Weighted vaginal cones for urinary incontinence.

BACKGROUND: For a long time pelvic floor muscle training (PFMT) has been the most common form of conservative (non-surgical) treatment for stress urinary incontinence (SUI). Weighted vaginal cones can be used to help women to train their pelvic floor muscles. Cones are inserted into the vagina and the pelvic floor is contracted to prevent them from slipping out. OBJECTIVES: The objective of this review is to determine the effectiveness of vaginal cones in the management of female urinary stress incontinence (SUI).We wished to test the following comparisons in the management of stress incontin…

New drug treatments for urinary incontinence.

Urinary incontinence remains a common and distressing condition affecting many women and is known to have a significant effect on quality of life (QoL). Whilst conservative and behavioural therapy are important in the management of women with both stress incontinence and overactive bladder (OAB) ultimately many may benefit from pharmacological therapy. Antimuscarinic drugs are the commonly used agents in the treatment of OAB although often compliance and persistence are affected by adverse effects. Consequently many newer agents remain under investigation. In addition duloxetine has recently b…

[Prevention of postoperative urinary stress incontinence].

Postoperative stress incontinence following operative treatment of prostate cancer represents a considerable percentage of overall male incontinence. Postoperative incontinence following radical prostatectomy ranges between 2.9 and 87% depending on author and patient characteristics. Especially patient-related factors such as body mass index, age, size of prostate, preoperative incontinence, and concomitant diseases as well as classification of incontinence and modality of data collection influence postoperative continence rates. However, recent publications demonstrate the important impact of

Urinary incontinence management in geriatric patients.

OBJECTIVE: To review prevalence, causes, types, and appropriate therapy to treat urinary incontinence (UI) in geriatric patients. DATA SOURCES: Selection and extraction from a PubMed/MEDLINE, English-language literature search from 2000 to the present using the search terms urinary incontinence and geriatrics. DATA SYNTHESIS: UI is a prevalent condition in geriatric patients. Unmanaged UI decreases quality of life and increases the risk of perineal skin breakdown, which can lead to painful skin irritation and pressure sores. Identifying the type of UI, and treating or eliminating transient c…

Mixed urinary incontinence: continuing to confound?

PURPOSE OF REVIEW: Mixed incontinence remains a complex clinical problem for urogynaecologists and generalists alike, as research for new tr … gical treatment have also improved. SUMMARY: There is greater awareness of the prevalence of mixed urinary incontinence, which has a large impact on the quality of life of women, irrespective of their desire to seek medical help. Tools are available to identify different subgroups within the sphere of mixed urinary incontinence, and, once accurately assessed, patient …

[Drug therapy of female urinary incontinence].

Drug treatment for female urinary incontinence requires a thorough knowledge of the differential diagnosis and pathophysiology of incontinence as well as of the pharmacological agents employed. Pharmacotherapy has to be tailored to suit the incontinence subtype and should be carefully balanced according to efficacy and side effects of the drug. Women with urge incontinence require treatment that relaxes or desensitizes the bladder (antimuscarinics, estrogens, alpha-blockers, beta-mimetics, botulinum toxin A, resiniferatoxin, vinpocetine), whereas patients with stress incontinence need stimulat…

[Surgical therapy of female urinary incontinence].

Within the past years, surgical concepts for treating females with urinary incontinence have greatly changed. The spectrum of indications is becoming increasingly narrower. All possible conservative treatment modalities must first be attempted. Should the incontinence still continue to evoke social or hygienic problems, stress incontinence is usually treated with the minimally invasive TVT procedure (Tension-free Vaginal Tape), and in special cases, a modern modification of colposuspension is undertaken. Based on a success rate and specific complications, it is now known which slings and colpo…

OBJECTIVES: To know the results of a pelvic floor muscle training program in the treatment of female stress urinary incontinence. METHODS: Information phase: patient is informed about the disease, her anatomy, the objectives to be reached and how to perform the exercises. Treatment phase: 6 weeks of visual and auditory biofeedback assisted pelvic floor exercises to develop the pelvic floor muscles. Follow-up phase: one, 3, 6 month and 1 year visits with control for muscle evolution, motivation reinforcement and improvement of the symptoms. RESULTS: 412 Women underwent pelvic floor rehabilita…

Laparoscopic management of female urinary incontinence.

Recent technologic developments in laparoscopic reconstructive surgery have generated an interest in the laparoscopic approach to bladder neck suspension. There have been numerous descriptions of a variety of techniques for the laparoscopic approach to bladder neck suspension. Initial reports seemed to suggest satisfactory rates of improvement in the stress urinary incontinence of these patients. Long-term follow-up has shown that although this minimally invasive approach to the management of stress urinary incontinence is associated with a short duration of urinary diversion, minimal postoper…

Weighted vaginal cones for urinary incontinence.

BACKGROUND: Pelvic floor muscle training has long been the most common form of conservative treatment for stress urinary incontinence. Many people have trouble identifying their pelvic floor, and thus difficulty training it. One aid to help with training the pelvic floor in women is weighted vaginal cones. These are inserted into the vagina and the pelvic floor is contracted to prevent them slipping out. OBJECTIVES: To evaluate the effects of weighted vaginal cones in the treatment of female stress urinary incontinence. SEARCH STRATEGY: We searched the Cochrane Incontinence Group trials regi…

Repeated lipoinjections for stress urinary incontinence.

A total of 30 women with stress incontinence underwent periurethral injection of autologous fat under spinal anesthesia. The fat was harvested from the abdominal wall by liposuction. Preoperative evaluation consisted of history, physical examination, and urodynamic evaluation. For study purposes, some patients also underwent bladder and urethral ultrasonography and magnetic resonance imaging studies. The first 13 patients received a single periurethral lipoinjection, and the following 17 patients received sequential injections when needed at 3-month intervals. Results were assessed by subjecti

Laparoscopic Treatment of Urinary Stress Incontinence.

From August 1995 to February 1996 I treated 20 women with urinary stress incontinence with a modified method replacing sutures with mesh and staples. Operating time ranged from 45 to 90 minutes, no intraoperative or postoperative complications occurred, and every patient was able to void spontaneously before discharge in 24 hours. This new approach has several advantages and can be applied by other endoscopists.

[Surgical indications in female urinary incontinence].

In addition to artificial urinary sphincters, the surgical management of female urinary incontinence includes many surgical procedures of the bladder neck: its retropubic placement within the confines of abdominal pressure, posterior support preventing effort opening or the reduction of its diameter. Whatever the surgical procedure, surgical indications depend on discomfort and patient desire after complete assessment to evaluate risk of failure and postoperative complications. Choice of a technique depends on the surgeon's individual preference; success rate varies from 50 to 80% after 5 year…

[Perineal retraining in urinary stress incontinence].

Twenty patients suffering from urinary stress incontinence were treated by perineal reeducation. The assessment included a medical and urological questionnaire, a physical examination, a urine analysis and culture, a cystoscopy, urinary flow and cystometry, a urethral pressure profile and a subjective evaluation of the perineal musculature. The 20 patients selected had documented stress incontinence, had never been operated on for incontinence and had a stable bladder at urodynamic assessment. Treatment was identical for all patients and included 12 biofeedback and electrostimulation sessions
