Tuberculosis.

Tuberculosis remains the leading cause of death from an infectious disease among adults worldwide, with more than 10 million people becoming newly sick from tuberculosis each year. Advances in diagnosis, including the use of rapid molecular testing and whole-genome sequencing in both sputum and non-sputum samples, could change this situation. Although little has changed in the treatment of drug-susceptible tuberculosis, data on increased efficacy with new and repurposed drugs have led WHO to recommend all-oral therapy for drug-resistant tuberculosis for the first time ever in 2018. Studies hav

Primary extensive perianal cutaneous tuberculosis: A challenging diagnosis. Case report and a review of the literature.

RATIONALE: Perianal tuberculosis is extremely rare without previous or active pulmonary infection. Ulcerative skin lesion is a rare presentation of extrapulmonary tuberculosis in the oral, perianal, or genital mucosa and the adjacent skin. CASE REPORT: A 71-year-old woman complained of pain during evacuation and fecal incontinence for two years. There was an ulcerated lesion in the perianal and intergluteal region and perianal fistulous tracts. A polymerase chain reaction test on blood and biopsies of perianal ulcers, perianal fistula, and the intergluteal area was positive for Mycobacterium

Mediastinal tuberculosis following descending necrotizing mediastinitis: A case report.

Tuberculosis is a disease that causes latent infection and is sometimes activated by a variety of factors. Descending necrotizing mediastinitis (DNM) is a serious disease caused by spreading oropharyngeal infection. We present a case of mediastinal tuberculosis following mediastinal dissection and antibiotic therapy for DNM. A 62-year-old man was admitted to the hospital with an increasing mass in the right mediastinum during outpatient follow-up after surgical drainage and antibiotic treatment for idiopathic cervical abscess and left DNM caused by oral bacteria. The patient underwent right me

Primary tuberculous gingival enlargement: a rare entity.

With the advent of effective drug therapy, tuberculous lesions of the oral cavity have become so rare that they are frequently forgotten. Primary gingival tuberculosis is extremely rare and usually manifests as ulcer. We report the first case of primary tuberculosis manifesting as gingival enlargement, which was the only presenting sign of tuberculosis. Diagnosis was based on histopathology (hematoxin and eosin staining), complete blood count, polymerase chain reaction assay and immunologic investigation with the detection of antibodies against Mycobacterium tuberculosis. The possibility of gi

Abdominal tuberculosis with autoimmune hemolytic anemia.

An eight-year-old male child presented with progressive distension of abdomen, fever, pallor and jaundice with a history of tubercular contact. Investigations were suggestive of abdominal tuberculosis with autoimmune hemolytic anemia. The child responded well to a course of oral steroids with antitubercular treatment. A literature search did not reveal any previous case report of an association between tuberculosis and autoimmune hemolytic anemia.

Primary Disseminated Pre-extensively Drug-Resistant Tuberculosis of the Lungs, Pleura, Chest Wall, and Abdomen: The World's First Case.

Drug-resistant tuberculosis is a perpetual threat to public health. In recent years, there has been an increase in the number of cases of this deadly Mycobacterium tuberculosis infection. The present case is a very rare case of primary disseminated pre-extensively drug-resistant tuberculosis of the lungs, pleura, chest wall, and abdomen in a 19-year-old Indian female patient who presented with fever, cough, abdominal pain, and anterior chest wall swelling. The diagnosis was established by a detailed laboratory and radiological workup. An all-oral longer regimen was initiated per national guide

Lipoarabinomannan-reactive human secretory immunoglobulin A responses induced by mucosal bacille Calmette-Guérin vaccination.

The ability of 17 recombinant mycobacterial proteins, native antigen 85 complex, lipoarabinomannan (LAM), and Mycobacterium tuberculosis lysate to detect antibody responses induced by bacille Calmette-Guérin (BCG) vaccination and active tuberculosis infection were studied in enzyme-linked immunosorbent assays. Only LAM-reactive serum immunoglobulin G responses were significantly increased in both BCG-vaccinated patients and patients with active tuberculosis (P<.05), and oral BCG vaccination also induced significant increases in LAM-reactive secretory immunoglobulin A (P<.05). LAM-reactive anti

[Perinatal tuberculosis presenting as endobronchial tuberculosis diagnosed by bronchoscopy: a case report].

A 3-month and 17-day-old male infant presented with a 4-week history of cough and was initially diagnosed with pneumonia. Despite over 20 days of antibiotic therapy (including ceftriaxone, piperacillin-tazobactam, and vancomycin), his symptoms did not improve. Chest CT revealed worsening pulmonary infiltrates. Bronchoscopic findings revealed stenosis of the left main bronchus with mucosal erosion, granuloma, and caseous necrosis. Rapid on-site evaluation of bronchoalveolar lavage fluid demonstrated positivity on acid-fast staining. Together with positive T-SPOT and high-throughput sequencing r

Direct molecular detection of Mycobacterium tuberculosis suspected to be the specific infection in a case of recurrent tonsillitis.

We report a case of a 21-year-old man with recurrent tonsillitis caused by Mycobacterium tuberculosis. For a period of 5 months, the patient had tonsillitis seven times and was treated with several oral or parenteral antibiotics. On one of these occasions, tonsillitis was complicated with a peritonsillar abscess that was treated by incision. According to relevant bibliographic data, this is the first case of Mycobacterium tuberculosis confirmed by direct molecular microbiology methods from the tonsillar tissue of a young immunocompetent male reported in Europe. In a case of recurrent tonsillit

Primary tuberculous glossitis in an immunocompetent patient.

Tuberculous glossitis is a rare entity that has been described sporadically. Primary tuberculous glossitis, as described in this case report, is still exceptional. A 25-year-old male with no known immunosuppressive disorder presented with a tuberculoma at the base of his tongue. This was confirmed by tongue biopsy and a positive polymerase chain reaction response to the mycobacterium. The patient had a favourable response to anti-tubercular treatment. This highlights the importance of considering tuberculosis in the differential diagnosis of chronic tongue lesions, even in the absence of pulmo

Tuberculoma of the tongue presenting as macroglossia.

Lingual tuberculosis presenting as macroglossia in a 20-year-old woman has been reported. The histopathologic picture was similar to lupus vulgaris and the intradermal tuberculin test showed a strongly positive vesiculobullous reaction after forty-eight hours. No active internal focus of tubercular infection could be detected. The patient responded well to antituberculous chemotherapy. The relative lack of symptoms suggest that the enlargement had been a slow process resulting from the lodging of bacilli in the tongue.

The Diagnostic Dilemma of a Granulomatous Gingival Enlargement.

INTRODUCTION: The oral cavity is considered an easily accessible window to the body. The mouth is frequently involved in conditions that affect multiple organs. In many instances, oral involvement precedes the appearance of many other symptoms or lesions. A complete examination of the oral cavity provides a gateway for an accurate diagnosis and precise management of many systemic conditions. Gingival enlargement is one of the varied manifestations of many systemic diseases. Here, a case report of a gingival enlargement is presented that provided information for the diagnosis of post-primary pu

Lingual ulcer as the only sign of recurrent mycobacterial infection in an HIV/AIDS-infected patient.

The report describes an HIV/AIDS patient seen at a referral center in Mexico City, in whom a mycobacterial infection in the oral mucosa, probably tuberculosis (TB) was identified. The purpose is to describe the clinical and histological findings in an HIV-infected patient, who after being treated successfully for tuberculous lymphangitis 4 years ago, presented with a lingual ulcer as the only suggestive sign of recurrence of mycobacterial infection, probably M. tuberculosis. A 39-year-old man seen in the HIV clinic of the Instituto Nacional de Ciencias Médicas y Nutrición "Salvador Zubirán" in

Disseminated Penicillium marneffei mimicking paradoxical response and relapse in a non-HIV patient with pulmonary tuberculosis.

Clinical deterioration during the treatment of tuberculosis remains a diagnostic challenge. We describe the case of a 46-year-old man with a history of oral cancer status after a radical operation who had pulmonary tuberculosis with pleura and neck lymph node involvement. The clinical condition improved after antituberculosis therapy. However, the patient suffered from low-grade fever, progressive dyspnea, and cough after 7 weeks of the therapy. The findings of chest plain films were relapse and progression of left lung haziness. The deterioration was caused by disseminated Penicillium marneff

Tuberculosis of parotid gland: a rare clinical entity.

Tuberculosis (TB) can affect almost any organ of the body; however, TB of the salivary glands is a rare condition. The purpose of this paper was to report a case of a 14-year-old boy who presented with a painless swelling and discharging sinus in the parotid gland and cervical region on the left side. There was no clinical evidence of systemic signs and symptoms of active TB elsewhere in the body. The combination of clinical suspicion, a positive family history, and the cytological findings confirmed the diagnosis. The patient was successfully treated via antitubercular therapy for six months.
