The Independent Effect of Nerve Injury and Tendon Injury on Rotator Cuff Fatty Infiltration.

BACKGROUND: Muscular fatty infiltration (FI), atrophy, and fibrosis commonly occur after chronic rotator cuff tear and are barriers to successful rotator cuff repair. It is unclear if these phenomena result from tendon injury alone, neurological injury, or a combination. PURPOSE: To determine the role of suprascapular nerve mediated FI with and without chronic rotator cuff tear in a rabbit model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 22 rabbits underwent bilateral shoulder procedures. Extremities were randomized to surgical procedures: suprascapular neurotomy, supra

Comparing the outcomes of fingertip-to-palm and fingertip-to-forearm two-stage flexor tendon reconstruction for isolated flexor digitorum profundus tendon injuries.

Flexor tendon injuries of the hand have devastating consequences when primary tendon repair fails or left untreated in the first place. Flexor tendon reconstruction is a substantial treatment option to obtain functional digit. In this study, we aimed to compare the functional outcomes and technical feasibility of fingertip-to-palm and fingertip-to-forearm tendon reconstruction methods. Thirty-five patients were divided into two groups according to the proximal attachment site of the free tendon grafts. Group I consisted of 18 patients whose tendon grafts were placed from fingertip-to-palm (zon…

Musculoskeletal disease and injury, now and in the future. Part 2: Tendon and ligament injuries.

Tendon and ligament injuries continue to be one of the most frequent causes of orthopaedic injury and early retirement in the Thoroughbred racehorse. In the UK, the Horserace Betting Levy Board has invested considerable research funding in the last decade in order to investigate aspects of tendon epidemiology, biology and therapy, in order to decrease both the welfare and economic consequence of such injuries. This review summarises key aspects of this research effort and identifies specific research directions for the future.

Ankle Syndesmotic Injury With Posterior Tibial Tendon Rupture.

Isolated ankle syndesmotic injuries are uncommon and are typically discussed in the context of athletic high ankle sprains; however, concomitant rupture of the posterior tibial tendon represents an exceedingly rare injury pattern. This case highlights the importance of maintaining a high index of suspicion for associated soft-tissue pathology in patients presenting with syndesmotic instability in the absence of fracture. This case underscores the need for comprehensive clinical and imaging evaluation of isolated syndesmotic injuries and supports primary tendon repair when rupture is identified

Extensor Tendon Repair Outcomes Based on Zone of Injury.

BACKGROUND: The purpose of this study was to examine the clinical outcomes of extensor tendon repairs based on zone of injury. METHODS: A retrospective chart review was conducted on all patients who underwent primary extensor tendon repair at our institution. Extensor tendon repairs were performed using a multiple figure-of-8 suture technique for extensor zones 1-4 and a modified Kessler suture technique for extensor zones 5-8. Inclusion criteria included a minimum of 8 weeks of follow-up, complete data available for review, and extensor tendon injury requiring primary surgical repair. RESUL

Injuries and Conditions Affecting the Elbow Flexor/Pronator Tendons.

Common flexor-pronator tendon injuries and medial epicondylitis can be successfully treated nonoperatively in most cases. Operative treatment is reserved for patients with continued symptoms despite adequate nonoperative treatment or in high-level athletes with complete rupture of the common flexor-pronator tendon. The physical examination and workup of patients with flexor-pronator tendon injuries should focus on related or concomitant pathologies of the medial elbow. The gold standard for surgical treatment of flexor-pronator tendon ruptures or medial epicondylitis includes tendon debridemen…

Ulnar Nerve Injury after Flexor Tendon Grafting.

A 43-year-old female is presented who underwent a two-stage tendon reconstruction and developed a low ulnar nerve palsy postoperatively. Exploration found that the tendon graft was passing through Guyon's canal and that the ulnar nerve was divided. This is a previously unreported complication. The reconstruction is discussed, the literature reviewed and a guide is given on how to identify the correct tissue plane when passing a tendon rod.

Acute finger injuries: part I. Tendons and ligaments.

Improper diagnosis and treatment of finger injuries can cause deformity and dysfunction over time. A basic understanding of the complex anatomy of the finger and of common tendon and ligament injury mechanisms can help physicians properly diagnose and treat finger injuries. Evaluation includes a general musculoskeletal examination as well as radiography (oblique, anteroposterior, and true lateral views). Splinting and taping are effective treatments for tendon and ligament injuries. Treatment should restrict the motion of injured structures while allowing uninjured joints to remain mobile. Alt

Early cellular response in tendon injury: the effect of loading.

The effect of loading on the early cellular response to tendon injury was studied in a partial tenotomy (window) model in the extensor digitorum longus of the rat. Normal use of the limb was allowed, such that tendons were either loaded (group 1) or unloaded (group 2) when a distal tenotomy was performed. Thirty-four male Fischer rats were used. The tendons were harvested at intervals of 6 hours and 1, 3, 5, and 7 days. Quantitative cell counts were performed on light microscopic cross-sections of the window and tendon substance together with recording of cell orientation. Matrix changes in th

Acute injuries of the extensor tendons proximal to the metacarpophalangeal joints.

The results of acute repair of the extensor tendons proximal to the metacarpophalangeal joint vary with the degree of associated injuries. Shortening should be kept to a minimum at the time of repair. The Kleinert modification of the Bunnell technique affords the greatest tensile strength. A 3-0 or 4-0 nonabsorbable suture on a small tapered needle should be used. The extensor retinaculum should be resected or transposed for injuries in zones VII and T V. Sensory branches of the ulnar and radial nerves should be repaired primarily, if possible. The most frequent complication is loss of metacar

Chronic ankle instability as a cause of peroneal tendon injury.

Chronic lateral ankle pain and instability can be associated with a split in the peroneus brevis tendon. This case reports a peroneus brevis split that was centered over the sharp posterior edge of the fibula and was associated with laxity of the superior peroneal retinaculum, and chronic ankle instability. The mechanism of the split was easily demonstrated during surgery by everting the foot and pulling longitudinally on the peroneus longus tendon. The interrelationship of lateral ankle instability with superior retinacular laxity and resultant peroneus brevis splits can account for posttraum

[Injuries of the flexor and extensor tendons of the hand and forearm and their treatment].

The general principles of treatment of injuries of the flexor and extensor tendons in the hand and distal forearm are mentioned. For the extensor tendons the different types of treatment performed for injuries in the different areas, the finger, the back of the hand, and the wrist, are briefly discussed. Knowledge recently acquired from experimental and clinical studies, especially that of the anatomy, the blood supply, the nutrient pathways, and the healing of the flexor tendons, is discussed, and its applications in our daily clinical work are shown.

Retardation of finger growth after injury to the flexor tendons.

The absence of any part of the total mechanical force (i.e., flexor tendons) may be responsible for retardation of bone growth during a child's growth phase. The cause of retardation may be dedifferentiation of cartilage cells at the epiphyseal plate. Four cases in which unrepaired flexor tendons in digits caused retardation of growth illustrate this possible phenomenon. We feel that this is not a well recognized problem and thus deserves mention to stimulate further study. We believe that meticulous primary repair in children is desirable not only to preserve function, but also to prevent gro

[Ambulatory treatment of extensor-tendon injury in general surgery].

Extensor tendon injuries are mostly treated by younger general surgeons. Good follow-up results in the zone 2, 3, 4 and 6 of VERDAN are observed only 6 to 17 percent (fig. 1), which may be explained by: 1. the complicated anatomy of the extensor tendon in zone 2 to 4, 2. lack of attention to handsurgical principles, 3. infections in up to 40 per cent, which appear especially in connection with extensive soft tissue damage, 4. posttraumatic arthrosis (fig. 3) caused by lesions of the finger-joints or temporary KIRSCHNER wire fixation. The highest restriction of movement is found in the finger-j…

Recent progress in macrophage-mediated tendon injury and healing.

Recently, the role of macrophages in tendon repair has received increased attention. These cells are versatile, playing multiple roles, such as defending the host, engulfing debris, producing growth factors, and releasing substances that can both promote and alleviate inflammation. Within the scope of tendon repair and tendinopathy resolution, macrophages are essential contributors. However, the current understanding of macrophage involvement in tendon healing remains limited. Hence, understanding macrophages' impact on tendon healing is of considerable importance for devising innovative treat
