BACKGROUND: Tissue expansion is a reconstruction method often used to repair the tissue loss that results after removing soft tissue lesions such as scars, tumors, and giant hairy nevi. However, because the pressure of a tissue expander applies in all directions almost equally, along with the desired expansion of skin and subcutaneous fat tissue, the muscle and bone underneath the expander become depressed simultaneously. Even with a successful expansion of the tissue itself, the conventional surgical procedure results to frequent aesthetically dissatisfying outcomes because of contour irregul…

A female baby with an antenatal diagnosis of gastroschisis was transferred to our institution. The defect was large but the bowel was in good condition and a silo was placed. After successful reduction of the bowel the abdominal wall defect was too large to allow fascial or even skin closure. We utilised a Gore-tex patch with two prolene purse string sutures placed concentrically to enable the diameter of the patch to be sequentially reduced. This enabled gradual stretching of the tissues with a progressive reduction in the size of the defect. The patch was removed after 8 days and a delayed f

BACKGROUND: Tissue expansion is associated with a relatively high complication rate. The aim of this study was to quantify the complication risk of burn scar patients who underwent tissue expansion in comparison to patients with other indications such as skin tumors. Furthermore it was attempted to compare the complication rates in children and adults.
METHODS: A retrospective analysis was performed on 148 expanders implanted in 73 patients during the years 1994-2011. Two patient cohorts (burn scar cohort n=31 and other indication cohort n=42) were identified and analyzed.
RESULTS: 27 male a…

Approximately 5% of patients with neurofibromatosis type 1 (NF1) have deletions of the entire NF1 gene. The phenotype usually includes early onset, large number of neurofibromas, presence of congenital anomalies, cognitive deficiency, and variable dysmorphic features and growth abnormalities. Connective tissue abnormalities are not generally recognised as a part of NF1 microdeletion syndrome, but mitral valve prolapse, joint laxity, and/or soft skin on the palms have been reported in a few patients. We describe clinical findings in six newly diagnosed patients with NF1 microdeletions, five of

Myotonic dystrophy (DM), the most common inherited muscle disorder, is caused by a CTG expansion in the 3"-untranslated region of a protein kinase gene ( DMPK ). The complex and variable phenotype is most likely caused by a complex molecular pathogenesis, including deficiency of the DMPK protein, a trans -dominant misregulation of RNA homeostasis and haploinsufficiency of a neighboring homeobox gene [DM locus-associated homeodomain protein (DMAHP )]. Here, we study the allele-specific transcriptional activity of the DMAHP/SIX5 gene in DM patient tissues. Using a quantitative fluorescent RT-PCR

Tissue expansion facilitates coverage of cutaneous defects both through the generation of additional skin surface area and by increasing random flap length/width viability factors. The opportunity to apply tissue expansion techniques to head and neck defects within previously irradiated skin fields continues to increase. To study the effect of tissue expansion on the cutaneous perfusion and wound-healing capacity of irradiation-damaged skin, rabbit scalps were subjected to 5 weeks of fractionated radiation followed at 4 months by prolonged tissue expansion. Standardized random flaps were then …

Three-dimensional (3D) scaffold-based culture systems offer a promising approach for expanding primary hepatocytes in vitro, potentially overcoming donor shortages in liver failure treatment. In this study, we designed a multifunctional Alg1SBC scaffold that actively loads and activates platelet-rich plasma (PRP) as a bioinspired platform. This system effectively combines low-cost PRP with a dual-network matrix composed of alginate (Alg) and sulfonated bacterial cellulose (SBC). The negatively charged surface of the Alg1SBC scaffold mimics the electrostatic characteristics of hepatic sinusoids

One of the most striking features of the adipose depot surrounding the prostate [periprostatic adipose tissue (PPAT)] is that its accumulation is independent of body mass index. Its volume varies considerably between individuals, with some patients exhibiting abundant PPATs, which have been correlated to the occurrence of aggressive prostate cancer (PCa). However, abundant PPAT is not well defined at the biological level. We used a new statistical approach to define abundant PPAT by normalizing PPAT volume to prostate volume in a cohort of 351 patients using a linear regression model. Applying

PURPOSE: Various flap techniques have been developed for injuries to the middle and distal phalanges, with the dorsal nerve fascial island (DNFI) flap offering distinct advantages. We aimed to elucidate the process of skin pedicle expansion (SPE) during DNFI flap transplantation and its effectiveness in relieving pressure on the fascial pedicle.
METHODS: Ninety-two patients with soft tissue defects in the middle and distal phalanges were treated using a DNFI flap from February 2008 to August 2023. Eighty-three patients underwent SPE. The SPE technique involves creating a 3- to 4-mm wide skin

Vertebral growth is an essential developmental process to support the expansion of the vertebrate body. In teleosts, the lateral side of the vertebral bodies develops to form different structures among species in the late stages of vertebral growth, although lateral structures are not apparent in the early stages. Lateral structures are one of the structural features that determine the diversity of teleost vertebrae. However, explanations for the formation of lateral structures are conflicting because few reports have investigated the growth of teleost vertebral bodies. To clarify the growth p

Upper-extremity wounds from various etiologies such as trauma and fasciotomies can prove to be problematic for the upper-extremity surgeon. These defects can result in considerable morbidity often requiring prolonged wound care and the eventual use of skin grafting from a separate painful donor site. Tissue expansion takes advantage of the viscoelastic properties of skin to provide additional tissue for reconstruction. The authors present a technique using a continuous external tissue expansion device for closure of upper-extremity wounds.

OBJECTIVE: To evaluate three-dimensional (3-D) soft tissue facial changes following rapid maxillary expansion (RME) and to compare these changes with an untreated control group.
MATERIALS AND METHODS: Patients who need RME as a part of their orthodontic treatment were randomly divided into two groups of 17 patients each. Eligibility criteria included having maxillary transverse deficiency with crossbite, and to be in the normal range according to body mass index. In the first group (mean age = 13.4 ± 1.2 years), expansion was performed. The second group received no treatment initially and s

BACKGROUND: Resection of cranial tumors involving both bone and scalp tissue may require the recruitment of soft tissue using a flap above the bone reconstruction. When a transposition flap has been chosen, the alopecia zone on the donor site may be difficult to treat afterward. Scalp expansion is the gold standard in these situations, but this has never been described above cranial implants. We report the first case of a patient who underwent a scalp tissue expansion above a custom-made hydroxyapatite cranial implant to correct sequelar alopecia.
CASE DESCRIPTION: A 30-year-old man presented…

INTRODUCTION: The force delivered during rapid maxillary expansion (RME) produces areas of compression on the periodontal ligament of the supporting teeth. The resulting alveolar bone resorption can lead to unwanted tooth movement in the same direction. The purpose of this study was to evaluate periodontal changes by means of computed tomography after RME with tooth-tissue-borne and tooth-borne expanders.
METHODS: The sample comprised 8 girls, 11 to 14 years old, with Class I or II malocclusions with unilateral or bilateral posterior crossbites Four girls were treated with tooth-tissue-borne

A series of experimental studies on sutural expansion osteogenesis for management of the bony-tissue defect in cleft palate repair was performed between 1995 and 1997. Forty-five young dogs in weaning were used in four experiments that were divided into two parts. Part I probed the possibility of closing the surgically constructed hard palate cleft not only with mucoperiosteum but also with bony tissue by the technique of sutural expansion of lateral palatine sutures. Part II explored the possibility of pushing the palatine bone posteriorly and advancing the maxillary segment anteriorly by tra

OBJECTIVES: The paucibacillary nature of spinal tuberculosis (STB) challenges diagnosis. Tissue sites of Mycobacterium tuberculosis (Mtb) and HIV-1 co-infection are suggested to favor reciprocal replication, and reservoir expansion, including in people living with HIV-1 (PLWH) on antiretroviral treatment (ART); however, evidence is scant. This study aimed to evaluate the utility of droplet digital polymerase chain reaction (ddPCR) to detect and quantify Mtb DNA in spinal biopsy tissue, with sensitivity validated against culture and Xpert Mtb/rifampicin (MTB/RIF) Ultra, and determine whether Mt

Tissue expansion is a handy reconstructive technique for the head and neck region; however, its implementation requires careful planning and surgical experience. If tissue expansion is inadequate, forced closure results in wound tension and risks complications, such as postoperative deformity, wide scarring, and wound dehiscence. We report a case of adult forehead melanocytic nevus excision using a tissue expander (TE) where complications caused by insufficient tissue expansion were avoided by creating a flap using a dog ear. The patient was a male in his 20s who underwent surgery with a TE fo…
