Feasibility, tolerability, and outcomes of nebulized liposomal amphotericin B for Aspergillus infection prevention in lung transplantation.

BACKGROUND: Nebulized amphotericin B deoxycholate (n-ABD) is used to prevent Aspergillus infection in lung transplantation. Nebulized liposomal amphotericin B (n-LAB) is another option; however, no clinical data are available on the results of n-LAB for this purpose. METHODS: In an observational study performed in 2 centers to assess the feasibility, tolerability, and outcomes of n-LAB prophylaxis, 104 consecutive patients undergoing prophylaxis with n-LAB were compared with 49 historical controls who received n-ABD. Patient follow-up lasted 12 months. The n-LAB prophylaxis regimen was 25 mg

Early presence of regulatory cells in transplanted rats rendered tolerant by donor-specific blood transfusion.

Mechanisms by which donor-specific blood transfusion (DSBT) promotes organ allograft acceptance are unclear. In a rat fully mismatched cardiac allograft model, we found that DSBT alone (without immunotherapy) induces the development of regulatory T cells (DSBT-Tregs) posttransplant, thereby shedding new light in the mechanisms of the transfusion effect. Compartments and timing of expansion, requirements, and phenotype of DSBT-Tregs are unknown. It is generally assumed that some time is necessary before Tregs develop. However, we show-by adoptive transfer from DSBT-tolerant into naive recipient

Operational tolerance following intestinal transplantation: A case report with comprehensive immune follow-up from transplant day to 3 years postimmunosuppression withdrawal.

Operational tolerance after intestinal transplantation is exceptionally rare, with few reported cases and limited mechanistic understanding. We describe a 12-year-old girl who underwent multivisceral transplantation and subsequently developed graft-versus-host disease (GvHD) followed by Epstein-Barr virus-associated posttransplant lymphoproliferative disease. Due to severe posttransplant lymphoproliferative disease complications, immunosuppression was withdrawn 8 months posttransplant. The patient achieved viral remission and has remained free of rejection for more than 3 years without immunos

Mesenchymal stem cells enhance the induction of mixed chimerism and tolerance to rat hind-limb allografts after bone marrow transplantation.

BACKGROUND: Mixed hematopoietic chimerism via bone marrow transplantation has been shown to induce donor specific tolerance to solid organ allografts, but graft versus host disease (GVHD) remains to be a risk. Composite tissue allografts may need a higher percentage of donor chimerism compared with less immunogenic solid organ allografts. In this study, we investigated the potential of mesenchymal stem cells (MSCs) in the induction of stable and high level chimerism and subsequent donor-specific tolerance to composite tissue allograft without the incidence of graft versus host disease (GVHD).

Chimeric donor cells play an active role in both induction and maintenance phases of transplantation tolerance induced by mixed chimerism.

Donor hemopoietic cell engraftment is considered to be an indicator of allograft tolerance. We depleted chimerism with cells specifically presensitized to the bone marrow donor to investigate its role in mixed chimera-induced tolerance. Three experimental models were used: model A, B10.A cells presensitized to B6 (a anti-b cells) were injected into (B6 x D2)F(1) --> B10.A mixed chimeras grafted with DBA/2 skin; model B, anti-B6 presensitized cells prepared in DBA/2 --> B10.A mixed chimeras, thus unresponsive to DBA/2 (a anti-b/tol-d cells), were injected into (B6 x D2)F(1) --> B10.A mixed chim

Evidence that major histocompatibility complex restriction of foreign transplantation antigens occurs when tolerance is induced in neonatal mice and rats.

Studies on the survival of skin-specific antigen (Skn)-incompatible skin grafts in mice rendered tolerant at birth with major histocompatibility complex (MHC)-incompatible lymph node and spleen or bone marrow cells, as well as studies concerned with the survival of third-party skin grafts in rats rendered tolerant at birth with MHC-incompatible bone marrow cells, indicate that MHC restriction of foreign transplantation antigens occurs when tolerance is induced. Thus, evidence is presented that animals rendered tolerant with MHC-incompatible bone marrow cells depleted of mature T lymphocytes wi

Kidney transplantation in patients with severe aplastic anemia: A case series of four patients bridging to tolerance or hematologic recovery.

Severe aplastic anemia (SAA) is a bone marrow failure syndrome characterized by pancytopenia and hypocellular marrow. When accompanied by end-stage kidney disease (ESKD), it presents a significant therapeutic challenge. Although allogeneic hematopoietic stem cell transplantation (HSCT) is the definitive treatment for SAA and kidney transplantation (KT) is the optimal therapy for ESKD, the ideal sequencing and strategy for performing both procedures remain unclear. We report successful outcomes in 4 patients with concurrent SAA and ESKD managed using a KT-first approach. All patients demonstrat

Current Tolerance-Associated Peripheral Blood Gene Expression Profiles After Liver Transplantation Are Influenced by Immunosuppressive Drugs and Prior Cytomegalovirus Infection.

Spontaneous operational tolerance to the allograft develops in a proportion of liver transplant (LTx) recipients weaned off immunosuppressive drugs (IS). Several previous studies have investigated whether peripheral blood gene expression profiles could identify operational tolerance in LTx recipients. However, the reported gene expression profiles differed greatly amongst studies, which could be caused by inadequate matching of clinical parameters of study groups. Therefore, the purpose of this study was to validate differentially expressed immune system related genes described in previous stu

Irradiation before and donor splenocyte infusion immediately after transplantation induce tolerance to lung, but not heart allografts in miniature swine.

Solid organs may differ in their potential to induce and maintain a state of donor-specific tolerance. Previously, we induced stable immunological tolerance in a lung transplantation model in miniature swine. Here, we wished to transfer this established protocol into a heart transplantation model in miniature swine. Heterotopic heart transplantation (HTX) was performed in four and left-sided lung transplantation (LTX) in seven minipigs from gender- and SLA-mismatched donors. All recipients received nonmyeloablative irradiation, donor splenocyte infusion and intravenous pharmacologic immunosupp

Tolerance induction permits the development of graft-versus-host disease: donor-mediated attack following small bowel transplantation in mixed chimeras.

The induction of tolerance to organ allografts would eliminate acute and chronic rejection as well as the need for nonspecific immunosuppression. We have shown that tolerance induced through the creation of mixed allogeneic bone marrow chimeras allows for the long-term engraftment of cardiac and small bowel allografts across strong multiple major histocompatibility barriers. The possibility that tolerance might render the host susceptible to graft-versus-host disease (GVHD) has not been investigated in this or other models of tolerance. To test this possibility chimeras were created by transpl

Dosing, Safety, and Tolerability of Extended-Release Tacrolimus in Pediatric and Young Adult Solid Organ Transplant Recipients.

BACKGROUND: Complicated immunosuppression regimens can contribute to non-adherence in pediatric solid organ transplant (SOT) recipients. Once-daily tacrolimus formulations, such as LCP-tacrolimus (LCPT), may help simplify dosing, but pediatric data on dosing, safety, and early tolerability are limited. METHODS: We conducted a single center retrospective cohort study of SOT recipients prescribed LCPT from December 2016 to July 2024 at our institution. LCPT:IR-Tac daily dosing ratios were calculated. The frequency of laboratory-associated adverse events (AE) and of new patient-reported adverse

Improved Tolerability of De Novo Letermovir Over Valganciclovir for Cytomegalovirus Prophylaxis in Seropositive Liver Transplant Recipients.

INTRODUCTION: Cytomegalovirus (CMV) can drive negative outcomes after liver transplant (LT). Though valganciclovir (VGC) remains the guideline-recommended standard-of-care, letermovir (LTV) may be beneficial in LT given reduced bone marrow suppression. METHODS: Adult patients receiving a LT with moderate-risk CMV serostatus (R+) 6/1/2021-9/30/2024 were evaluated. Patients were assigned to a cohort based on de novo antiviral prophylaxis regimen: VGC or LTV. The primary objective was tolerability and safety of LTV compared to VGC. RESULTS: Eighty-five patients met inclusion criteria: 60 in VGC

Receptor tyrosine kinase MerTK suppresses an allogenic type I IFN response to promote transplant tolerance.

Recipient infusion of donor apoptotic cells is an emerging strategy for inducing robust transplantation tolerance. Daily clearance of billions of self-apoptotic cells relies on homeostatic engagement of phagocytic receptors, in particular, receptors of the tyrosine kinase family TAM (Tyro3, Axl, and MerTK), to maintain self-tolerance. However, an outstanding question is if allogeneic apoptotic cells trigger the same receptor system for inducing allogeneic tolerance. Here, we employed allogeneic apoptotic splenocytes and discovered that the efferocytic receptor MerTK on recipient phagocytes is …

Comparison of Rifampin and Isoniazid for Latent Tuberculosis Infection in Kidney Transplant Candidates: Focus on Tolerability and Treatment Completion.

BACKGROUND: A 4-month course of rifampin is one of the recommended first-line regimens for latent tuberculosis infection (LTBI). However, data on its use among kidney transplant candidates (KTC) remain limited. METHODS: We conducted a retrospective study of all KTC treated with either 4-month rifampin or 9-month isoniazid (INH) for LTBI at a transplant infectious disease clinic in Miami from January 1, 2021 to December 31, 2024. We assessed rates of treatment completion, adverse reactions leading to discontinuation of therapy, and transaminase elevation (> 2 times the upper limit of normal).

Outcomes and Tolerability of Early Mammalian Target of Rapamycin Inhibitor Initiation for Renal Sparing in Heart Transplant Recipients.

PURPOSE: Renal dysfunction in heart transplant recipients (HTRs) is a significant post-transplant complication. This study aims to assess the effects of early mammalian target of rapamycin inhibitor (mTORi) initiation on renal and graft outcomes in HTRs. METHODS: A single-center retrospective analysis of first-time adult HTRs transplanted from 2018 to 2023 was conducted. HTRs converted to mTORi were compared to those maintained on tacrolimus and mycophenolate immunosuppression (SOC). Patient demographics along with renal and graft outcomes were collected for the first 2 years post-transplant.

The safety and tolerability of pirfenidone for bronchiolitis obliterans syndrome after hematopoietic cell transplant (STOP-BOS) trial.

Bronchiolitis obliterans syndrome (BOS) is the most morbid form of chronic graft-versus-host disease (cGVHD) after hematopoietic cell transplantation (HCT). Progressive airway fibrosis leads to a 5-year survival of 40%. Treatment options for BOS are limited. A single arm, 52-week, Phase I study of pirfenidone was conducted. The primary outcome was tolerability defined as maintaining the recommended dose of pirfenidone (2403 mg/day) without a dose reduction totaling more than 21 days, due to adverse events (AEs) or severe AEs (SAEs). Secondary outcomes included pulmonary function tests (PFTs) a
