Acetaminophen improves tardive akathisia induced by dopamine D2 receptor antagonists.

Tardive akathisia is a movement disorder characterized by internal restlessness with an uncontrollable urge to move, leading to repetitive movements. It is a common side effect of long-term treatment with dopamine D2 receptor antagonists. In the present study, we analyzed the FDA Adverse Event Reporting System and IBM MarketScan Research Database to find a drug that can be used concomitantly with dopamine D2 receptor antagonists and still reduce the risk of akathisia. Acetaminophen was determined to be the most effective akathisia-suppressing drug. In an experimental validation of the hypothes

Rett syndrome diagnostic odyssey: Limitations of NextGen sequencing.

Typical (or classic) Rett syndrome (RTT) is an X-linked neurodevelopmental disorder characterized by a period of regression, partial or complete loss of purposeful hand movements, and acquired speech, impaired gait, and stereotyped hand movements. In over 95% of typical RTT, a pathogenic variant is found in the methyl-CPG binding protein 2 gene (MECP2). Here, we describe a young woman with clinically diagnosed typical RTT syndrome who lacked a genetic diagnosis despite 20 years of investigation and multiple rounds of sequencing the MECP2 gene. Recently, additional genetic testing using next-ge

An analogue assessment of repetitive hand behaviours in girls and young women with Rett syndrome.

BACKGROUND: Rett syndrome is a neurodevelopmental disorder that almost exclusively affects females. In addition to neurodevelopmental regression and loss of hand skills, apraxia, deceleration of head growth, and increasing spasticity and scoliosis, a number of behavioural features are also seen, including stereotypic hand movements, hyperventilation and breath holding. The aim of the study was to investigate the extent to which analogue environmental conditions affected the frequency of repetitive hand behaviour in eight girls and young women with Rett syndrome. METHOD: The frequency of repet

Non-Motor Aspects of Tic Disorders-New Developments.

The cardinal characteristics of tic-related disorders are stereotyped motor movements and vocalizations. However, they may be accompanied by non-motor features that appear sequentially during the course of the disorder and can sometimes be more disabling than the tics themselves. This review presents our perspectives on several non-motor aspects of Tourette syndrome based on the long experience of the Neuropsychiatric Tourette Clinic of a tertiary pediatric medical center. The effect of premonitory urges, sensory modulation disorder, tic-related cognitions, and environmental conditions on the

Important role of striatal catalase in aging- and reserpine-induced oral dyskinesia.

Tardive dyskinesia, the most serious iatrogenic movement disorder, has been tentatively associated with nigrostriatal dopaminergic supersensitivity and with oxidative stress. It is also suggested that long-term neuroleptic treatment does not cause oral dyskinesia (OD), but interacts with some substrate of brain aging, resulting in the premature emergence of OD, that can occur spontaneously with aging. In order to investigate a possible role of nigrostriatal dopaminergic supersensitivity and of oxidative stress in aging- and reserpine-induced OD, the stereotyped behavior induced by dopaminergic

Motor stereotypies.

Stereotypic movements are ubiquitous, occur in a variety of forms, and exist in different populations, ranging from individuals with autism to typically developing children. Although such movements are required to be restricted, repetitive, and purposeless, their definition and included activities remain broad and imprecise. Movements are typically classified into 2 groups, primary (physiological) and secondary (pathological), depending upon the presence of additional signs or symptoms. Although some view these movements as behaviors produced to alter a state of arousal, there is increasing ev

Repetitive behaviours in typically developing 2-year-olds.

BACKGROUND: Repetitive behaviours are an essential part of the diagnosis of autism but are also commonly seen in typically developing children. The current study investigated the frequency and factor structure of repetitive behaviours in a large community sample of 2-year-olds. METHODS: A new measure, the Repetitive Behaviour Questionnaire (RBQ-2) was completed by 679 parents. RESULTS: The RBQ-2 had good psychometric properties. A four-factor model provided the best fit for the data, accounting for 51% of the variance, and suggested 4 sub-scales: unusual sensory interests, repetitive motor m

Childhood movement disorders: Clinicoetiological pattern and long-term follow-up at tertiary care center from South India.

OBJECTIVES: Movement disorders are common neurological problems. There is a considerable delay in the diagnosis of movement disorders which indirectly indicates their under-recognition. The studies regarding relative frequencies and their underlying etiology are limited. Describing and classifying them with a diagnosis helps in treating the condition. To study the clinical pattern of various movement disorders in children and to establish their etiology and outcome. MATERIALS AND METHODS: This observational study was conducted in tertiary care hospital from January 2018 to June 2019. Children

An Experimental Study to Assess the Professional and Social Consequences of Tardive Dyskinesia.

OBJECTIVE: Antipsychotic medications may cause tardive dyskinesia (TD), an often-irreversible movement disorder characterized by involuntary movements that are typically stereotypic, choreiform, or dystonic and may impair quality of life. This study evaluated others' perceptions of abnormal TD movements in professional and social situations. METHODS: This was an experimental, randomized, blinded, digital survey in a general population sample. Participants were randomized 1:1 into a test or control group to view a video of a professional actor simulating TD movements or no TD movements prior t

Successful Treatment of Tourette Syndrome With a Combination of Guanfacine and Aripiprazole: A Case Series.

Tourette syndrome (TS) is a disorder of the nervous system that causes motor and vocal tics. Tics occur as sudden onset, rapid stereotyped purposeless movements or sounds. Combination therapies can be utilized for adequate control of motor and vocal tics. Patients diagnosed with TS and treated with aripiprazole and guanfacine from 2011-2022 at Saint Louis University Hospital were retrospectively surveyed. Three patients with TS treated with aripiprazole and guanfacine experienced significant improvement or complete resolution of their motor and vocal tics. In our cohort of three patients, the

The meaning of autistic movements.

What is already known?Moving the body in 'stereotyped', 'repetitive', 'ritualised' or 'unusual' ways is part of the criteria for receiving a diagnosis of autism. However, the reasons for these movements and their personal value are not well understood. Certain ways of moving have become part of a disorder, and have received negative judgements, whereas other movements have not. What this paper adds?We searched online blogs for descriptions of movement written by autistic adults, using their preferred language and definitions. The blog authors said that many types of movement attracted negative

[Stepwise motor rehabilitation of children with neurologic complications of tuberculous spondylitis].

The proposed protocol for postoperative motor rehabilitation of children with tuberculous spondylitis complicated by severe neurological disorders is based on the procedures developed by the authors to induce spinal locomotor activity. The developed algorithm of motor rehabilitation of spinal patients involves separate restoration of gait components (rhythmic stereotypic walking (spinal locomotion), maintenance of a standing posture and voluntary control over the movements ensuring gait. The stages of motor rehabilitation were defined by taking into account these factors and a child's neurolog

Paroxysmal Hemidystonia as the Presenting Manifestation of Previously Undiagnosed Diabetes Mellitus.

BACKGROUND: Hyperglycemia-related movement disorders classically present as hemichorea-hemiballismus, whereas dystonia is far less common and may occasionally precede the recognition of diabetes mellitus. CASE REPORT: A 72-year-old man presented with a 3-month history of recurrent, stereotyped dystonic spells involving the right side of the face, arm, and leg. These episodes occurred approximately twice per minute and lasted about 5 seconds each. Initial laboratory testing revealed a plasma glucose level of 494 mg/dL and an HbA1c of 16.1%, consistent with previously unrecognized diabetes mell

Design and custom fabrication of specialized orthoses for the upper-limb stabilization in childhood dyskinesia.

BACKGROUND: Childhood dyskinesia (CD) is a complex movement disorder with components of dystonic and hyperkinetic nature, characterized by involuntary, sometimes stereotypical postures and gestures that are often impossible to control and hinder the execution of willful motion. The standard orthoses for the treatment of neurological diseases, including CD, are generally poorly differentiated for functional characteristics. The application of similar devices for movement disorders is far less generalized because of the very different symptoms, including the incapacity to control rather than ini

Adult headbanging: sleep studies and treatment.

Headbanging is a rhythmic movement disorder (RMD) along with headrolling, bodyrocking and bodyrolling. The International Classification of Sleep Disorders defines RMD as a group of stereotyped, repetitive movements involving large muscles, usually of the head and neck, that typically occur immediately prior to sleep onset and are sustained into light sleep. The average onset is 9 months, and by 10 years of age the majority of subjects no longer complain of headbanging. If it continues, it is usually associated with mental retardation of autism. Headbanging is said to occur during presleep drow

Clinical assessment of Tourette syndrome and tic disorders.

Tourette syndrome (TS) is a neuropsychiatric disorder involving multiple motor and phonic tics. Tics, which usually begin between the ages of 6 and 8, are sudden, rapid, stereotyped, and apparently purposeless movements or sounds that involve discrete muscle groups. Individuals with TS experience a variety of different sensory phenomena, including premonitory urges prior to tics and somatic hypersensitivity due to impaired sensorimotor gating. In addition to other conditions, stress, anxiety, fatigue, or other heightened emotional states tend to exacerbate tics, while relaxation, playing sport

Periodic limb movements in Parkinson's disease: a critical review of pathophysiology and a framework for clinical management.

Periodic limb movements (PLM) refer to periodic episodes of repetitive and stereotypical limb movements, predominantly affecting the lower extremities. The prevalence of PLM is estimated at 17.6% to 86.7% in patients with Parkinson's disease (PD), which is much higher than 4% to 11% in the general population. PD is a chronic neurodegenerative movement disorder characterized by both motor and non-motor symptoms (NMS), and PLM represents one of the common non-motor manifestations. PLM in PD has garnered increasing attention due to its high prevalence, association with reduced sleep quality, elev
