Chronic foreign body of the nasal cavity and sphenoid sinus: surgical implications.

Chronic sinonasal foreign bodies present unique surgical challenges including tissue integration and anatomic migration. A patient with a history of multiple surgeries for cleft lip, palate, and nasal deformity presented with radiographic findings of a 4.3-cm linear foreign body traversing the nasal cavity and sphenoid sinus. Mucosalization and integration into the sphenoid rostrum were noted at the time of surgery. Endoscopic surgery techniques facilitated removal of the overlying mucosa and encasing bone, allowing successful surgical extraction. On inspection, the foreign body was consistent

Lateral transzygomatic approach to sphenoid wing meningiomas.

BACKGROUND: Sphenoid wing meningiomas are slow-growing, well-circumscribed, and histologically benign lesions. The recurrence rate is low if removed completely at the time of surgery. Adequate surgical exposure with minimal morbidity is a challenge for those treating these lateral skull base lesions. OBJECTIVE: To describe our experience with the lateral tranzygomatic approach for resection of sphenoid wing meningioms in which the entire zygoma is mobilized and remains vascularized by masseter muscle attachments. METHODS: A retrospective review of the records of 19 patients who underwent sph

Sphenoid and optic nerve sheath meningioma revealed by recurrent brain infarctions.

Meningioma, though benign, may invade adjacent structures such as bone, soft tissues, dural sinuses and arteries. However brain infarctions secondary to meningioma involving the cavernous sinus and encasing and narrowing the intracranial carotid artery are rare. We report the case of a young man with recurrent left carotid artery infarctions due to a left sphenoid meningioma infiltrating the posterior optic nerve sheath through the optic canal and circumscribing the intracranial carotid artery. The patient had a gradually progressive occlusion of the middle cerebral artery, the distal internal

The posterior or tubal sphenoidal plate extending into the tympanosquamous fissure - A novel finding.

OBJECTIVE: The tympanic plate typically joins the articular tubercle in the medial glenoid wall. It is aimed to report a peculiar anatomic v … postero-medial sphenotympanic one. CONCLUSION: As far as the authors know, the tubal plate of the sphenoid bone was not observed previously to participate in the glenoid fossa's medial wall. It could be regarded as a rare anatomic variation.

Thyroid papillary carcinoma with an insular component metastasizing to the sella turcica and sphenoid sinus: Case report.

INTRODUCTION: Papillary carcinoma accounts for approximately 80% of all thyroid carcinomas. It is associated with relatively good survival. Distant metastases occur in approximately 10% of the patients, with the lung and bone being the most commonly reported sites. We present a case of unusual metastasis to the sphenoid bone and sella turcica from papillary thyroid carcinoma with an insular component. CASE PRESENTATION: We present a case of 70 years old female patient who presents a voluminous goiter with an 11 cm mass of the left sixth rib. Trans-parietal biopsy proved its metastatic origin …

MR Imaging of the Pituitary Gland and Postsphenoid Ossification in Fetal Specimens.

BACKGROUND AND PURPOSE: A thorough knowledge of fetal growth and development is key to understanding both the normal and abnormal fetal MR imaging findings. We investigated the size and signal intensity of the normal pituitary gland and the intrasphenoidal ossification around the Rathke pouch in formalin-fixed fetuses on MR imaging. MATERIALS AND METHODS: Thirty-two fetuses with undamaged brains were included in this study (mean age, 19.93 weeks; age range, 12-31 weeks). Visual inspection of the pituitary and ossification around the Rathke pouch in the sphenoid bone or the postsphenoid ossifi…

Rare Case of Metastatic Breast Cancer to the Sphenoid Sinus: Diagnosis and Treatment.

INTRODUCTION: Breast cancer (BC) is the most common cancer among women worldwide. While metastases typically affect the lungs, liver, and bones, spread to the paranasal sinuses, especially the sphenoid sinus, is extremely rare. CASE REPORT: A 57-year-old woman with a history of infiltrative ductal carcinoma of the breast diagnosed 12 years earlier presented with progressive left-sided vision blurring and headaches for 3 weeks. Imaging revealed a heterogeneous lesion in the left sphenoid sinus compressing the optic nerve. The metastatic breast carcinoma with histopathological and immunohistoch

Aggressive resection of craniopharyngioma achieved by drilling the tuberculum sellae/planum sphenoidale using a frontal basal interhemispheric approach: A review of 55 cases.

OBJECTIVE: The frontal basal interhemispheric approach (FBIA) is preferable for resection of craniopharyngioma (CP), achieving desirable total resection rates in early reports of lesions located in the suprasellar region to the third ventricle. For tumours that have created a larger obstruction of the tuberculum sellae and planum sphenoidale, aggressive resection in the intrasellar region and medial wall of the cavernous sinus is not feasible compared to improving tumour visualization by drilling the tuberculum sellae and planum sphenoidale. In a report of drilling the sellar tuberculum and sp

Transcranial correction of orbital neurofibromatosis.

Neurofibromatosis is a systemic disease that often produces striking disfigurement. Orbital manifestations are common and include sphenoid dysplasia with or without infiltration of the periorbital soft tissues. The resultant deficiency of the posterolateral orbital wall may lead to protrusion of the temporal lobe into the orbit, displacement of the globe, and pulsatile exophthalmos. Treatment at our unit has consisted of transcranial orbital reconstruction with bone grafts and periorbital soft-tissue correction. Observation of complete bone graft resorption in one patient prompted an assessmen

The natural history and pathogenesis of the cranial coronal ring articulations: implications in understanding the pathogenesis of the Crouzon craniostenotic defects.

The craniostenotic birth defects seen in patients with Crouzon syndrome have prompted this developmental study on the system of articulation … t a coronal ring exists beginning at 8 weeks. Those portions of the ring separating the frontal and sphenoid bones (within the orbit and laterally along the coronal suture) show the typical structure of a five-layered suture. This sutural component of the ring is C-shaped with a cartilaginous bridge between the optic foramina completing the ring. This is the sp …

Surgical treatment of trigonocephaly. Experience with 30 cases.

Trigonocephaly results from premature closure of the metopic suture. We report our experiences with 30 patients who underwent surgical treatment. In our opinion, this type of craniosynostosis also involves sphenoid bone dysplasia. For this reason, our surgical procedure includes turning the orbital ridge away from the midline, frontal vault remodeling, and a wide ostectomy of the pterional bone.

Transantral repair of cerebrospinal fluid fistulas using bone chips, Tisseel, fascia and plaster of Paris.

Cerebrospinal rhinorrhea is a potentially serious symptom due to the risk of an ascending infection, which may produce a fulminant meningitis. Plaster of Paris has been used to obliterate the sphenoidal sinus and posterior ethmoids in five patients with cerebrospinal rhinorrhea. The fistulas were identified by computed cisternography and repaired by a transantral approach using a microsurgical technique, and bone chips, Tisseel and fascia to seal the fistulas. There have been no postoperative problems or leakage in the three patients with fistulas to the sphenoidal sinus. The cerebrospinal rhi

The volumetric relationship of mastoid cavity and paranasal sinuses in unilateral chronic otitis media.

AIMS: A number of studies link ear and paranasal sinus disorders even though correlation between the development of the paranasal sinuses and the mastoid has not been demonstrated clearly. In this study, we examined temporal bone computed tomography (CT) scans to calculate mastoid and paranasal sinus volumes using three-dimensional (3D) reconstruction in patients with unilateral nonsuppurative chronic otitis media (COM) and investigated whether there was a correlation between mastoid and paranasal sinus volumes. METHODS: Of 85 patients with unilateral nonsuppurative COM, 51 patients who under

Effect of Fe₃O₄ Nanomaterial-Based MRI on the Efficacy of One-Sided Butterfly Sinus Lesions in the Nose.

Fe₃O₄ materials have many crystal forms, and the morphology of their nanomaterials is more diverse. Under the influence of nano-effects, they exhibit unique physical and chemical properties. The synthesized Fe₃O₄ materials with different morphologies have different properties. They are widely used in related fields such as electromagnetics, chemical engineering, catalysis, sensors, acoustics, medicine, environmental protection and so on. Choosing the appropriate contrast agent to display the anatomical characteristics of diseased and normal tissue is an important thing before using MRI (Magnet
