Kidney transplantation in patients with a prior heart transplant.

BACKGROUND: Chronic kidney disease among prior heart transplant recipients is a growing problem that is likely to place an increased demand on a limited supply of kidney allografts. Allocation of the limited resource of kidneys for transplantation requires consideration of the demands of fair distribution and optimizing patient and graft survival. The aim of this study was to compare the kidney transplant outcomes among recipients of kidney after prior heart transplantation (KAH, n=456) with kidney transplantation in other clinical settings. METHODS: A retrospective cohort study using United …

Cystic fibrosis liver disease: to transplant or not to transplant?

Biliary cirrhosis complicates some adults with cystic fibrosis (CF) and may require transplantation. Cardio-respiratory disease severity varies such that patients may require liver transplantation, heart/lung/liver (triple) grafts or may be too ill for any procedure. A 15-year experience of adults with CF-related liver disease referred for liver transplantation is presented with patient survival as outcome. Twelve patients were listed for triple grafting. Four died of respiratory disease after prolonged waits (4-171 weeks). Eight underwent transplantation (median wait 62 weeks); 5-year actuari…

Post-transplant lymphoproliferative disease in liver transplantation.

Post-transplant lymphoproliferative disease (PTLD) is a well recognized complication of solid organ transplantation and therapeutic immunosuppression, first reported in 1968. PTLD incorporates a spectrum of abnormalities ranging from a benign infectious mononucleosis-like illness to non-Hodgkin's lymphoma with nodal and extranodal site involvement. The first liver transplant was performed at our institution in January 1982. This retrospective study examined the incidence of PTLD, reason for the original transplants, presenting symptoms, radiological findings, immunosuppression regimens and out…

Stem cell transplantation (cord blood transplants).

Allogeneic stem cell transplantation is an accepted treatment modality for selected malignant and non-malignant diseases. However, the ability to identify suitably matched related or unrelated donors can be difficult in some patients. Alternative sources of stem cells such as cord blood provide a readily available graft for such patients. Data accumulated over the past several years have demonstrated that the use of cord blood is an accepted source of stem cells for pediatric patients. Since the cell numbers of hematopoietic progenitors in cord blood is limited and the collection can occur onl…

Cardiopulmonary transplantation: experience of a lung transplant group.

Cardiopulmonary transplantation (CPT) is indicated for patients eligible for heart transplantation (HT) or lung transplantation (LT) who have severe concomitant lung or heart disease. Only 2 groups perform CPT in Spain. We report our experience with 18 CPTs representing 8.2% and 5% compared with LT (220) and HT (362), respectively, from February 13, 1990 to October 15, 2002. The mean time on a waiting list was 138 days. The current number of surviving patients is 7 (39%), with a mean follow-up of 602 days (range, 3 to 4627 days). They all remain asymptomatic with normal respiratory function in…

Transplantation and Employment Earnings in Kidney Transplant Recipients.

IMPORTANCE: Kidney transplantation improves survival and quality of life for individuals with end-stage kidney disease (ESKD). However, its effect on objective economic outcomes, such as employment income, remains insufficiently studied. OBJECTIVE: To quantify changes in employment income before and after kidney transplantation in Canada using linked administrative health and tax data. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study included nationwide data from Canadian adults receiving a kidney transplant between 2007 and 2016. Data were from the Canadian Hospitalizat…

Transplant 101: Post-Kidney Transplant Complications.

Kidney transplantation offers individuals with end stage kidney disease the best chance for long-term survival without dialysis and at the lowest cost to the health care system. However, kidney transplantation comes with risks and complications. These complications can be categorized as short-term, long-term, infectious, and noninfectious. Short-term complications can occur immediately after transplant and for up to six months post-transplant. These complications can involve delayed graft function, infection, rejection, and technical surgical causes. Long-term complications occur six months an…

Split liver transplantation in Shiraz Transplant Center.

BACKGROUND: Cadaveric organ splitting emerged from an improved understanding of the surgical anatomy of the liver as a possible mechanism to expand the organ pool. In this study, we have reported our first series of split liver transplantations (SLT). MATERIALS AND METHODS: From June 2006 to June 2008, we performed 17 pairs of SLT: 70.6% ex situ and 29.4% in situ. The mean age of the donors (32 males, 2 females) was 23.15 +/- 9 years. All of them had been stable at the time of harvest according to vital signs, liver function tests, electrolytes, and urine output. The decision on splitting was…

Early post-transplant hyperglycemia and post-transplant diabetes mellitus following heart transplantation.

INTRODUCTION: Heart transplantation is an important treatment for end-stage heart failure. Early post-transplant hyperglycemia (EPTH) and post-transplant diabetes mellitus (PTDM) are common following heart transplantation and are associated with increased morbidity and mortality. AREAS COVERED: This review summarizes the clinical characteristics, diagnosis, and treatment of EPTH and PTDM in cardiac transplant patients, incorporating findings from non-cardiac solid organ transplant studies where relevant due to limited heart-specific research. EXPERT OPINION: EPTH following heart transplantat…

Bone marrow transplantation for aspartylglucosaminuria: follow-up study of transplanted and non-transplanted patients.

We describe the state of health, intellectual skills, and dysmorphic features of 19 young patients with aspartylglucosaminuria. Of them, 5 had undergone a successful bone marrow transplantation between 1991 and 1997. The first 2 patients who received transplants were, after 7 and 5 years' follow-up, more severely mentally retarded than the non-transplanted patients. The general health of the later patients was quite good, whereas the 5 patients who underwent bone marrow transplantation had post-transplant complications. Their dysmorphic status remained unchanged. We cannot encourage bone marro…

Intestinal transplantation for tumors.

PURPOSE OF REVIEW: Transplantation of the intestine is a vital component in the treatment of many intrabdominal tumors. This is a comprehensive review of current practice and outcome of intestinal transplantation for tumors. RECENT FINDINGS: Most commonly described for the intrabdominal desmoid tumor, excellent survival outcomes can be attained, particularly for the more aggressive desmoid tumors associated with familial adenomatous polyposis syndrome and early referral is critical in order to avoid diffuse tumor spread which would necessitate a more complex multivisceral transplant. Currentl…

Pediatric heart-kidney transplantation.

Heart transplantation is regarded as the definitive treatment for advanced pediatric heart failure. However, concomitant kidney dysfunction often complicates candidate selection for isolated heart transplantation. Prolonged venous congestion, nephrotoxic exposures, and recurrent episodes of acute kidney injury can result in varying degrees of renal impairment. Differentiating between reversible kidney injury secondary to cardiorenal syndrome and irreversible intrinsic renal disease remains a major challenge. Pediatric heart-kidney transplantation has shown survival benefits for patients with e…

[Malignant neoplasms and transplantation].

Malignant neoplasms constitute a major burden of morbidity and mortality in the general population. This necessitates intense screening of transplant candidates and even closer surveillance of immunosuppressed solid organ recipients. Active malignancy is an exclusion criterion to solid organ transplantation, with few exceptions, namely localized hepatic neoplasms. Accelerated tumor progression characterizes post-transplantation malignancies. Intensified surveillance is justified in elevated rates, e.g., of skin cancer and virus-associated neoplasms, especially Epstein-Barr virus-associated pos…

Orthotopic Rat Forelimb Transplantation.

Vascularized composite allotransplantation (VCA) involves the transplantation of multiple tissue types -- including skin, muscle, bone, and nerves -- offering a promising reconstructive option for patients with severe traumatic injuries or disfigurements. Despite its transformative potential, VCA has encountered significant challenges such as graft rejection, chronic immunosuppression complications, and neuromuscular recovery's intricacies. We utilize a rat forelimb model as a cost-effective and anatomically relevant platform to address these challenges. The rat forelimb closely mirrors human …

Proteinuria After Kidney Transplantation.

Proteinuria is a relatively frequent complication in both adults and children after kidney transplantation (40%-80%). It is usually mild and predominantly of tubular origin and is caused mainly by rejection, mTOR inhibitors, or hypertension; however, proteinuria could also be in the nephrotic range and of glomerular origin if caused by the recurrence of idiopathic FSGS or rejection. Proteinuria is a risk factor impacting graft and patient survival in adults and graft survival in children. Proteinuria should be assessed by protein/creatinine ratio regularly in pediatric kidney transplant recipi…
