Chylomicron retention disease: A rare cause of chronic diarrhea.

Chylomicron retention disease (CRD) is a rare autosomal recessive hereditary hypocholesterolemic disorder. The disease most frequently presents in infants and is characterized by a lipid malabsorption syndrome with steatorrhea, chronic diarrhea, and growth retardation. The disease is characterized by normal fasting serum triglyceride levels combined with the absence of apolipoprotein (apo) B48 and chylomicrons after a fat load. In this report, we describe the clinical, laboratory, and histological data as well as the molecular DNA analysis of a 12-month-old girl from Tunisia with CRD. The pati…

Duodenal somatostatinoma presenting with complete somatostatinoma syndrome.

Somatostatinomas are the rarest pancreatic endocrine tumors and can arise in the pancreas or duodenum. Duodenal somatostatinomas are less common than, and are distinguished from, their pancreatic counterparts by a frequent association with type I neurofibromatosis, the presence of psammoma bodies, the less frequent presence of metastatic disease, and the absence of somatostatinoma syndrome (diabetes mellitus, steatorrhea, and cholelithiasis). We report a case of somatostatinoma with metastases and psammoma bodies presenting with all three features of the syndrome in a patient with neurofibroma…

The Neurologic Syndrome of Vitamin E Deficiency: Laboratory and Electrophysiologic Assessment.

A human requirement for vitamin E was established in the 1960s; however, its role in neurologic function has been recognized only for the past 10 years. The neurologic syndrome of vitamin E deficiency is seen in patients with chronic steatorrhea and principally affects functions of the spinal cord posterior column and retina; however, it can also involve other parts of both the central and peripheral nervous system and the proximal muscles. This review examines the clinical and laboratory assessment of patients suspected of having vitamin E deficiency and points out the importance of recognizi…

Chronic pancreatitis in Sicily. Preliminary reports.

The aetiology and clinical characteristics of 42 patients affected with chronic calcifying pancreatitis in a Sicilian population were investigated and compared with a series of other Italian and foreign reports. It was found that the aetiology was 62% alcoholics and 38% idiopathic in origin and that the M/F ratio was 2.5:1. Clinical features as pain, weight loss, pancreatic calcifications, steatorrhea, pseudocysts and associated cirrhosis are significantly more frequent in the group of alcoholics, while cholelithiasis was more frequent in the non-alcoholic group. Two aspects are worth noting i…

Methyltestosterone-induced night blindness.

A 59-year-old man presented with a 3-month history of night blindness and a 9-month history of steatorrhea. Both symptoms had appeared after he had begun taking methyltestosterone. Investigations revealed low serum levels of carotene (0.1 mmol/L) and vitamin A (0.4 to 0.7 mmol/L), anomalous colour perception, elevation of the rod threshold by 3.5 log units in dark adaptometry, and decreased b-wave amplitudes in photopic and scotopic electroretinograms. No biochemical evidence of cholestasis was elicited. The symptoms and the biochemical and electrophysiologic abnormalities resolved within 9 mo…

Integrated transcriptome profiling of plasma exosomes reveals molecular stratification of exocrine and endocrine disorders and S100A8-mediated cell interactions in chronic pancreatitis.

Exocrine and endocrine disorders and insufficiency are two major harmful pathological processes in chronic pancreatitis (CP) and can lead to steatorrhea and diabetes. However, there is a lack of reliable clinical classification schemes for evaluating the severity of exocrine and endocrine disorders in CP, and the underlying mechanisms are also unclear. In particular, exosome-based liquid biopsy and classification in CP are lacking. Here, we performed transcriptome sequencing on plasma exosomes from CP patients with different degrees of CP severity. Additionally, we analyzed single-cell sequenc…

Acute Appendicitis as the First and the Only Presentation of Systemic Mastocytosis.

Systemic mastocytosis is a rare myeloproliferative disorder characterized by abnormal accumulation of mast cells in a variety of organs. When affecting the gastrointestinal tracts, it may manifest with steatorrhea, malabsorption, hepatomegaly, splenomegaly, portal hypertension, and ascites, among others. To our knowledge, only one case of systemic mastocytosis has been reported affecting the appendix. We present another case of a 47-year-old woman who was admitted for right-sided acute abdominal pain and found to have systemic mastocytosis in her appendectomy specimen as the first and only man…

[The small-intestine-stasis syndrome following side-to-side anastomosis].

This review describes the small intestine stasis syndrome after side-to-side anastomosis. Symptoms of intermittent abdominal pains with steatorrhea and loss of weight are caused by disturbance of passage with a rise in bacterial concentrations. Of 27 patients observed 15 had no disorders, while 12 had gastrointestinal symptoms which had been misinterpreted and given conservative treatment for a long time (3--22 years). Treatment is either by antibiotics or by excision of the lateral anastomosis followed by end-to-end anastomosis of the normal intestine. Special attention must be paid to the co…

Renal Mucosa-Associated Lymphoid Tissue Lymphoma Mimicking Autoimmune Pancreatitis: A Diagnostic Challenge.

An 87-year-old man presented with progressive steatorrhea, weight loss, and subsequent hyperglycemic crisis. Initial workup revealed combined exocrine and endocrine pancreatic insufficiency with mildly elevated immunoglobulin G4 below the diagnostic threshold and diffuse pancreatic inflammation on magnetic resonance imaging, raising suspicion for autoimmune pancreatitis. Follow-up imaging revealed an incidental renal mass, which biopsy confirmed as mucosa-associated lymphoid tissue lymphoma. Positron emission tomography-computed tomography demonstrated fluorodeoxyglucose uptake in both the ren…

The importance of exclusive breastfeeding in infants at risk for celiac disease.

This article presents a review of studies examining the association between breastfeeding and delayed symptomatic celiac disease and provides implications for practice. Celiac disease is a chronic intestinal disease involving intolerance to gluten. Symptomatic celiac disease varies in presentation and may include malabsorption, diarrhea, steatorrhea, and malnutrition. Its etiology is multifactorial, related to genetic susceptibility and exposure to gluten. Exclusive breastfeeding, with its many health benefits, may mitigate or delay symptomatic celiac disease. Because infants with a positive f…

Novel ATP8B1 Gene Mutations in a Child with Progressive Familial Intrahepatic Cholestasis Type 1.

Progressive familial intrahepatic cholestasis (PFIC) is a group of severe genetic disorders, inherited in an autosomal recessive manner, causing cholestasis of hepatocellular origin, later progressing to biliary cirrhosis and liver failure. This is the first report of PFIC type 1 with novel compound heterozygous mutations in Korea. The patient was presented with intrahepatic cholestasis, a normal level of serum γ-glutamyl transferase, steatorrhea, and growth failure. Genetic testing of this patient revealed novel compound heterozygous mutations (p.Glu585Ter and p.Leu749Pro) in the ATP8B1 gene.…
