a major feature of sleep-disordered breathing - has deleterious effects on rat upper airway dilator muscle contractile function and motor control. In the present study, we sought to test the hypothesis that CIH

Cerebellar-limbic dysregulation and sensorimotor network alterations in male primary insomnia: a resting-state functional magnetic resonance imaging study.

OBJECTIVE: The neuropathological mechanisms specific to male insomnia disorder (ID) remain underexplored, particularly regarding intrinsic brain activity patterns. Using functional magnetic resonance imaging (fMRI), this study aims to investigate the characteristics of amplitudes of low-frequency fluctuations (ALFF) and regional homogeneity (ReHo) in male patients with primary ID, and to explore the correlations between these neuroimaging indicators and sleep scale scores. METHODS: A total of 30 male patients diagnosed with ID and 30 age-matched healthy controls (HCs) were enrolled. All parti

Successful antidepressant chronotherapeutics enhance fronto-limbic neural responses and connectivity in bipolar depression.

The identification of antidepressant response predictors in bipolar disorder (BD) may provide new potential enhancements in treatment selection. Repeated total sleep deprivation combined with light therapy (TSD+LT) can acutely reverse depressive symptoms and has been proposed as a model antidepressant treatment. This study aims at investigating the effect of TSD+LT on effective connectivity and neural response in cortico-limbic circuitries during implicit processing of fearful and angry faces in patients with BD. fMRI and Dynamic Causal Modeling (DCM) were combined to study the effect of chron

Nonmotor symptoms of Parkinson's disease revealed in an animal model with reduced monoamine storage capacity.

Parkinson's disease (PD) is a progressive neurodegenerative disorder that is characterized by the loss of dopamine neurons in the substantia nigra pars compacta, culminating in severe motor symptoms, including resting tremor, rigidity, bradykinesia, and postural instability. In addition to motor deficits, there are a variety of nonmotor symptoms associated with PD. These symptoms generally precede the onset of motor symptoms, sometimes by years, and include anosmia, problems with gastrointestinal motility, sleep disturbances, sympathetic denervation, anxiety, and depression. Previously, we hav

[Falls and renal function: a dangerous association].

Falls are an important health problem and the risk of falling increases with age. The costs due to falls are related to the progressive decline of patients' clinical conditions, with functional inability inducing increasing social costs, morbidity and mortality. Renal dysfunction is mostly present in elderly people who often have several comorbidities. Risk factors for falls have been classified as intrinsic and extrinsic, and renal dysfunction is included among the former. Chronic kidney disease per se is an important risk factor for falls, and the risk correlates negatively with creatinine c

Circadian variation of the effects of immobility on symptoms of restless legs syndrome.

STUDY OBJECTIVES: It is now well established that symptoms of restless legs syndrome (RLS) are worsened by immobility and that their severity fluctuates according to a circadian pattern with a maximum occurring in the late evening or during the night. However, no study has ever attempted to dissociate these two effects. The objective of this study was to evaluate the nycthemeral variations in the effects of duration of immobility on symptoms of RLS. DESIGN: A 28-hour modified constant routine protocol. SETTING: Sleep Disorders Center, Montreal Sacré-Coeur Hospital. PARTICIPANTS: Seven patie

Histomorphometrical Analyses of the Mouse Suprachiasmatic Nucleus.

The mammalian central circadian clock resides in the suprachiasmatic nucleus (SCN) of the hypothalamus in the brain and is responsible for coordinating daily rhythms of biological processes spanning from gene expression to behavior. Light, the primary environmental zeitgeber, entrains the SCN via melanopsin-expressing intrinsically photosensitive retinal ganglion cells that project through the retino-hypothalamic tract. Altered circadian rhythms are common in individuals diagnosed with neurodevelopmental and neurodegenerative disorders, and often, associated with structural alterations of the
