Tarsal tunnel syndrome (TTS) is a relatively uncommon nerve entrapment neuropathy. Many pathologies are reported as possible causes for TTS. The diagnosis of TTS can be difficult and often missed. We present a rare case of TTS due to an accessory flexor digitorum longus muscle. Together with a high index of suspicion, MRI is the investigation of choice in making the diagnosis. These patients are best managed with excision or transposition of the flexor digitorum accessorius longus (FDAL) and neurolysis of the posterior tibial nerve and its branches.

Peroneal nerve palsy with resultant foot drop has significant impacts on gait and quality of life. Traditional management includes ankle-foot-orthosis, tendon transfer, and arthrodesis-each with certain disadvantages. While nerve transfers for peroneal nerve injury have been reported in adults, with variable results, they have not been described in the pediatric population. We report the use of partial tibial nerve transfer for foot drop from deep peroneal nerve palsy in three pediatric patients. The first sustained a partial common peroneal nerve laceration and underwent transfer of a single

INTRODUCTION/AIMS: There is limited data on the sensitivity and responsiveness of high-resolution imaging techniques in the longitudinal assessment of hereditary neuropathies. In this study, our aims were to investigate the ability of ultra-high field magnetic resonance imaging to detect longitudinal changes in the peripheral nerves of Charcot-Marie-Tooth (CMT) 1A patients, and to evaluate the potential benefits of doing so at the nerve fascicle level.
METHODS: We performed magnetic resonance imaging (MRI) to simultaneously obtain high-resolution anatomical and quantitative data at ultra-high

PURPOSE: To assess the risk of damage to the popliteal neurovascular structures when inserting the needle through the posterior aspect of the knee during inside-out suture of the posterior horn of the medial meniscus.
METHODS: The first stage of our study consisted of simulating a virtual meniscal suture during magnetic resonance imaging by tracing a line from 3 different points (located medially [MP], centrally [CP], and laterally [LP] to the patellar tendon) to the posterior horn of the medial meniscus. This procedure was undertaken both at rest and with valgus stress. The next phase involv

Schwannomas are the most common tumours of the sheath of peripheral nerves. The clinical diagnosis is usually straightforward, but may be delayed for many years in a schwannoma of the posterior tibial nerve. The symptoms are often attributed to entrapment neuropathy or to lumbosacral radiculopathy. We describe 25 patients with a schwannoma of the posterior tibial nerve. Only three were diagnosed within a year of presentation. The mean time to diagnosis was 86.5 months with a median of 48 months (2 to 360). All the patients complained of pain, which was felt specifically in the sole of the foot

The involvement of skeletal striated muscle in leprosy is considered secondary due to peripheral neuropathy, but some studies point it to a primary muscle lesion. In order to investigate the muscle involvement in leprosy, we studied 40 patients (lepromatous 23, tuberculoid 13, borderline 2 and indeterminate 2). The motor nerve conduction of the peroneal nerves had a reduction of the velocity, decreased compound muscle action potential and sometimes absence of potentials. The electromyographic study of the anterior tibial muscle showed signs of recent and chronic denervation in 77.5% of the cas

BACKGROUND: Diabetic sensorimotor polyneuropathy (DSPN) occurs in 50% of patients with diabetes mellitus (DM). Up to 70% of DM patients incur diabetic foot ulcers (DFUs). Of those with DSPN, 20% develop DFUs. Decompression nerve surgery (DNS) purportedly improves DSPN and DFU. As there is no accurate, pooled DFU incidence following DNS, the authors performed a systematic review and meta-analysis of DFU incidence following DNS. Given the 60% global DFU incidence, they hypothesized a lower, pooled DFU incidence following DNS.
METHODS: Two authors searched 6 databases from database inception to

BACKGROUND: Diabetic peripheral neuropathy (DPN) is one of the most common chronic complications of diabetes and the strongest initiating risk factor for diabetic foot ulceration. Early diagnosis of DPN through screening measures is, therefore, of great importance for diabetic patients. Recently, shear wave elastography (SWE) has been used as a method that is complementary to neuroelectrophysiological examination in the diagnosis of DPN. We aimed to conduct a meta-analysis based on currently available data to evaluate the performance of tibial nerve stiffness on SWE for diagnosing DPN.

OBJECTIVE: Tibial paresis is commonly seen in bovine practice as a sequel to dystocia. The tibial nerve supplies the extensor muscles of the hock joint and the flexor muscles of the digits; affected cows are lame and have a dropped hock and knuckling of the fetlock. Complete functional recovery occurs not consistently after a conservative "wait and see" approach. The aim of this study was to investigate the usefulness of a cast applied to the lower portion of the affected limb as a supportive treatment of tibial nerve paresis in cows.
MATERIALS AND METHODS: Eight dairy cows with tibial nerve

Background Autoimmune connective tissue disorders (AICTDs) are a group of chronic inflammatory diseases characterized by immune responses targeting the body's own tissues. Peripheral neuropathy, which can be symmetrical or asymmetrical, is a common complication in AICTD patients. Early detection of subclinical peripheral neuropathy remains challenging, as nerve conduction studies are not routinely performed. This study aimed to determine the prevalence of subclinical peripheral neuropathy among patients with AICTD and to assess its association with potential predictor variables, including labo

BACKGROUND: To elucidate the mechanisms of peripheral nerve damage in diabetic neuropathy, various animal models have been studied. The WBN/Kob rat is a new animal model that develops chronic pancreatitis and spontaneous diabetes. It remains unestablished whether spontaneously diabetic rats of this strain develop peripheral neuropathy, comparable to the human diabetic neuropathy.
EXPERIMENTAL DESIGN: In the present study, we examined electrophysiologic, biochemical, and structural changes of peripheral nerve in 12- and 20-month-old diabetic WBN/Kob rats.
RESULTS: All the WBN/Kob rats showed

AIMS: Emerging evidence shows, that distal symmetric peripheral neuropathy (DSPN) also involves alterations in the central nervous system. Hence, the aims were to investigate brain metabolites in white matter of adults with diabetes and DSPN, and to compare any cerebral disparities with peripheral nerve characteristics.
METHODS: In type 1 diabetes, brain metabolites of 47 adults with confirmed DSPN were compared with 28 matched healthy controls using proton magnetic resonance spectroscopy (H-MRS) in the parietal region including the sensorimotor fiber tracts.
RESULTS: Adults with diabetes ha

Tarsal tunnel syndrome (TTS) defines an entrapment neuropathy of the posterior tibial nerve or one of its branches, within the tarsal tunnel. Numerous etiologies have been described explaining this entrapment, including trauma, space-occupying lesions, foot deformities, etc. We present an unreported cause of a space-occupying lesion in the etiology of TTS, namely the combination of a hypertrophic long distally extended muscle belly of the flexor hallucis longus and repetitive ankle motion. Surgical debulking of the muscle belly in the posterior ankle compartment resolved all symptoms.

OBJECTIVES: Neuropathic pain is associated with hyperexcitability of spinal cord dorsal horn neurons. Spinal cord stimulation (SCS) can reduce this hyperexcitability and provide analgesia. We evaluated the effects of two novel SCS waveforms, designed with long pulse widths and short interpulse intervals with slowly varying amplitudes, on neuropathic pain behaviors and dorsal horn neuronal activity in a rat model of tibial nerve injury (TNI).
MATERIALS AND METHODS: Rats underwent TNI followed by implantation of epidural stimulation leads. Novel waveforms, specifically stochastic amplitude modu

Enzyme replacement therapy in infantile onset Pompe disease has led to a new phenotype with features not known in the pre-enzyme replacement therapy era. We investigated the origin of a rapidly emerging and severe weakness of the foot dorsiflexors in a 7-year-old boy after 6.5 years of enzyme replacement therapy. Electroneurography yielded normal findings except low compound muscle action potentials of the extensor digitorum brevis muscles after stimulation of the peroneal nerves. Electromyography of the tibial muscle demonstrated a myopathic pattern. Tibial muscle, sural nerve, and skin biops
