Dorsoradial and anterior oblique ligaments were harvested during surgery in 13 patients with symptomatic trapeziometacarpal osteoarthritis, which had been graded preoperatively by a modified Eaton-Littler radiographic grading. Ligaments, including the periligamentous synovium, were stained with S100 protein, neurotrophic receptor p75, protein gene product 9.5, calcitonin gene related peptide, acetylcholine, substance P, neuropeptide Y, noradrenaline, N-methyl-D-aspartate-receptor and Met/Leu-enkephalin. The synovium was classified as showing no, low-grade or high-grade synovitis. Free nerve en

A carpal injury called a lunate fracture can cause severe carpal instability if treatment is not received. After the scaphoid, triquetrum, and trapezium, the lunate is the fourth most frequently fractured carpal bone. Due to lunate fracture, the functional prognosis is uncertain, and conservative treatment frequently results in surgery. Lunate fracture may be caused by anatomical features such as ulnar and radial variation, although necrosis is not commonly one of them. Vascularization in the lunate is unstable and dependent on the capsular arterioles. High shear stresses are frequently applie

INTRODUCTION: Scaphotrapeziotrapezoid (STT) joint osteoarthritis may influence the outcome after trapeziometacarpal prosthesis (TMP) implantation. The literature regarding its clinical and radiological assessment is unclear. The aim of our study was to determine by means of a pre- and post-intervention CT study whether the degree of STT involvement influences the clinical-functional or radiological results after TMP implantation, and to establish whether or not STT osteoarthritis could be a contraindication for the use of these implants.
METHODS: Prospective study of 60patients with trapeziom

PURPOSE: Trapeziometacarpal (TM) arthrodesis may increase adduction motion of the thumb metacarpophalangeal (MCP) joint, causing radial collateral ligament laxity. Stability of the MCP joint is important to the long-term functional outcome after TM arthrodesis. This study assessed preoperative and postoperative radial collateral ligament laxity using dynamic radiographs to confirm whether laxity was exacerbated after surgery and examined whether there is a relationship between the fixation angle of arthrodesis and the degree of laxity.
METHODS: Forty-four thumbs in 33 patients who underwent T

We compared an uncemented joint replacement (Elektra) with trapeziectomy (with ligament reconstruction and tendon interposition, LRTI) for the treatment of primary osteoarthritis in the first carpometacarpal joint (CMC1) in a randomised controlled trial. Forty patients were included (20 in each group) and followed for 2 years. The median age in the groups was 64 and 61, respectively, with 14 females in each. At final follow-up, there was no difference in the primary outcome measure (the quick disabilities of the arm, shoulder and hand score, QDASH), but the joint replacement group had signific

A case of trapezometacarpal dislocation associated with metacarpophalangeal dislocation (floating first metacarpal) is described. The trapezometacarpal dislocation re-occurred twice despite well conducted orthopedic treatment. Ligamentoplasty of the carpal radial flexor tendon using the Eaton and Littler method then provided an excellent result. Failure of orthopedic treatment was probably related to the congenital hyperlaxity presented by this patient. Considering this hypothesis, the presence of hyperlaxity would affect management practices used for trapezometacarpal dislocation. In the pres

PURPOSE: Trapeziometacarpal (TMC) joint arthrodesis is an established surgical option to manage TMC joint arthritis. However, nonunion remains a complication after arthrodesis. In this study, we aimed to evaluate the outcomes of using a locking plate and headless compression screw with autogenous bone grafting.
METHODS: We identified 21 thumbs that underwent TMC arthrodesis using this procedure. Radiographs and computed tomography scans were obtained to define bone union. Furthermore, we asked patients whether they had returned to their original work at every follow-up visit. To assess object

PURPOSE: Surgical options are limited for distal humerus and proximal ulna injuries with articular bone loss in physiologically young patients. Osteochondral grafts are a potential treatment option between open reduction internal fixation and arthroplasty that does not impart lifelong activity restrictions and has longer longevity. Feasible graft harvest sites that match the surface geometry of the distal humerus and proximal ulna are unknown. This study assesses the surface geometry of potential grafts with known well-defined donor site morbidity for the reconstruction of distal humerus and p

BACKROUND: Trapeziometacarpal osteoarthritis has been treated with interposition of autologous fascia into the trapezial resection cavity to prevent scaphometacarpal impingement with suboptimal results. Autologous fascia may undergo necrosis and gradual shrinkage due to avascularity. We propose vascularization of the interposed fascia using microvascular techniques as a new alternative to achieve a durable graft with sufficient soft tissue volume.
PATIENTS AND METHODS: In this retrospective study, 11 surgeries for 8 patients (6 women, 2 men) (5 primary and 6 revision cases) were performed usi

Trapeziectomy with suture-button suspensionplasty is a surgical treatment option for thumb carpometacarpal osteoarthritis refractory to nonsurgical management. We describe the cases of 3 patients who presented with index metacarpal fracture, in the absence of traumatic injury, over 4 months after trapeziectomy with suture-button suspensionplasty. All 3 fractures demonstrated the same pattern: short oblique/spiral, oriented proximal radial to distal ulnar with the distal end in the vicinity of the index metacarpal button, presumably after the orientation of the metacarpal drill hole. Two of the

OBJECTIVE: MRI of the hands is valuable for risk-stratification in patients with arthralgia at-risk for developing rheumatoid arthritis (RA). Contrast-enhanced MRI is considered standard for assessment of RA, but has practical disadvantages. It also shows inflammation-like features in the general population, especially at older age, which should be considered in image interpretation. The modified-Dixon (mDixon) technique is reliable compared to contrast-enhanced sequences. Moreover, this short protocol without contrast-enhancement is patient-friendly. Whether it also shows inflammation-like fe

OBJECTIVE: To study the effectiveness of surgical treatment of congenital type V thumb syndactyly.
METHODS: Between March 2010 and May 2015, 12 cases of congenital type V thumb syndactyly were treated. There were 7 males and 5 females, aged from 1 to 25 years (mean, 8 years). The right thumb was involved in 8 cases, and the left thumb in 4 cases. There were 2 cases of radial type, and 10 cases of ulnar type. The basement of polydactylism was far away from the carpometacarpal joint in 7 cases, and was close to the carpometacarpal joint in 5 cases (slight ulnar deviation in 1 case). X-ray films

BACKGROUND: Numerous patient-reported outcome measures are used to assess thumb and hand function, but none are specific to the trapeziometacarpal (TMC) joint. The purpose of this study was to describe and validate a TMC-specific patient-reported outcome measure: the Buffalo Score.
METHODS: The Buffalo Score was compared with validated functional scores (QuickDASH, Kapandji score, Trapeziometacarpal Arthrosis Symptoms Disability (TASD) questionnaire) and with objective hand measurements (lateral pinch strength and subjective strength evaluation by number of paper folds a patient can do). PURPOSE: Thumb osteoarthritis is a debilitating condition that affects a large portion of the elderly population. Conservative treatments for this condition often fail, and a surgical solution is required. Many different surgical techniques have been described, but the current literature has not yet demonstrated the superiority of one over the others. In this study, we analyzed the clinical and radiological findings of a population of 26 patients who were operated on both hands but with different techniques.
METHODS: One hand underwent trapeziectomy with suspension arthroplasty, while the oth

BACKGROUND: The trapeziometacarpal joint is a common site for osteoarthritis development in the hand. When osteoarthritis is present, it results in significant functional disabilities due to the broad range of activities performed by this joint. However, our understanding of osteoarthritis initiation and progression at this joint is limited because of the current lack of knowledge regarding the properties and structure of the corresponding cartilage layers. The objective of this study is to assess the morphological and mechanical properties of trapeziometacarpal cartilage via the combination o

The thumb carpometacarpal (CMC) joint facilitates multidirectional motion of the thumb and affords prehensile power and precision. Traditional methods of quantifying thumb CMC kinematics have been largely limited to range-of-motion (ROM) measurements in 4 orthogonal primary directions (flexion, extension, abduction, adduction) due to difficulties in capturing multidirectional thumb motion. However, important functional motions (e.g., opposition) consist of combinations of these primary directions, as well as coupled rotations (internal and external rotation) and translations. Our goal was to p

BACKGROUND: The contact biomechanics of the trapeziometacarpal joint have been investigated in several studies. However, these led to conflicting results and were mostly performed in vitro. The purpose of this study was to provide further insight on the contact biomechanics of the trapeziometacarpal joint by in vivo assessment of healthy and osteoarthritic subjects.
METHODS: The hands of 16 healthy women and 6 women with trapeziometacarpal osteoarthritis were scanned in positions of maximal thumb extension, flexion, abduction and adduction during three isometric tasks (lateral key pinch, powe

This paper describes a surgical technique to achieve root coverage in deep, wide gingival recessions using a root isolation procedure with ePTFE membrane, combined with tetracycline conditioning of the root surface and fibrin-fibronectin sealing system application. The technique was used on 15 patients with isolated mucogingival defects 4 to 6 mm deep. A large trapezium-shaped flap was raised 3 to 4 mm apical to the margin of the bone dehiscence. The root surface was thoroughly scaled by hand and rotating instruments. Tetracycline HCl solution (100 mg/ml) was topically applied for 4 minutes. E

INTRODUCTION: Trapeziometacarpal joint stability during pinch depends on the balance between the opponens pollicis and the first dorsal interosseous muscles, which control pronation and supination of the first metacarpal, respectively. Disruption of this muscular balance may contribute to joint instability in patients with trapeziometacarpal osteoarthritis, although the mechanisms underlying movement pattern changes in these patients remain unclear. This study aimed to quantitatively compare thumb rotation patterns during tip pinch between patients with trapeziometacarpal osteoarthtits and hea
