STUDY DESIGN: A case of spondylolisthesis caused by osteoporosis is described.
OBJECTIVES: To describe the different etiologies of spondylolisthesis, and to report a new cause of pathologic spondylolisthesis previously unreported in the literature.
SUMMARY OF BACKGROUND DATA: The literature on the etiology of spondylolisthesis is reviewed. Pathologic spondylolisthesis has been described with structural abnormalities, such as Paget's disease and tumors, but not with osteoporosis.
METHODS: A clinical and radiographic interpretation of the reported case is presented.
RESULTS: De Novo developm…

This article provides an overview of studies that address the medical and surgical treatment of lumbar spondylolisthesis, both degenerative and isthmic. Although the efficacy of decompression for symptomatic lumbar stenosis recalcitrant to conservative treatment has been demonstrated, the addition of instrumentation to a fusion procedure remains controversial. The senior author's (VKHS) experience with pedicle screw fixation and fusion for lumbar spondylolisthesis, the addition of interbody fusion, and spinal navigation is reviewed.

Degenerative spondylolisthesis is four times more common in women than in men. Although this gender difference has long been recognised there has been no explanation for it. We have examined the radiographs and CT scans of 118 patients over the age of 55 years and of a control group under the age of 46 years. Our findings confirmed the presence of more sagittally-orientated facet joints in patients with degenerative spondylolisthesis but did not show that the gender difference can be explained by the morphology of the facet joint. Furthermore, we conclude that the increased angle of the facet …

We performed a case-control study on the influence of oophorectomy on the development of degenerative spondylolisthesis, including a clinical review and determination of serum oestradiol levels. We also compared the radiological appearance of the lumbar spine at L4/5 in patients with and without spondylolisthesis and with and without oophorectomy. Oophorectomy was a risk factor for degenerative spondylolisthesis with an odds ratio of 7.5 (95% confidence interval, 1.6 to 46). The incidence of degenerative spondylolisthesis in 69 oophorectomised patients was about three times higher than in 69 n…

Degenerative lumbar spondylolisthesis associated with spinal stenosis is a common condition of the aging spine. This article presents a detailed description of the pathophysiology, clinical presentation, and nonoperative and operative intervention of this condition.

Degenerative spondylolisthesis is most often seen at the L4-5 level. The most common complaint is back pain, but the advent of leg symptoms, such as claudication and restless legs syndrome, is often the reason for seeking specialized medical attention. Conservative treatment usually suffices; extended bed rest is of little value. The 15% of patients who are surgical candidates are those with clinical signs and symptoms of cauda equina abnormality, progressive muscular weakness, or progressive incapacitating radicular pain or claudication. The author advocates pedicle-to-pedicle decompression w…

Conservative treatment has been applied to patients suffering from low degree of lumbar spondilolysthesis, better to say those with light symptoms, or as an attempt before an operation. The treatment aims to spine functional capacity improvement and to reduce patients' unhealthy behaviour. The stress has been put to patient's active role in the treatment, and to the importance of the right choice of kinesitherapeutic exercises. In this report there is a preliminary review of observation of 28 patients suffering from lumbar spondilolystheses and conservatively treated with a definite kind of ki

We evaluated the clinical and radiologic results and the immobilizing effect of the Knight-Kim brace for bony union in 75 consecutive cases of spondylolisthesis treated by anterior interbody fusion. The fifth lumbar vertebra was the most common site involved (55%). The patients were followed for a minimum of 2 1/2 years (range: 2 1/2 to 14; average: 4.5). All cases were approached retroperitoneally, and autogenous bone graft was obtained from the iliac crest. The donor site was reconstructed by polymethylmethacrylate bone cement. Among the 75 cases of grade I, II, and III slippage according to…

The basic principles of the operative treatment of lumbar spondylolisthesis became uniform in the literature in the last years. Authors deal shortly with some questions of the indication and of the preoperative examination and describe shortly the basic principles of the operative treatment (decompression, reduction, stabilization). The operative methods used by them are reported and illustrated (Morscher's "hookscrew" method, fusion according to Harrington-Vidal, Cloward's posterior corpodesis--PLIF--and Steffee's transpedicular system).

In order to determine the factors that produce a successful outcome in the surgical management of spondylolisthesis, 44 consecutive patients who underwent surgical treatment were reviewed. Preoperative evaluation was directed towards determining the presence or absence of nerve root compression. The patients without nerve root compression typically underwent fusion alone, while those with evidence of nerve root compression had decompression and fusion. After an average 36-month follow-up, results were good in 78%, fair in 18%, and poor in 4%. There were five complications, all of which were su…

The literature documents progression of spondylolisthesis, most commonly during the adolescent growth spurt. Twenty-eight patients with Grades I and II spondylolisthesis were treated with antilordotic braces. Presenting signs and symptoms included back pain (61%), tight hamstrings (53%), increased lordosis (25%), and mild scoliosis (21%). Three patients presented with spondylolysis and progressed to a slip prior to initiation of brace treatment. Mean duration of brace treatment was 25 months. In the brace, lateral roentgenograms demonstrated a significant reduction of lumbar lordosis and sacra…

Little attention has been given to the amount of further slip and the success rate after decompression for degenerative spondylolisthesis. In this retrospective study, the answers to the following were sought: (1) Did decompression relieve symptoms; (2) Which decompressive technique was best; (3) Did further slip occur; (4) Did further slip make patients worse? Seventeen patients were available for an average 20-month follow-up. Six patients had a decompression laminectomy with facetectomies, and 11 had a decompression laminectomy with foraminotomies. None was fused. Patients rated their resul…

The topographic relation of the spine to the great vessels let to the expectation that spondylolisthesis may lead to mechanical irritation of the cauda equina as well as of the great vessels. The angiological examination of 223 patients with spondylolisthesis of the lumbar spine shows that the changes in the anatomy of the great vessels are less significant than expected after experiments involving artificial dislocations of the spine in cadavers. In vivo arterial function is not altered even in true spondyloptosis. Pelvic veins on the other hand show considerable compression and diminished fl…

The importance of spondylolisthesis, aside from the obvious malalignment and malformation of the affected vertebrae, rests mainly on the possibility and the extent of impairment of activity. In this discussion the authors are concerned mainly with the myelographic signs associated with spondylolisthesis in patients with isthmic changes.
