Efficacy and Safety of Reteplase Versus Alteplase in Acute Ischemic Stroke Based on Fibrinogen Levels: The RAISE Trial Subgroup.

BACKGROUND: The effects of intravenous thrombolytic agents on fibrinogen differ due to structural differences among the agents. Using data from the RAISE (Reteplase Versus Alteplase for Acute Ischemic Stroke) trial, we aimed to investigate the impact of differences in baseline plasma fibrinogen levels on the efficacy and safety of reteplase versus alteplase within 4.5 hours of acute ischemic stroke symptom onset. METHODS: This post hoc subgroup analysis of the multicenter RAISE trial categorized participants by baseline fibrinogen levels: low (<2 g/L), normal (2-4 g/L), and high (>4 g/L). The

Optimizing Extracorporeal Cardiopulmonary Resuscitation Candidate Selection in out-of-Hospital Cardiac Arrest: A Machine-Learning Individualized Treatment Effect Approach Versus Rule-Based Criteria.

BACKGROUND: Extracorporeal cardiopulmonary resuscitation (ECPR) has demonstrated survival benefit in selected patients with out-of-hospital cardiac arrest, yet optimal selection criteria remain uncertain. Machine-learning-based individualized treatment effect (ITE) modeling may identify patients most likely to benefit by capturing heterogeneity of treatment response, potentially providing a more accurate strategy for ECPR candidate selection than current rule-based criteria. METHODS: We retrospectively analyzed adult, nontraumatic, emergency medical services-attended patients with out-of-hosp

Classification and regression trees to identify COPD subgroups in clinical trial populations: insights from the IMPACT trial.

BACKGROUND: Heterogeneity in chronic obstructive pulmonary disease (COPD) challenges the identification of optimal treatment populations. Subgroups derived from real-life cohorts using classification and regression trees (CARTs) have shown prognostic value for mortality, but their applicability to clinical trial populations to predict treatment response needs to be further investigated. We sought to evaluate whether previously identified CART-based subgroups are relevant for predicting outcomes and treatment response in the IMPACT trial, and to assess the stability of these subgroups using de

Pharmacogenomics of antiepileptic drug mood stabilizer treatment response in bipolar disorder: A MoStGen Consortium study.

Identifying biological and clinical factors associated with response to mood-stabilizing medications is critical for improving bipolar disorder (BD) treatment. The Mood Stabilizer Genomics (MoStGen) Consortium was established to investigate pharmacogenomic and clinical predictors of response to treatment of BD with antiepileptic drug mood stabilizers (AMS). Here we present the first pharmacogenomic analyses of AMS treatment outcomes based on MoStGen Consortium data, including 917 individuals across contributing sites. We performed genome-wide association analyses in subcohorts followed by meta

Pharmacogenomics for stratified antidepressant treatment in major depressive disorder: evidence, limits and a roadmap for clinical use.

Major depressive disorder (MDD) is treated largely by trial and error, despite marked interindividual variation in antidepressant exposure, efficacy and tolerability. Pharmacogenomics (PGx) has therefore been proposed as a route to stratified antidepressant treatment. This Review asks whether current evidence supports that claim by distinguishing actionable drug-gene guidance, clinical utility in broad trial populations and subgroup evidence for stratified care. Contemporary guidance supports genotype-informed prescribing for selected antidepressants, most consistently through CYP2D6 and CYP2C

Seamless dose optimization design accounting for unknown patient heterogeneity in cancer clinical trials.

Project Optimus, an initiative by the FDA's Oncology Center of Excellence, seeks to reform the dose-optimization and dose-selection paradigm in oncology. We propose a dose-optimization design that considers plateau efficacy profiles, integrates pharmacokinetic data to inform the exposure-toxicity curve, and accounts for patient characteristics that may contribute to heterogeneity in response. The dose-optimization design is carried out in two stages. First, a toxicity-driven stage estimates a safe set of doses. Then, a dose-ranging efficacy-driven stage explores the set using response and pati

Linking electric field directionality to treatment outcome in OCD: Insights from patient-specific tDCS modeling.

Transcranial direct current stimulation (tDCS) is a promising intervention for treatment-resistant obsessive-compulsive disorder (OCD), yet clinical outcomes remain inconsistent. To investigate the neural mechanisms underlying therapeutic variability, we conducted a patient-specific finite element (FE) modeling study of electric fields (EF) induced by tDCS in OCD patients. Forty-two patients from a double-blind, randomized clinical trial received active tDCS with the cathode over the pre-supplementary motor area and the anode over the right supraorbital region. Clinical response was assessed u

Ketamine effects on EEG and their links to therapy differ across treatment-resistant major depression, post-traumatic stress disorder, and obsessive-compulsive disorder.

INTRODUCTION: Neurotic disorders-major depressive disorder (MDD), panic disorder, social anxiety disorder, generalized anxiety disorder, obsessive-compulsive disorder (OCD), post-traumatic stress disorder (PTSD), and specific phobia-have differing pharmaceutical profiles. But all, even when resistant to conventional treatment (TR), respond quickly to low-dose (0.5-1.0 mg/kg I.M.) ketamine. We explore the variation in the neural effects of ketamine across its treatments of TR-MDD, TR-PTSD, and TR-OCD. MATERIALS AND METHODS: We recorded 10 min of resting frontal activity and diagnosis-related s
