A primipara affected by late-onset diabetes and sensory deafness because of mitochondrial myopathy was hospitalized for threatened preterm delivery. Magnesium sulfate was started for tocolysis, resulting in general muscle damage, although the mitochondrial myopathy did not deteriorate during pregnancy. Magnesium sulfate may be contraindicated in pregnancy with mitochondrial myopathy.

We performed cervical cerclage on a completely dilated and effaced cervix after decompression of the amniotic sac with intraoperative amniocentesis. Indomethacin was used for tocolysis and to maintain mild oligohydraminios (amniotic fluid index 5-7 cm). The pregnancy was prolonged by 12 days and a viable infant was delivered at 25 weeks' gestation.

We describe two cases treated for cervical incompetence with prolapsed fetal membranes with emergency cerclage. Two methods of membranes reducing were used: transabdominal amniocentesis and bladder overfilling. We recommend these procedures in patients with advanced cervical incompetence when the membrane reduction by Trendelenburg position or pharmacological tocolysis were ineffective.

A triplet pregnancy is described in which there was a second trimester abortion of one fetus at 16 weeks' gestation. A cervical cerclage was placed, and tocolysis was used. The other two triplets were delivered 131 days later (at 35 weeks' gestation) by cesarean section. Both infants survived. This case is unique in the literature.

Despite the widespread use of beta-sympathomimetic agents for preterm labour there appears to be a limited appreciation of the need for cardiovascular monitoring in the mother. Four patients in whom pulmonary oedema developed during tocolysis with hexoprenaline are described and the aetiological factors and pathogenesis of this potentially lethal complication discussed. Guidelines for the safe use of hexoprenaline in preterm labour are suggested.

Sixty-two cases of preterm placenta previa were managed in our hospital since September, 1979 through August, 1983. Seventy patients with preterm labor and intact membranes were initially treated with ritodrine hydrochloride to delay preterm delivery. Tocolysis beyond 24 hours was achieved in 59 patients. The aim of this study is to be able to perform on the animal (Macacus cynomolgus) a specific in utero surgical procedure for correcting congenital diaphragmatic hernia, in order to do the same on human fetuses. The surgical technique consist in operating in utero, and to put a silastic patch on the diaphragm. A good tocolysis is very necessary to prevent early abortion.
