A review of upper extremity fasciotomies in a level I trauma center.

The purpose of this study was to review recent experience with upper extremity fasciotomy. This study is a retrospective review of injured patients undergoing fasciotomy in the upper extremity at an urban trauma center. Mechanisms of injury, indications for and timing of fasciotomy, role of compartment pressures, techniques of closure, amputation rate, and patient outcomes were collected. Over a 3-year period, 201 fasciotomies were performed in the extremities of 157 injured patients, including 37 in the upper extremities of 27 patients. The mechanisms of injury were penetrating trauma in 13 p

Penetrating axillary trauma in a survivor of the World Trade Center disaster.

Penetrating axillary trauma is classically associated with gunshot wounds or stab wounds, and it is a cause for concern because of the high risk of vascular injury. We report a case of penetrating axillary trauma to a victim of the World Trade Center disaster. In this case report, we discuss the CT and angiographic findings of both blunt and penetrating axillary injury, and present a diagnostic algorithm.

Lethal injuries and time to death in a level I trauma center.

BACKGROUND: The purpose of this study was to identify the causes and time to death of all trauma victims who died at a level I trauma center during an 11-year period. STUDY DESIGN: Autopsies were performed on all patients who died secondary to trauma. Retrospective review of these autopsies was carried out and appended to existing trauma registry data. Standard definitions were used to attribute the cause of death in each case. Preventable deaths were determined by a standardized peer review process. RESULTS: Between January 1985 and December 1995, a total of 900 trauma patients died. This r

Management of the injured colon: evolving practice at an urban trauma center.

The records of 239 patients surviving more than 24 hours with full-thickness intraperitoneal colonic injuries over a 7-year period were reviewed. During the first 3 years, 29% (31/106) of the patients were managed by primary repair without colostomy. In the next 4 years, almost twice as many patients, 56% (75/133), with similar colonic trauma were treated without fecal diversion (p less than 0.05). Although there was no difference in the mean Trauma Score in the patients with primary repair in the two time periods, the Injury Severity Score (mean +/- sd) in the patients without colostomy in th

Postconcussive hospital observation of alert patients in primary trauma center.

The medical justification for many postconcussive hospital admissions (one fourth of all trauma admissions) is unsettled, as are the potential legal consequences of nonadmission. Three hundred thirty-three patients with Class I level of consciousness (9) (alert, responsive to questions and complex commands, may be disoriented and/or confused) were admitted during a 6-month period. Seventy-nine per cent had sustained loss of consciousness. Eighty-one per cent were admitted solely for neurologic observation. Thirteen per cent of the patients had skull fractures. Patients admitted with localized

Should they stay or should they go? Who benefits from interfacility transfer to a higher-level trauma center following initial presentation at a lower-level trauma center.

BACKGROUND: Interfacility transfer of patients from Level III/IV to Level I/II (tertiary) trauma centers has been associated with improved outcomes. However, little data are available classifying the specific subsets of patients that derive maximal benefit from transfer to a tertiary trauma center. Drawbacks to transfer include increased secondary overtriage. Here, we ask which injury patterns are associated with improved survival following interfacility transfer. METHODS: Data from the National Trauma Data Bank was utilized. Inclusion criteria were adults (≥16 years). Patients with Injury Se…

Focused assessment with sonography for trauma exam for diagnosis of pericardial effusion in penetrating thoracic trauma - A retrospective review from a level 1 trauma center.

INTRODUCTION: Point of care ultrasound has long been used in the trauma setting for rapid assessment and diagnosis of critically ill patients. Its utility for diagnosis of pericardial effusion in the setting of penetrating thoracic trauma has more recently been a topic of consideration, given the rapid decompensation that these patients can experience. OBJECTIVES: This study aims to identify the diagnostic accuracy of point of care ultrasound in the diagnosis of pericardial effusion among patients with penetrating thoracic trauma. METHODS: Retrospective review of 2099 patients brought to the

Association Between Trauma Mechanism and Mandibular Fracture Pattern: A 13-Year Retrospective Analysis at a Regional Trauma Center.

The mandible, due to its anatomical position, mobility, and functional role, is one of the bones most frequently involved in maxillofacial trauma, with fracture patterns influenced by impact mechanisms and anatomical characteristics. This study aimed to analyse the relationship between trauma mechanisms and affected anatomical subsites in patients with isolated mandibular fractures treated at a regional public hospital in Buenos Aires Province. A retrospective cross-sectional observational study was conducted using medical records, surgical reports, and diagnostic imaging of patients treated b

Maxillofacial fractures associated with electric scooter trauma: a cohort study from a national level I trauma center.

PURPOSE: Electric scooters (e-scooters) have become a widely used mode of urban transportation, but their increasing popularity has been accompanied by a rise in trauma-related emergency department visits. Craniofacial injuries represent a particularly relevant subset due to the exposed riding position and the limited use of protective equipment. This study analyzed the epidemiological characteristics, fracture patterns, associated injuries, and treatment requirements of patients presenting with facial fractures after e-scooter accidents. METHODS: A retrospective observational study was condu

Clinical Epidemiology of Trauma Patients: A Retrospective Analysis of 3705 Consecutive Patients Treated at a Level I Trauma Center.

BACKGROUND: Trauma is a leading cause of morbidity and mortality globally, particularly in low- and middle-income (LMIC) countries like India, where road traffic injuries (RTIs) dominate. Despite advancements in medical technology, trauma care remains underdeveloped due to resource limitations, inadequate pre-hospital care, and poor compliance with safety measures. This study aimed to analyze the clinical-epidemiological profile of trauma patients and develop strategies for effective trauma prevention and management. METHODS: A retrospective review of 3,705 trauma patients admitted to a level
