The management of syncope in older adults.

Syncope is a frequent symptom in older patients. The diagnostic and therapeutic management may be complex, particularly in older adults with syncope and comorbidities or cognitive impairment. Morbidity related to syncope is more common in older persons and ranges from loss of confidence, depressive illness and fear of falling, to fractures and consequent institutionalization. Moreover, advan-ced age is associated with short and long-term morbidity and mortality after syncope. A standardized approach may obtain a definite diagnosis in more than 90% of the older patients with syncope and may red

A novel device in evaluating syncope.

Syncope may be a manifestation of many diseases. The etiology is often difficult to determine. Much effort, time, and expense have been required to determine a diagnosis. A thorough history and physical examination are essential (Table 3). Several clinical pearls may be of help in the diagnosis of syncope: (a) the most important elements in the evaluation of syncope are a detailed history and physical examination; (b) syncope is a common problem in young healthy adults and the elderly; (c) a heavy meal is a specific cause of syncopy in the elderly (postprandial), however this etiology often go

[Syncopes from the neurologic viewpoint].

A syncope is defined as a sudden, temporary loss of consciousness, associated with loss of postural tone with spontaneous recovery. The incidence is high and the differential diagnosis broad; therefore, the first observations are essential for the management of the patient. In this review diseases will be described which manifest themselves with syncopes that fall under the auspices of either internal medicine or neurology. First, the etiologies of syncopes are discussed in the strict sense of the word, i.e. due to a global cerebral ischemia such as in orthostatic hypotension and vasopressor s

[Evaluation of syncope in medicolegal expertise].

The authors summarise the clinical characteristics of the syncope and neurological signs respectively caused by basilar vertebral artery insufficiency as well as carotid sinus hypersensitivity. Several authors have seen an increased number of patients with neurological symptoms caused by ischaemia in the region of the basilar artery occurring while driving and causing a traffic accident. Carotid sinus hypersensitivity as cause of driving accidents has much less importance. In the case of the authors on a 51 years old man after turning his head to the right a sudden transient loss of consciousn

[Syncopes in patients of advanced age].

This review covers the various etiologies and therapeutic possibilities of syncopes in the elderly. The importance of normal blood pressure and the danger of hypotension are stressed: The frequently occurring vascular stenoses may become especially dangerous if pre-stenotic pressure drops as a consequence of orthostatic dysregulation or of inadequate treatment of hypertension. Orthostatic dysregulation--a common phenomenon in old age--is possibly due to a diminished activity of the baroreceptor reflexes or to the pooling of blood in the venous system and/or to the diminution of blood volume by

A clinical study of convulsive syncope.

An analysis of the clinical features of patients having evidence of a syncopal onset to convulsive episodes is reported. The range of convulsive phenomena was quite wide and did not appear to differentiate the syndrome from primary epilepsy in terms of seizure morphology, occurrence of incontinence or pattern of post-ictal recovery. It is therefore suggested that the premonitory and precipitating features are more specific in the diagnosis of convulsive syncope. Skin pallor was often reported during attacks. Although EEG abnormalities were seen in some cases, the follow-up results indicate a l

[Diagnosis of syncopes in suspected arrhythmias].

Several mechanisms lead to attacks with unsuspected sudden and intermittent loss of consciousness. A major cause for such syncopes are arrhythmias. Only in rare cases it is possible to register an ECG during a typical attack despite many newer methods as long-term ECG (LT-ECG), exercise stress test and electrophysiologic investigations. LT-ECG does not record the ECG only during symptomatic periods (syncopes, dizziness, palpitations etc), but also registers asymptomatic AR, which can be precursors of SY. Carotid sinus massage is a valuable tool for the detection of a cardio-inhibitory Carotid-

Syncope and episodic loss of consciousness.

Syncope is a common problem for which accurate diagnosis is the key to effective therapy. A detailed description of each of the syncopal spells with attention to the aura, association with effort, patient position at the onset of syncope, description of ictal events from witnesses, and detailed description of the postictal state will permit accurate diagnosis and appropriate therapy.

Syncope or seizure: that is the question-case report of a young patient with convulsive cardioinhibitory syncope treated with cardioneuroablation.

BACKGROUND: Differentiation of syncope from seizure is challenging and has therapeutic implications. Cardioinhibitory reflex syncope typically affects young patients where permanent pacing should be avoided whenever possible. Cardioneuroablation may obviate the need for a pacemaker in well-selected patients. CASE SUMMARY: A previously healthy 24-year-old woman was referred to the emergency department after recurrent episodes of transient loss of consciousness (TLOC). The electrocardiogram (ECG) and the echocardiogram were normal. An electroencephalogram (EEG) showed intermittent, generalized

Psychiatric profile, quality of life and risk of syncopal recurrence in patients with tilt-induced vasovagal syncope.

AIM: The aim of this study was to investigate the prevalence of psychiatric disorders and quality of life (QoL) in patients with tilt-induced vasovagal syncope and no other comorbidities, and their relationship with the syncopal burden and the risk of recurrence. METHODS: We studied 61 patients with recurrent syncope and positive tilt testing. Controls consisted of 61 sex- and age-matched healthy subjects. Psychiatric diagnoses were formulated on the basis of a structured interview and the Minnesota Multiphase Personality Inventory-2 questionnaire. QoL was assessed by means of the Short-Form

Outcome of an integrated approach to the investigation of dizziness, falls and syncope in elderly patients referred to a 'syncope' clinic.

Sixty-five consecutive elderly patients (mean age 78 years) referred to a 'syncope' clinic over a six-month period were prospectively studied. Initial evaluation included ambulatory electrocardiography, carotid sinus massage before and after atropine and prolonged head-up tilt. Diagnostic criteria for causes of syncope were assigned at the beginning of the study. Overall, a diagnosis was attributed to symptoms in 92% of patients; overlap was present in a quarter. Diagnoses were cardioinhibitory carotid sinus syndrome (CSS; 5%), vasodepressor CSS (26%), mixed CSS (14%), orthostatic hypotension

Adams-Stokes attacks associated with carotid sinus syncope. Pathogenesis and therapy of the carotid sinus syncopes.

The pathophysiology and therapy of the carotid sinus syncope were studied by means of electrophysiological investigations and clinical observations in 84 patients suffering from carotid sinus syncope. The most important data and conclusions of the study are the following: 1. The most frequent cause of the carotid sinus syncope (Adams-Stokes) attack is the sinus arrest and a sinus block. 2 degrees-3 degrees av block can be observed less frequent in the course of syncope attack. 2. In a patient suffering from carotid sinus syncopes, disorders of impulse formation and impulse conduction of the he

Brain MRI and EEG overemployment in patients with vasovagal syncope: results from a tertiary syncope unit.

BACKGROUND: The diagnosis of vasovagal syncope (VVS) is mainly based on history-taking and physical examination. However, brain Magnetic Resonance Imaging (MRI) and Electroencephalogram (EEG) are commonly used in the diagnostic course of VVS, despite not being indicated in the guidelines. This study aims to find the possible associated factors with the administration of brain MRI and EEG in patients with VVS. METHODS: Patients with a diagnosis of VVS from 2017 to 2022 were included. Several demographic and syncope features were recorded. The association of these was assessed with undergoing M

Role of extended external loop recorders for the diagnosis of unexplained syncope, pre-syncope, and sustained palpitations.

AIMS: To assess the diagnostic yield of new external loop recorders (ELRs) in patients with history of syncope, pre-syncope, and sustained palpitations. METHODS AND RESULTS: Since 2005, we have established a registry including patients who consecutively received ELR monitoring for unexplained syncope or pre-syncope/palpitations. The registry included 307 patients (61% females, age 58 ± 19 years, range 8-94 years) monitored by high-capacity memory ELR of two subsequent generations: SpiderFlash-A(®) (SFA(®), Sorin CRM), storing two-lead electrocardiogram (ECG) patient-activated recordings by lo
