An ectopic pituitary adenoma, exclusively involving the sphenoid sinus, associated with a normal size pituitary gland and an empty sella is a rare clinical entity. Only four cases have been found in the literature. We describe the case of a 47-year-old woman with clinical and biochemical acromegaly. Computed tomography (CT) appeared to show expansion of the sellar floor inferiorly into the sphenoid sinus, and magnetic resonance imaging (MRI) demonstrated an empty sella and a mass lesion confined to the sphenoid sinus. The tumor was totally resected by taking a transnasal endoscopic approach an…

OBJECTIVE: To report 3 cases of pneumosinus dilatans of the sphenoid sinus associated with visual loss.
MATERIALS AND METHODS: Retrospective case series describing history of visual loss, visual examination, visual field deficits, and radiologic imaging.
RESULTS: Three patients developed visual loss associated with pneumosinus dilatans of the sphenoid sinus.
CONCLUSIONS: Pneumosinus dilatans of the sphenoid sinus is a rare disorder that should be considered in patients presenting with unexplained visual loss.

PURPOSE: Optic neuritis secondary to sinus disease is an infrequent but well-documented association. When a patient presents with signs of o … 11 patients with optic neuritis who, because of atypical headache, underwent neuroimaging revealing sphenoid sinus opacity. Six patients had endoscopic drainage of the sphenoid sinus; 4 were treated medically.
RESULTS: Sinus contents included fungal infection (2), mucopurulent material (5), polyps (1) and necrotic tumour (1). Narrowing of the optic canal due to …

This report concerns one case of a sphenoid sinus mucocele occurring 17 years after transsphenoidal surgery for acromegaly. In 1979, a 51-year-old man was successfully operated by the transnasal transsphenoidal approach for a growth hormone (GH) adenoma 1 cm in diameter. In 1996, the patient was hospitalized for headaches and diplopia. He presented a loss of right visual acuity with paralysis of the right oculomotor nerve. The basal GH level was normal with a satisfactory decrease after oral glucose ingestion. Pituitary sellar radiography showed a disappearance of the posterior clinoid while m…

INTRODUCTION: Benign diseases of the sphenoid sinus can be classified as intrinsic, originating from the mucosa and sinus bone walls, and extrinsic, arising from adjacent structures. We analyze the CT features of both intrinsic and extrinsic benign sphenoid conditions and their diagnostic role and impact on treatment planning.
MATERIAL AND METHODS: We reviewed the CT scans of 75 patients with benign sphenoid sinus conditions from our 1993-96 series. The conditions were diagnosed at histology of the surgical specimen or from clinical findings and patient's history. Both axial scans, parallel t…

Paranasal sinuses development and its ultimate mode of happening as well, has been the purpose of numerous hypotheses, that can be synthetized in this two: One which sees the pneumatization as mechanical event resulting from muscular tractions exerted on the sphenoidal sinus walls and another one supposing the pneumatization as an active phenomenon due to the osteoclastic potential of the same respiratory mucous membrane. In this work were employed 200 radiogramms belonging both to patients suffering chronic ENT infections and subjects without antecedents. The conclusion drawn out are that chr

A case of aspergillosis of the sphenoid sinus manifested as an isolated sixth nerve palsy occurred in a 74-year old diabetic woman who had no complaints of headache or symptoms suggestive of sinusitis. A CT scan demonstrated a large mass occupying the sphenoid and ethmoid sinuses extending posteriorly to the clivus. There was a calcific density within the opacified sinus and bony erosion of the sphenoid walls and the sella turcica. The patient underwent a sublabial transseptal sphenoidotomy with removal of necrotic material and debridement of the surrounding tissue. Histologic examination reve…

Morphological relation between the sphenoidal sinus and the petrous apex cell was studied using HRCT images obtained by 339 adults and 43 infants. The adults were classified into normal group (without chronic otitis media or its surgery) 237 cases, and otitis group (chronic otitis media or its post. op.), 102 cases. The posterior extent of the sphenoidal sinus was evaluated in reference to the carotid canal, and the petrous apex cells were evaluated by their mode of distribution in the apex. In the normal adults, no evident relation was seen between the posterior extent and the mode of distrib

A man of 74 years of age, suffering from a left-sided ophthalmoplegia and a radiologically detected opacification of the sphenoid sinus with destruction of the bony roof of the sphenoid sinus, was operated on because a malignant tumor was suspected. A destruction of the bony walls of the sphenoid sinus was found. The histological examination of the "glue"-like "tumorous" material revealed an aspergillosis. The patient developed an aspesrgillus meningitis postoperatively. Intrathecally administered Amphotericin B led to an improvement of the meningitis, but caused a fatal renal failure. A revie…

Pleomorphic adenoma is the most common benign salivary gland tumour of the head and neck region, most commonly seen in parotid glands. These may arise also from minor salivary glands of the upper aerodigestive tract comprises the upper lip, cheek, floor of the mouth and rarely from mucoserous glands in the nasal cavity and paranasal sinuses. A middle-aged man, who had undergone surgery for a nasal mass 14 years ago, presented with headache and visual complaints from a recurrent mass originating from the sphenoid sinus. Isolated sphenoid sinus space-occupying lesions have always been a diagnost…
