Incidence, clinical and epidemiological risk factors, and outcome of drug-induced hepatitis due to antituberculous agents in new tuberculosis cases.

Drug-induced hepatitis (DIH) is an important issue in tuberculosis (TB) treatment. We intend to assess the incidence, risk factors, and outcome of hepatitis due to anti-TB drugs. The study is carried out at the national TB referral center 2006-2008 including all documented new cases of TB. All patients received standard anti-TB treatment. If DIH occurred, all drugs were discontinued and reinitiated after liver function tests (LFT) normalization in a stepwise way. Of total 761 patients, 99 (13.0%) patients developed DIH during anti-TB treatment. There was no difference in sex, nationality, smok

Rare detection of Hepatitis C virus genotype 4 in a patient with pulmonary tuberculosis: First report from central India.

Hepatitis C virus (HCV) genotype 4 (GT4) is exceptionally rare in central India. We report the first molecularly confirmed case of HCV GT4a in a 26-year-old severely malnourished male (body weight 39 kg) presenting with active pulmonary tuberculosis (TB) in Madhya Pradesh. Following the detection of active viremia (1.1 × 105 IU/mL), Sanger sequencing of the Core region and nucleotide homology analysis definitively confirmed genotype 4a. Despite compounding hepatotoxic risks from severe cachexia and viral co-infection, no hyperacute or immediate acute hepatic injury was observed during the init

Fulminant hepatitis during treatment for per-extensively drug-resistant tuberculosis: A case report and call for improved patient monitoring.

RATIONALE: Drug therapy is the most effective therapy for tuberculosis; however, it can also lead to a common and serious adverse reaction known as antituberculosis drug-induced liver injury. The incidence of antituberculosis drug-induced liver injury ranges from 2.0% to 28.0%. PATIENT CONCERNS: Here, we report the successful treatment of drug-induced fulminant hepatitis in a 19-year-old patient with extensively drug-resistant tuberculosis. DIAGNOSES: The patient was treated with a combination of bedaquiline, linezolid, cycloserine, clofazimine, amikacin, ethambutol, and pyrazinamide. This c

Risk of drug-induced liver injury in chronic hepatitis B and tuberculosis co-infection: A systematic review and meta-analysis.

Patients with tuberculosis (TB) disease treated with multi-drug regimens are at risk of developing drug induced liver injury (DILI), and DILI risk might be even higher in patients with underlying liver disease. We aimed to evaluate whether underlying chronic hepatitis B virus (HBV) and TB co-infection are associated with a higher risk of TB therapy-related DILI. We conducted a systematic review and meta-analysis using MEDLINE/PubMed from inception to 31 December 2021. Primary outcome assessed was development of DILI following multi-drug TB treatment. Meta-analysis using random-effects models w

Improving healthcare for patients with HIV, tuberculosis and hepatitis C in eastern Europe: a review of current challenges and important next steps.

OBJECTIVES: In some eastern European countries, serious challenges exist to meet the HIV-, tuberculosis (TB)- and hepatitis-related target of the United Nations Sustainable Development Goals. Some of the highest incidence rates for HIV and the highest proportion of multi-drug-resistant (MDR) tuberculosis worldwide are found in the region. The purpose of this article is to review the challenges and important next steps to improve healthcare for people living with TB, HIV and hepatitis C (HCV) in eastern Europe. METHODS: References for this narrative review were identified through systematic se

Epidemiology of HIV, Sexually Transmitted Infections, Viral Hepatitis, and Tuberculosis Among Incarcerated Transgender People: A Case of Limited Data.

Transgender people experience a disproportionate burden of human immunodeficiency virus (HIV) and incarceration. Discrimination, victimization, poverty, and poor mental health drive vulnerability to HIV and related infections, as well as risk of arrest, detention, and incarceration. In this paper, we systematically review published data on HIV, sexually transmitted infections, viral hepatitis, and tuberculosis among incarcerated transgender people; describe potential structural determinants of HIV risk and transmission; identify gaps in the literature; and make recommendations for research and

Takayasu's Arteritis with Isolated Pulmonary Artery Involvement in a Middle-Aged Asian Woman with Hepatitis B and Latent Tuberculosis Infection.

We report a rare case of Takayasu's arteritis with isolated pulmonary artery stenosis in the presence of active hepatitis B and latent Mycobacterium tuberculosis infection in a middle-aged Asian woman who initially presented with severe dyspnea on exertion and recurrent syncope, occasional burning chest pains, and fatigue. Therapy of the active hepatitis B and latent M. tuberculosis infection together with a course of methotrexate led to a slight reversal of the symptoms, as angioplasty with or without stenting was not an option. The constellation described here hints at the possible link betw

High prevalence of hepatitis B virus infection among tuberculosis patients with and without HIV in Rio de Janeiro, Brazil.

To determine the prevalence and exposure factors associated with hepatitis B infection in tuberculosis patients with and without HIV type 1 coinfection, the presence of hepatitis B virus serological markers was investigated in a retrospective study. The seroprevalence of hepatitis B virus in patients with tuberculosis only was 14.6%, and in tuberculosis patients coinfected with HIV it increased to 35.8%. In patients with HIV and tuberculosis coinfection, homosexuality constituted the principal exposure factor, while in tuberculosis patients without HIV, a gradual increase in hepatitis B virus

Isoniazid preventive therapy, hepatitis C virus infection, and hepatotoxicity among injection drug users infected with Mycobacterium tuberculosis.

Treatment of latent Mycobacterium tuberculosis infection with isoniazid can cause hepatotoxicity, but the risk of isoniazid-associated hepatotoxicity among persons coinfected with hepatitis C virus (HCV) is unknown. We conducted a prospective study among 146 injection drug users with M. tuberculosis infection and normal baseline hepatic transaminase values who were treated with isoniazid. Of 146 participants, 138 (95%) were HCV-seropositive. Thirty-seven participants (25%) were human immunodeficiency virus (HIV)-seropositive. Thirty-two (22%; 95% confidence interval [CI], 16%-30%) of 146 parti

A systematic review on community-based screening of newly arrived migrants in Europe for tuberculosis, human immunodeficiency virus, and hepatitis B and C.

Increases in the number of migrants (economic, educational, and involuntary) to Europe from countries with high incidence of communicable diseases [tuberculosis (TB), HIV, and hepatitis B (HBV) and C (HCV)]; has increased the need for cost-effective early disease diagnosis programmes to improve outcomes. We aimed to synthesize and evaluate current literature on community-based screening (CBS) initiatives in Europe, the diseases being screened for, and acceptance when offered. Biologic therapy in geriatric psoriasis: 6-month real-world data on PASI, inflammatory indices, and hepatitis B/tuberculosis safety.

Real-world evidence regarding biologic therapy in geriatric psoriasis is limited, particularly concerning systemic inflammatory burden and infection-related safety. This study evaluates the clinical efficacy of biologic therapy and its impact on systemic inflammatory indices while emphasizing safety related to hepatitis B virus (HBV) serology and tuberculosis screening. We conducted a retrospective analysis of eighty biologic-naïve patients aged 65 years and older with plaque psoriasis undergoing biologic therapy. Patients were categorized by biologic class: tumor necrosis factor-α (TNF-α) inh

Tuberculosis and Chronic Hepatitis B Virus Infection Screening Among Non-US-Born Persons in an Integrated Health System in California.

BACKGROUND: Tuberculosis infection (TBI) and chronic hepatitis B virus (HBV) infection disproportionately affect non-US-born persons. Early identification and treatment are critical to reduce transmission, morbidity, and mortality, but little is known about screening in the United States. METHODS: We conducted a cross-sectional study in a large integrated California health system in September 2022 assessing TBI and HBV screening among persons aged ≥18 years who were born in countries with high TB burden (TB disease incidence rates ≥20/100 000 population) and/or HBV burden (hepatitis B surface

Diabetes Among People With Tuberculosis, HIV Infection, Viral Hepatitis B and C, and STDs in New York City, 2006-2010.

Matching infectious disease surveillance data has become a routine activity for many health departments. With the increasing focus on chronic disease, it is also useful to explore opportunities to match infectious and chronic disease surveillance data. To understand the burden of diabetes in New York City (NYC), adults with select infectious diseases (tuberculosis, HIV infection, hepatitis B, hepatitis C, chlamydial infection, gonorrhea, and syphilis) reported between 2006 and 2010 were matched with hemoglobin A1c results reported in the same period. Persons were considered to have diabetes wi

Estimating the effect of hepatitis C infection on multidrug-resistant tuberculosis treatment outcomes under hypothetical interventions on regimen composition and adherence.

Hepatitis C virus (HCV) infection is associated with unfavorable multidrug- and rifampicin-resistant (MDR/RR) tuberculosis (TB) outcomes. We examined whether this association would decrease in settings where no participants were lost-to-follow-up or where all adhered to regimens comprised of priority TB drugs. We analyzed data from 1,530 participants with HCV testing in the endTB observational cohort (NCT03259269). We estimated the relative risk of death, treatment failure, and loss-to-follow-up comparing participants with and without HCV, using inverse probability weighting to adjust for conf

INCIDENCE OF TUBERCULOSIS AMONG HIV/HCV CO-INFECTED PATIENTS RECEIVING HEPATITIS C TREATMENT WITH PEGYLATED INTERFERON AND RIBAVIRIN IN GEORGIA.

Treatment of hepatitis C is necessary for ensuring higher life expectancy among HIV/HCV co-infected patients. However antiviral treatment for chronic HCV infection with Pegylated interferon (PEG-IFN) and Ribavirin (RBV) is associated with a variety of side effects. In Georgia up to 22% of HIV-infected patients were found to have active Tuberculosis (TB) and 22.4 to 32.6% had latent TB. The objective of this study was to describe the characteristics and clinical outcomes of tuberculosis in HIV/HCV co-infected patients receiving hepatitis C treatment with pegylated interferon and ribavirin and c
