BACKGROUND: Relapses following nonoperative treatment for clubfoot occur in 29% to 37% of feet after initial correction. One common gait abnormality is supination and inversion of the foot caused by an imbalance of the anterior tibialis tendon muscle. The purpose of this study was to determine if plantar pressures are normalized following an anterior tibialis tendon transfer (ATTT).
METHODS: Thirty children (37 clubfeet) who underwent an ATTT, were seen for plantar pressure testing preoperatively and postoperatively. Each foot was subdivided into 7 regions: medial/lateral hindfoot and midfoot

BACKGROUND: Tendon transfers are commonly used for correction of pathology or deformity of the foot and ankle. Bioabsorbable implants have been developed as an alternative to metal interference screws. The purpose of this study was to document complications following tendon transfers of the foot and ankle using bioabsorbable poly-L-lactide interference screws.
METHODS: A retrospective chart review was used to identify patients in whom either of the 2 senior authors had performed a tendon transfer of the foot and ankle using a bioabsorbable interference screw between 1999 and 2005. A minimum o

Although knee dislocations are relatively rare injuries; associated drop foot complication as a consequence of common peroneal nerve palsy (CPN) is substantially high. Even after successful ligament constructions; unresolved CPN palsy is a factor contributing to bad outcome after knee dislocations. CPN palsy is seen more after open dislocations, rotatory dislocations, and especially in patients with posterolateral corner injuries. CPN palsy can readily be diagnosed clinically, although high index of suspicion is needed. Surgical exploration in the acute setting is still debatable. Conservative

OBJECTIVES: We evaluated the effectiveness of extramembranous transfer of the tibialis posterior (TP) tendon for the treatment of drop foot deformity.
METHODS: The study included 13 patients (6 females, 7 males; mean age 30 years; range 10 to 46 years) who underwent 15 tendon transfers for drop foot deformity. Ten patients (76.9%) had deformity due to unrepairable nerve injuries, which were associated with surgical procedures in six patients and trauma in the remaining four. In four feet (26.7%), the TP tendon was turned from the intero-anterior aspect of the tibia and fixed by tenodesis to t

Children with brachial plexus birth palsy may have permanent loss of shoulder external rotation strength. This impairment may result to a difficulty in reaching the face and head with the affected hand for grooming activities, and in reaching overhead for participation in sports or work-related tasks. In addition, the contracture that results from unopposed internal rotation may further restrict range of motion and cause glenohumeral joint deformity and subluxation.A combination of muscle release and transfers reliably improves the child's ability to position the hand, and may halt the develop

Strength and stability of the shoulder girdle affect function of the entire upper extremity. Isolated muscle weakness may be compensated for by adjacent muscles; however, profound weakness of the deltoid muscle, for example, affects not only placement of the extremity, but also strength and endurance of the forearm and hand. Orthotic substitutes for weakened muscles are not successful. If the muscle weakness is nonprogressive and stable, then surgical treatment will improve glenohumeral stability, increase endurance, and provide more uniform strength for many activities of daily living.

Transfer of the subscapularis tendon using a slightly modified Magnuson-Stack procedure was done in 154 shoulders with recurrent anterior dislocation. A follow-up to two to twenty-two years (average, 7.6 years) revealed only three recurrences of the dislocation and no serious complications.

OBJECTIVES: Restoration of extension in the metacarpophalangeal joints of the fingers as well as in the interphalangeal joint of the thumb by transfer of the superficial flexor tendons of the long and ring fingers (flexor digitorum superficialis III and IV).
INDICATIONS: The indications for surgery are substantial loss and palsy of muscles innervated by the radial nerve and its roots.
CONTRAINDICATIONS: The procedure is contraindicated by reversible radial palsy, palsy or substantial loss of flexors, limited passive mobility due to contracture, ankylosis or instability of the affected joints

BACKGROUND: The study aimed to compare the outcomes of combined calcaneocuboid arthrodesis and split anterior tibialis tendon transfer (SPLATT) procedure to isolated SPLATT surgery for the treatment of the spastic equinovarus deformity in children with cerebral palsy (CCP).
METHODS: Forty-one ambulatory CCP with 56 equinovarus feet, with positive flexor withdrawal reflex test results, were studied. The average age was 9.1 ± 3.2 years (range 3-22). Patients were assigned into 2 groups based on the surgical procedures. Patients in group 1 underwent isolated SPLATT surgery, whereas patients in g

Background: Camitz opponensplasty is a widely used reconstruction procedure for severe carpal tunnel syndrome. However, it has several drawbacks, including poor pronation during thumb opposition and a tendency for flexion at the thumb metacarpophalangeal (MP) joint. We developed a novel procedure to overcome these issues while preserving its advantages. Methods: Our procedure involves mini open carpal tunnel release (KnifeLight) and the transfer of the palmaris longus (PL) tendon to the rerouted extensor pollicis brevis (EPB) tendon using the distal part of the PL tendon as a pulley. We assess

Recurrent rotator cuff tears in young patients represent a complex therapeutic challenge. Latissimus dorsi tendon transfers (LDTT) are considered an effective treatment option for massive tears with deficiencies of the posterosuperior rotator cuff. However, the procedure can still produce complications, among which neurological complications are rare, particularly those involving the axillary nerve. This review presents the case of a 53-year-old male patient with an extensive, irreparable posterosuperior rotator cuff retear who underwent arthroscopically assisted LDTT without intraoperative co

BackgroundChronic Achilles tendon rupture (CATR) is a debilitating condition affecting activities such as walking and jumping. Surgical reconstruction remains preferred for larger defects, with local tendon transfers including the flexor hallucis longus (FHL) and peroneus brevis (PB). However, these techniques carry risks of donor-site morbidity and neurovascular injury, particularly in high-risk patients.ObjectivesThis article describes a novel minimally invasive endoscopic technique and reports 3 cases of CATR reconstruction using peroneus longus (PL) tendon transfer as an alternative.Method

Various surgical techniques for reconstruction of chronic Achilles tendinosis have been developed, including free semitendinosus tendon autograft and allograft for larger Achilles tendon defects. Flexor hallucis longus (FHL) transfer has emerged as an effective treatment option for patients with insertional Achilles tendinopathy (IAT). During FHL transfer, FHL tendon is harvested at its distal part and rerouted to augment or replace the diseased portion of the Achilles tendon, providing improved structural integrity and function, but the necessity of reattachment of the Achilles stump in FHL t

The intramuscular neurovascular branching pattern of the latissimus dorsi muscle provides 2 parts with independent neurovascular supply. The medial and lateral branches arising from the thoracodorsal pedicle run parallel to the muscle fibers and, therefore, 2 independent motor flaps can be harvested. We propose a modification to the latissimus dorsi tendon transfer for external rotation based on these 2 muscular parts. The medial (transverse) part is transferred to the rotator cuff insertion to dynamically enhance external rotation, and the lateral part acts as a stabilizer of the humeral head

PURPOSE: This study aimed to introduce a novel technique using the extensor pollicis brevis and extensor indicis proprius tendons as power sources for thumb opposition reconstruction in cases of severe carpal tunnel syndrome (CTS) associated with thenar muscle atrophy. Furthermore, the efficacy of this novel method and the traditional Camitz technique was compared.
METHODS: Patients with severe CTS and thumb opposition dysfunction who underwent surgery using the novel technique (n = 7 and 9 surgeries) or the Camitz technique (n = 8 and 8 surgeries) were included in the analysis. The pre- and
