Terror-related injuries: a comparison of gunshot wounds versus secondary-fragments-induced injuries from explosives.

BACKGROUND: Terror-related injuries caused by secondary fragments (SF) from explosive devices show a distinctive pattern in severity, distribution, outcomes of assault, and health-system resource use as compared with terror-related penetrating injuries caused by gunshot wounds. STUDY DESIGN: A case-comparison study conducted in a tertiary university hospital and the only Level I trauma center in the Jerusalem vicinity. During a period of 4 years, over 1,500 casualties of terror-related injuries were treated in one Level I trauma center. The study included 533 patients who were admitted for ho

Night terrors, sleepwalking, and confusional arousals in the general population: their frequency and relationship to other sleep and mental disorders.

BACKGROUND: Arousal parasomnias (night terrors, sleepwalking, and confusional arousals) have seldom been investigated in the adult general population. Clinical studies of parasomnias, however, show that these disorders may be indicators of underlying mental disorders and may have serious consequences. METHOD: A representative sample of the United Kingdom population (N = 4972) was interviewed by telephone with the Sleep-EVAL system. RESULTS: Night terrors were reported by 2.2% (95% CI = 1.8% to 2.6%) of the sample, sleepwalking by 2.0% (1.6% to 2.4%), and confusional arousals by 4.2% (3.6% to

Particle analysis of surgical lung biopsies from deployed and non-deployed US service members during the Global War on Terrorism.

The role that inhaled particulate matter plays in the development of post-deployment lung disease among US service members deployed to Southwest Asia during the Global War on Terrorism has been difficult to define. There is a persistent gap in data addressing the relationship between relatively short-term (months to a few years) exposures to high levels of particulate matter during deployment and the subsequent development of adverse pulmonary outcomes. Surgical lung biopsies from deployed service members and veterans (DSMs) and non-deployed service members and veterans (NDSMs) who develop lun…

The state of US trauma systems: public perceptions versus reality--implications for US response to terrorism and mass casualty events.

BACKGROUND: Injury has long been identified as the number one killer of Americans under the age of 34, and establishment of regional trauma systems and centers incorporating primary, secondary, and tertiary care and injury-prevention strategies has proved to be a vital element in reducing injury-related sequelae, deaths, and even costs. Despite these facts, trauma system development has not been given priority for funding in many local and state governments and only intermittently at the federal level. Consequently, many of the nation's trauma centers are strapped for funds to provide emergenc

Longitudinal PTSD trajectories before and after the October 7, 2023, terror attacks: A nationwide study of Israeli adults.

BACKGROUND: Existing research on post-traumatic stress disorder (PTSD) development over time covers brief periods predominantly among military personnel, rather than civilians, and baseline measurements from before traumatic experiences are rarely available. This longitudinal study examined PTSD trajectories among Israeli civilians before and after the October 7, 2023, terror attack, exploring their associations with current and previous trauma. METHODS: Data included 1,231 Israeli Jewish adults from a quasi-representative sample surveyed at four time points: January 2018, April 2022, Decembe

Direct evidence of trophic interaction between a crocodyliform and a large terror bird in the Middle Miocene of La Venta, Colombia.

Direct evidence of predation and other trophic relationships provide valuable information about trophic interactions between species in palaeo-communities. Data on ecological interactions amongst extant apex predators open a unique opportunity to better understand how sympatric apex predators coexisted or interacted with each other in the past. Here, we describe direct evidence of a predation or scavenging event in which we hypothesize that a medium-sized caiman (possibly Purussaurus neivensis) consumed (either through scavenging or through direct predation) a large terror bird. The distal par

Chronic pain among the hospitalized patients after the 22 July 2011 terror attacks in Oslo and at Utøya Island.

BACKGROUND: On 22 July 2011, 48 people were hospitalized due to physical injuries from gun shots or explosion, following 2 terror attacks in the Oslo area, Norway. In this study, we have investigated the occurrence of chronic pain, the severity and consequences of chronic pain in these patients, 3 to 4 years after the incidents. METHODS: Totally 43 eligible terror trauma patients were invited to participate in the study, 30 patients were included. They underwent a consultation with a psychologist and a physician; containing psychological assessment, neuropsychological screening, a standardize

Evoked Death-Related Thoughts in the Aftermath of Terror Attack: The Associations Between Mortality Salience Effect and Adjustment Disorder.

ICD-11 has provided a revised definition for adjustment disorder (AjD). The current study examined whether mortality salience effect, a possible consequence of a terror attack, may serve as a significant predictor associated with each of the AjD subscales. Using an online survey, 379 adult participants were recruited and filled out self-reported questionnaires dealing with adjustment disorder symptoms as well as mortality salience effect. Findings revealed that mortality salience effect was a significant predictor of all AjD subscales. The importance of mortality salience effect for AjD is dis

Growing Up after Terrorism: Parental Psychopathology and Adult Offspring PTSD and Emotional Disorders: A Long-Term Study in a Spanish Sample.

The present study investigates whether parental psychopathology developed after a terrorist attack influences the development of disorders in their children in the very long term, with a mean time of 24.87 years (SD = 8.5) having elapsed between the event and the assessment. For this purpose, participants included 66 adults (Mage = 32.17, SD = 7.25; 59.1% female) who were minors at the time of the attack (Mage = 5.92, SD = 4.98) and 67 parents (Mage = 58.82, SD = 7.86; 62.7% female). We examined a possible relationship between the development of parents' psychopathology after suffering the att

Assessing risks of violent extremism in depressive disorders: Developing and validating a new measure of Sympathies for Violent Protest and Terrorism.

OBJECTIVE: Clinicians are tasked with assessing the relationship between violence and mental illness. Yet, there is now a legal expectation in some countries that public services, including health professionals, assess risk of violent extremism - with few available measures. We previously developed a new measure of Sympathies for Radicalisation, using items that measure Sympathies for Violent Protest and Terrorism. In this paper, we present the detailed psychometric properties of a reduced item measure of Sympathies for Violent Protest and Terrorism. We use data from two studies to test replic…

A generalized linear mixed models approach for detecting incident clusters of disease in small areas, with an application to biological terrorism.

Since the intentional dissemination of anthrax through the US postal system in the fall of 2001, there has been increased interest in surveillance for detection of biological terrorism. More generally, this could be described as the detection of incident disease clusters. In addition, the advent of affordable and quick geocoding allows for surveillance on a finer spatial scale than has been possible in the past. Surveillance for incident clusters of disease in both time and space is a relatively undeveloped arena of statistical methodology. Surveillance for bioterrorism detection, in particula…

Longitudinal Associations of Past and Cumulative Terrorism Exposure with Community Violence Exposure Among Israeli Adolescents: Aggression and Severe Violence Commission as Moderators.

Terrorism exposure may be longitudinally associated with adolescents' community violence exposure, but these associations may depend on preexisting externalizing behavior. Guided by ecological-transactional theory and competing stress-diathesis, dual-vulnerability, and rival-vulnerability perspectives, this study examined whether aggression and severe violence commission moderated associations of past terror exposure and cumulative Kassam rocket exposure, referring to exposure to short-range rockets launched from the Gaza Strip toward southern Israel, with Time 2 violence witnessing and victim…

The differential effects of intimate terrorism and situational couple violence on mental health outcomes among abused Chinese women: a mixed-method study.

BACKGROUND: Distinctions have been made between the two main forms of intimate partner violence: intimate terrorism (IT) and situational couple violence (SCV), depending on whether the violence is part of a general pattern of control. Differential effects also exist between IT and SCV. However, the IT/SCV distinction and their differential effects have yet to be demonstrated in violent intimate relationships in China. We aimed to identify IT and SCV among Chinese women who reported partner violence in Hong Kong and to differentiate the effects of IT and SCV on their mental health outcomes. ME…

Complexity of exposure to mass-casualty conflict and terror stress: A population study following a major civilian targeted event.

OBJECTIVES: The October 7, 2023 events exposed both Israelis, Palestinians, and the entire middle east to unprecedented complex trauma. Guided by theoretical considerations and expert panel discussions, we identified seven distinct exposure domains: missile attacks, physical violence, evacuation, combat participation, hostage involvement, media exposure, and group-based marginalization. We examined actual exposure, that of self and significant others, to these domains alongside subjective stress and major stressful life events. Outcomes included PTSD, depression, anxiety, and somatic symptoms.

When Terror and War Supersede Cancer: A Case Study of a Ukrainian Cancer Survivor and Application of Trauma-Informed Care.

This case study describes the implementation of trauma-informed and culturally informed cancer care, for a 40-year-old Ukrainian female with recurrent follicular lymphoma, displaced to the United States in the Spring of 2022 following the Russian invasion of Ukraine. The patient was referred to the psychosocial oncology service at an NCI-designated Comprehensive Cancer Center for evaluation and treatment of depression and anxiety. Stressors, along with the chronic trauma of fearing for her family's safety and the stress of being displaced from Ukraine, compounded her emotional burden. A modifi

Three cases of parasomnias similar to sleep terrors occurring during sleep-wake transitions from REM sleep.

UNLABELLED: Sleep terrors are a non-rapid eye movement (NREM) sleep-related parasomnia characterized by abrupt terror with a frightening scream. However, it remains unknown whether sleep terrors occur only from NREM sleep. We evaluated 3 cases of a sleep terrors-like parasomnia that occurred exclusively during arousals from rapid eye movement (REM) sleep. These parasomnia events occurred at REM sleep-wake transitions, manifesting with screaming or crying, similar to sleep terrors, without alertness or clear orientation. The patients were all young-adult females without notable medical conditio

Factors associated with PTSD and partial PTSD among first responders following the Paris terror attacks in November 2015.

During the evening of 13 November 2015, the deadliest terror attacks in France in recent times occurred in the Paris area. Overall, 130 people were killed, 643 were physically injured and several thousands were psychologically impacted. Thousands of first responders, including health professionals, firefighters, affiliated volunteers and police officers were mobilized that night and during the subsequent weeks. The aims of our study were to measure the psychological impact on first responders in terms of post-traumatic stress disorder (PTSD) and partial PTSD as well as associated factors 12 mo

Lessons From Analyzing the Medical Costs of Civilian Terror Victims: Planning Resources Allocation for a New Era of Confrontations.

UNLABELLED: Policy Points: Across the globe, the threat from terrorist attacks is rising, which requires a careful assessment of long-term medical support. We found 3 major sources of costs: hospital expenditures, mental health services dedicated to acute stress reactions, and ambulatory follow-up. During the first year, most of the costs were related to hospitalization and support for stress relief. During the second year, ambulatory and rehabilitation costs continued to grow. Public health specialists should consider these major components of costs and their evolution over time to properly a

Just a scary dream? A brief review of sleep terrors, nightmares, and rapid eye movement sleep behavior disorder.

The clinical spectrum of sleep disorders in children is broad, ranging from primary snoring and obstructive sleep apnea (OSA) syndrome to complex sleep-related behaviors and movement disorders. Although snoring and OSA typically receive significant attention and discussion, other biologically based sleep disorders are as common, if not more common, in children. A general pediatrician is frequently presented with the complaint of sleep talking, sleep walking, or abnormal movements during sleep. Even more alarming is the presentation of the child suddenly and explosively screaming during sleep.
