Proteome Profiling in Cerebrospinal Fluid Reveals Increased Levels of Peroxiredoxin 2 Discriminating Japanese Encephalitis Virus and Scrub Typhus Infection.

Japanese encephalitis virus (JEV) and scrub typhus (ST) are major etiological agents of acute encephalitis syndrome (AES) in India and South Asia. The pathophysiological changes at the molecular level caused by JEV and ST have yet to be studied in detail. The cerebrospinal fluid (CSF) proteomic landscape is a critical indicator of CNS pathology. Here, we conducted label-free quantitative proteomics on CSF from AES patients (n = 15) to identify etiology-specific differentially expressed proteins (DEPs) linked to encephalitis. The key DEPs were validated via ELISA in CSF (n = 49) and serum (n =

Scrub typhus (Orientia tsutsugamushi)-associated intracranial infection: a case report and review of diagnostic and therapeutic challenges.

Intracranial infections caused by Orientia tsutsugamushi (O. tsutsugamushi), the causative agent of scrub typhus, are rare but potentially fatal. This case report describes a 45-year-old male patient presenting with persistent fever, headache, altered mental status, and skin lesions. High-throughput sequencing of cerebrospinal fluid confirmed O. tsutsugamushi, while respiratory samples detected Pseudomonas aeruginosa and Aspergillus fumigatus, suggesting a mixed bacterial and fungal infection. After timely adjustment of anti-infective treatment, the patient's condition significantly improved. …

A Seasonal Autoregressive Integrated Moving Average (SARIMA) forecasting model to predict the epidemic trends of scrub typhus in China.

Scrub typhus is a deadly infectious disease that is frequently underdiagnosed. Forecasting the emergence of infectious diseases using epidemiological models has emerged as a crucial instrument for comprehending the dynamics of their occurrence. This research aimed to investigate epidemic traits and create a predictive model for scrub typhus in mainland China, employing the Seasonal Autoregressive Integrated Moving Average (SARIMA) time series method. Monthly records of scrub typhus cases were gathered from the China Center for Disease Control and Prevention, covering the timeframe from 2006 to

Reversible high-grade AV block in a patient with scrub typhus.

A man in his late 30s presented with a prolonged febrile illness and recurrent syncopal episodes. Physical examination revealed hypotension, bradycardia and eschars on the bilateral thighs. A 12-lead ECG showed complete heart block with wide QRS escape rate of 30 beats per minute. Laboratory tests confirmed scrub typhus with elevated cardiac markers. The patient was managed with a temporary transvenous pacemaker and intravenous doxycycline which led to rapid resolution of symptoms and complete normalisation of cardiac conduction within 48 hours. This case highlights the importance of scrub typ

The Association Between Environmental Factors and Scrub Typhus: A Review.

Scrub typhus is an acute febrile vector-borne infectious disease caused by Orientia tsutsugamushi (O. tsutsugamushi) and transmitted through the bite of infected chigger mite larvae. Transmission involves complex ecological interactions among vectors, hosts, and environmental factors. Accumulating evidence indicates complex interactions between the scrub typhus incidence and multilevel environmental determinants, encompassing meteorological factors (e.g., temperature, humidity, precipitation, wind speed, sunshine duration, and atmospheric pressure), geographical conditions (e.g., topography, e…

Molecular species delimitation analysis of Leptotrombidium spp. and other chigger species parasitizing birds in Malaysia.

Trombiculid mites (Acariformes) are unique among arthropods of medical importance in that only the larval instar (chigger) is parasitic, which can result in the transmission of zoonotic scrub typhus. The use of molecular approaches for chigger species discrimination has been very limited until recently, especially for those parasitizing bird hosts, where data remain scarce. Here, we aimed to generate DNA barcodes of chiggers parasitizing birds in Malaysia based on the mitochondrial cytochrome c oxidase subunit I (COI) gene following DNA extraction, PCR and sequencing. Fifty-four COI sequences

Comparative virulence analysis of seven diverse strains of Orientia tsutsugamushi reveals a multifaceted and complex interplay of virulence factors responsible for disease.

Orientia tsutsugamushi is an obligate intracellular bacterium found in Leptotrombidium mites that causes the human disease scrub typhus. A distinguishing feature of O. tsutsugamushi is its extensive strain diversity, yet differences in virulence between strains are not well defined nor well understood. We sought to determine the bacterial drivers of pathogenicity by comparing seven strains using murine infections combined with epidemiological human data to rank each strain in terms of relative virulence. Murine cytokine expression data revealed that the two most virulent strains, Ikeda and Kat…

Assessment of influential factors and risk area prediction of scrub typhus in Gannan region, China, based on ecological niche modelling and geographical detector.

This study sought to define the potential infection risk zone of scrub typhus in Gannan and investigate the factors that contribute to this risk zone. Based on the distribution data of scrub typhus cases and environmental factors in Gannan from 2015-2021, a maximum entropy ecological niche modelling was employed to forecast potential risk areas in the region. The influence of each environmental factor on the risk was quantified using a geographical detector. The maximum entropy model's prediction remained robust, with an average area under the curve value of 0.852. The key predictor variables

The intracellular bacterium Orientia tsutsugamushi uses the autotransporter ScaC to activate BICD adaptors for dynein-based motility.

The intracellular bacterium Orientia tsutsugamushi relies on the microtubule cytoskeleton and the motor protein dynein to traffic to the perinuclear region within infected cells. However, it remains unclear how the bacterium is coupled to the dynein machinery and how transport is regulated. Here, we discover that O. tsutsugamushi uses its autotransporter protein ScaC to recruit the dynein adaptors BICD1 and BICD2 for movement to the perinucleus. We show that ScaC is sufficient to engage dynein-based motility in the absence of other bacterial proteins and that BICD1 and BICD2 are required for e…

Comparative Analysis of Scrub Typhus Rapid Kits and Their Relevance in Screening and Diagnosis.

BACKGROUND: Rapid diagnostic tests (RDTs) are increasingly recognized as practical tools for the early detection of infectious diseases, particularly in resource-limited settings. These tests offer the advantage of portability, ease of use, and quick turnaround times, making them ideal for point-of-care diagnostics. This study evaluates the diagnostic performance of three commercially available RDTs for scrub typhus, using IgM and IgG enzyme-linked immunosorbent assays (ELISAs) as reference standards, to assess their reliability and applicability in clinical and field settings. METHODS: A cro

Acute undifferentiated febrile illness in a traveler returning from Burkina Faso, West Africa to Puducherry, India- a case report.

BACKGROUND: Imported malaria remains a significant diagnostic challenge, particularly in regions approaching elimination. This case report is novel in its exploration of the complexities involved in diagnosing malaria imported from a high-endemic area to a low-endemic region, emphasizing the critical role of detailed travel history and molecular diagnostics in identifying the disease. The case underscores the potential public health implications of missing such diagnoses in regions where routine malaria testing is not performed due to low endemicity. CASE PRESENTATION: A 25-year-old male pres
