Small Opening, Big Problem: Esophageal Food Bolus Impaction and Microstomia.

Patients presenting to the hospital with esophageal food bolus impaction often need urgent upper endoscopy. However, patients with trismus and microstomia can pose a real challenge because endoscopic access in these patients can be difficult. We present a unique transoral endoscopic approach for esophageal food bolus disimpaction in a patient with microstomia and trismus resulting from chronic graft-vs-host disease.

Suspected Succinylcholine-Induced Masseter Muscle Rigidity During Rapid Sequence Intubation in the Air Medical Setting.

Trismus, or masseter muscle rigidity, is a rare but previously described complication of succinylcholine-induced neuromuscular blockade. We present a case report that details unique aspects of suspected masseter muscle rigidity in the prehospital setting air medical setting after attempted rapid sequence intubation with succinylcholine. We then discuss the need for knowledge base of this life-threatening and rarely described syndrome as well as the importance of working through a differential diagnosis and specific trismus-focused airway algorithm. Trismus, or masseter muscle rigidity (MMR), i…

Cephalic tetanus as a differential diagnosis of facial nerve palsy.

Cephalic tetanus is defined as a combination of trismus and paralysis of one or more cranial nerves. Since it is a rare cause of facial nerve palsy, it is often overlooked as a differential diagnosis. We present the case of a 75-year-old man admitted to our emergency department with left facial nerve palsy, left ptosis and dysphagia 2 weeks after a head laceration. Soon after admission, he was orally intubated because of massive aspiration. A delayed diagnosis of cephalic tetanus was reached only when he was extubated and trismus and risus sardonicus were identified in addition to facial nerve…

A clinical prospective study of peritonsillar abscess in children.

Distinguishing peritonsillar abscess from cellulitis is an important clinical problem, particularly in children, who may require a general anesthetic for drainage of these abscesses. In order to identify those clinical factors most significant for peritonsillar abscess, we did a prospective study of 21 patients who presented with sore throat, fever, trismus, and tonsillar bulge; all symptoms that are consistent with the diagnosis of peritonsillar abscess. On admission, the following parameters were recorded: patient age, duration of sore throat, fever, white blood cell count, drooling, the deg…

How I do it: transpharyngeal resection of base of tongue cancer with hyoid suspension.

OBJECTIVE: Transoral robotic surgery is increasingly used for selected base-of-tongue tumours, but is often impractical in patients with severe trismus and remains cost prohibitive. Conventional transmandibular approaches are associated with significant morbidity. We describe a transpharyngeal technique incorporating hyoid suspension to address these limitations. METHODS: A retrospective series of patients undergoing transpharyngeal resection for base-of-tongue carcinoma is presented. The technique includes neck dissection with vascular control, lateral-inferior pharyngotomy, tumour excision …

Management of restricted mouth opening caused by radiation: A clinical report.

Trismus, a commonly observed sequela in patients who have undergone radiation therapy to treat malignancies of the head and neck, causes a loss of function that reduces patients' overall quality of life. Radiation can cause intense fibrosis in the masticatory muscles, and this fibrosis may lead to trismus. This clinical report describes the management of a patient with radiation-induced trismus who was treated with a custom-made mouth-opening device. The device had maxillary and mandibular sections, was made of a thermoactive acrylic resin material, and was connected with 2 vertical screws in …

Two cases of masticator space abscess initially diagnosed as temporomandibular joint disorder.

Diseases causing trismus or pain of the temporomandibular joint (TMJ) include temporomandibular joint disorders (TMJD), trauma, tumors and an inflammation. This report describes two cases of masticator space abscesses causing trismus and TMJ pain, which were initially diagnosed as TMJD. The first case was a 62-year-old female suffering from trismus and right-TMJ pain without swelling or redness, which was diagnosed as TMJD with MRI (TMJ region) findings of bilateral anterior disc displacement without reduction. However, five days later the right-TMJ pain became more severe and MRI (maxillofaci…
