All-Inside Anterior Cruciate Ligament Reconstruction: A Review of Advance and Trends.

Anterior cruciate ligament (ACL) injury is a common disease in orthopedics and mostly occurs as a noncontact injury in athletes. Patients' knee joint stability, which is crucial to their athletic ability, cannot be restored through conservative treatment; it can only be restored through ACLR (ACL reconstruction) surgery. The surgical techniques of ACLR are constantly evolving, from bone tendon bone (BTB) grafting combined with interface screw fixation to hamstring tendon autograft or allogeneic tendon and of suspension device constructs. In particular, the currently prevalent all-inside techni

Clinical efficacy and outcome of intelligently inflatable reduction in conjunction with percutaneous pedicle screw fixation for treating thoracolumbar burst fractures.

OBJECTIVE: This study was performed to describe a new minimally invasive surgical technique and to explore its effects and practical use in the clinical setting. METHODS: In total, 22 patients with single-segment thoracolumbar burst fractures underwent treatment with an intelligently inflatable reduction device before common percutaneous pedicle screw fixation. Complications were recorded and short-term effectiveness was evaluated using the visual analogue scale (VAS) score for pain, Oswestry Disability Index (ODI), kyphotic Cobb angle, and anterior edge height of the fractured vertebra preop

Large complex odontomas associated with impacted mandibular second and third molars: a case report.

BACKGROUND: Odontomas are the most common odontogenic tumors and are composed of dental tissues such as enamel, dentin and cementum. They are classified into compound and complex types. Compound odontomas usually appear as tooth-like structures in the anterior maxilla, whereas complex odontomas present as irregular masses of dental tissues, most frequently in the posterior mandible. Although odontomas are generally slow-growing and asymptomatic, large lesions can cause jaw expansion, facial asymmetry, and delayed tooth eruption. Various surgical approaches have been described, ranging from int

[On the surgical treatment of pectus excavatum].

80 cases of funnel chest were treated surgically, 9 by Gross procedure, 41 by turn-over technique and 30 by reverse "V" shape osteotomy of the sternum and ribs with internal fixation. There were 56 boys and 24 girls, with ages ranging from 2 to 14 years. The sternal depression varied in depth from 3 to 4 cm, and in volume from 20 to 120 ml. Roentgenographically, there were different extents of deviation of the heart to the left. Some patients had abnormal ECG. To study the results of operations, 48 cases were followed-up for 0.5 to 19 years. We found that the results of reverse "V" shape osteo

New Technique for Mandibular Symphyseal Distraction by a Double-Level Anchorage and Fixation System: Advantages and Results.

INTRODUCTION: A surgical technique to widen the mandible is the mandibular midline distraction: the most common indications for mandibular midline distraction are severe mandibular anterior crowding, severe mandibular transverse deficiency, uni- or bilateral crossbite, impacted anterior teeth with inadequate space, and tipped teeth. Commonly used distraction devices can be divided into 2 systems: bone-borne distraction system appliance, dental-borne distraction systems. Each system has peculiar advantages, disadvantages, and different indications. To combine advantages of both systems we devel

Comparison of the effect of posterior lumbar interbody fusion with pedicle screw fixation and interspinous fixation on the stiffness of adjacent segments.

BACKGROUND: Adjacent segment degeneration could seriously affect the long-term prognosis of lumbar fusion. Dynamic fixation such as the interspinous fixation, which is characterized by retaining the motion function of the spinal segment, has obtained satisfactory short-term effects in the clinical setting. But there are few reports about the biomechanical experiments on whether dynamic fixation could prevent adjacent segment degeneration. METHODS: The surgical segments of all 23 patients were L4/5. Thirteen patients with disc herniation of L4/5 underwent Wallis implantation surgery, and 10 pa

Use of the Ilizarov method to correct lower limb deformities in children and adolescents.

The introduction to the West in the early 1980s of the Ilizarov circular external fixator and method resulted in rapid advances in limb lengthening, deformity correction, and segmental long-bone defect reconstruction. The mechanical features of and biologic response to using distraction osteogenesis with the circular external fixator are the unique aspects of Ilizarov's contribution. The most common indications for children and adolescents are limb lengthening and angular deformity correction. Surgical application and postoperative management of the device require diligent attention to detail

Suspensory Device Fixation of Lisfranc Injuries in a Southeast Asian Urban Population: Patient-Reported Functional Outcomes.

Introduction Open reduction internal fixation (ORIF) and primary arthrodesis are two conventional options for the treatment of Lisfranc injuries. However, they are associated with implant-related complications. An alternative suspensory device construct using interosseous nonabsorbable sutures with endobuttons has been described with satisfactory results. This study aims to explore functional outcomes after suture button fixation of Lisfranc injuries in a Southeast Asian population. Methods This was a single-surgeon retrospective study of patients with Lisfranc injuries treated surgically usin

Nail or plate fixation of intertrochanteric hip fractures: changing pattern of practice. A review of the American Board of Orthopaedic Surgery Database.

BACKGROUND: A new method of fixation for intertrochanteric hip fractures that involves the use of an intramedullary nail that interlocks proximally into the femoral head was introduced in the early 1990s. Anecdotal observation of practice patterns during the Part II (oral) American Board of Orthopaedic Surgery examination suggested that the use of this method had increased substantially in recent years in comparison with the more traditional sliding compression screw technique. A study of the Part II database was undertaken to detect changing patterns of care for intertrochanteric fractures.

Pull Out Suture Technique for Fixing Coronoid Fractures using a Single Lateral Extensor Digitorum Communis Split Approach and Fiberwire Endobutton Fixation by Retrograde Drilling - A Case Series.

INTRODUCTION: Coronoid fractures, while less common than other elbow injuries, present with significant challenges in maintaining elbow stability and function. Restoring the normal anatomy of the coronoid process (CP) is crucial for stabilizing the elbow joint, with surgical intervention often necessary for displaced or comminuted fractures. Suture button fixation offers advantages, such as ease of use, bone-sparing fixation, and reduced reliance on intraoperative X-rays. It is especially effective for small or fragmented fractures, with tools, such as aiming devices and repositioning pliers a

[Clinical classification and surgical correction of pectus excavatum].

We described the experience in clinical classification and surgical correction of 320 cases of pectus excavatum (1984-1990). The patients age ranged from 2 to 14 years. According to the configuration, the deformity was divided into types: extensive type (7.5%); common type (74.3%); localized type (12.5%); mixed type or irregular (5.6%). We found such as four characteristic signs shoulder anteversion, kyphosis, chest excavation and abdominal bulge. Rotation of sternum, costal margin eversion, chest asymmetry and flat chest were also encountered. All these made the correction very difficult. A r

The substantiation of the elastic-viscoplastic model of the human spine for modeling the correction process of kyphoscoliotic deformation.

PURPOSE: The relevance of the problem is caused by an increase in the number of spine-related diseases among children, including scoliosis. Currently, there are no methodologies for the treatment of scoliosis, which ensure an unambiguous positive result. The purpose of the article is to justify the spinal model as an elastic viscoplastic body for further mathematical modeling of the process of spine correction and search for its optimal conditions. METHODOLOGY: The leading approach to the study of this problem is the development of techniques for the surgical treatment of deformities of the v

Medial canthal open nasal fracture repair.

PURPOSE: Open repair of facial fractures may offer better outcomes than closed approaches. Advantages are inherent with direct visualization and the use of fixation devices. Yet, closed approaches are commonly favored for nasal fractures for simplicity, the avoidance of a facial scar, and because exposure to the nasal bones through traditional approaches is limited. Inadequate nasal repair, however, may result in collapsed or crooked noses caused by failure to adequately reduce and fixate fractures. We describe a medial canthal approach that provides optimal bony nasal exposure, minimizes faci

[Surgical-associated expansion of the jaws with asymmetric deformations].

One of the most common abnormalities in the development of the jaw is the narrowing of the alveolar process of the maxilla. In childhood, the elimination of such a deformation is quite common. But with age, the only effective option is the inclusion of a surgical procedure in the rehabilitation plan. The medial and parasagittal osteotomy of the maxilla for the purpose of its expansion by a distraction device has a number of important nuances which can significantly affect the result. These include: localization of sagittal osteotomy of the palate and alveolar process; the direction of cutting

A Novel Device for Closed Reduction and Percutaneous Fixation of Thoracolumbar Fractures.

BACKGROUND/AIM: Open surgical reduction/fixation of thoracolumbar fractures results in significant soft-tissue trauma and related complications. Minimally-invasive technical developments could deliver similar radiological outcomes, while avoiding the related complications. We evaluated radiological and perioperative outcomes in thoracolumbar fractures by using a novel minimally-invasive device. PATIENTS AND METHODS: Twenty-six patients with 29 thoracolumbar fractures using the NForce device were analyzed. Postoperative reduction and alignment were assessed by radiographic measurement of the l

[Humeral lengthening: case report].

INTRODUCTION: Difference in length of upper extremities has mainly esthetic significance and is therefore not so often a subject of operative treatment, compared to lower extremities. CASE OUTLINE: We are presenting a case of a 16-year-old patient in whom a shortening of 9 cm of the right humerus was determined at the end of growth. This shortening was the result of surgical treatment of solitary bone cyst at the proximal end of the humerus done at the age of 10 years. In order to correct the length of the humerus we applied distraction osteogenesis with a compressive-distracting device accor

Sacroiliac stabilization for sacral metastasis: A case series.

BACKGROUND: The sacrum is a rare location for spinal metastasis. These lesions are typically large and destructive by the time of diagnosis, making treatment difficult. When indicated, surgical stabilization offers pain relief and preserves independence in patients with impending and acute pathological sacral fractures. CASE DESCRIPTION: Three consecutive patients presented with sacral metastases. After either failing radiation therapy or presenting with acute fracture and instability, the patients underwent intralesional excision, bilateral L4 to ilium fusion with instrumentation, and sacroi
