Assessment of vertigo.

Vertigo is defined as a subjective sense of imbalance and includes unsteadiness as well as a sensation of rotation. Vertiginous symptoms may arise from defects in the labyrinth, the visual system, or the central nervous system, although in most patients presenting with balance problems, the lesion lies within the labyrinth or its central connections. The most important step in the diagnosis is an unhurried and detailed history, bearing in mind that many patients will have difficulty in describing their symptoms. A detailed neurological examination is usually unnecessary but one must examine th

[Vestibular test in Menière's disease].

When it is well known that the electronystagmographic examination cannot diagnose the Meniere's disease, it is very useful to ensure that no other pathology is involved and for the follow up of the patients. The vestibulo-oculogram, on the other hand, suggests the presence of hydrops, according to the mechanical model.

[Contribution of the vestibular evoked myogenic potentials to the study of the vestibular neuritis].

INTRODUCTION: Until recently, the only tests available to provide information about vestibular function were caloric and kinetic tests, which only give us information about the external semicircular canal and the superior vestibular nerve. In recent years the development of vestibular evoked myogenic potentials has allowed us to assess the saccule and the inferior vestibular nerve. Our aim is, by studying the caloric test results as well as the vestibular evoked myogenic potentials in patients with Vestibular Neuritis, to determine whether they have involvement of the superior, inferior or bot

[Diagnosis and treatment of vestibular migraine - recent developments and future treatment strategies].

Vestibular migraine is a common cause of episodic vertigo and dizziness and affects up to 3% of all people. Characteristic symptoms include frequent episodes of vertigo (at least 5 attacks required) of varying duration (5 minutes to 72 hours) and at least moderate intensity, accompanied by other symptoms typical of migraine. To diagnose vestibular migraine, a (pre-existing) diagnosis of migraine with or without aura is required, as well as the simultaneous occurrence of migraine headaches, visual aura, or photo/phonophobia in at least every second vertigo attack. The spectrum of clinical prese

Assessment and management of vestibular migraine within ENT.

OBJECTIVE: Vestibular migraine is a newly recognised and debilitating condition. This article aims to provide an overview of what is known of vestibular migraine, delineating its diagnostic criteria and presenting some initial management strategies to aid ENT professionals in delivering optimal care when patients first present to the otolaryngology clinic. METHOD: Although traditionally underdiagnosed, there are now clearly defined diagnostic criteria to aid accurate diagnosis of vestibular migraine. RESULTS: A detailed history and clinical examination are the cornerstone of the diagnostic p

Can hyperventilation test and duration of spontaneous nystagmus help differentiate between vascular and inflammatory aetiology of acute unilateral vestibular deficit?

OBJECTIVE: To relate clinically the duration of spontaneous nystagmus and hyperventilation-induced nystagmus (HVIN) to vascular or inflammatory aetiology of acute unilateral vestibulopathy observed in a very early stage. METHODS: This is a retrospective study on 198 patients with acute unilateral vestibulopathy. RESULTS: In the short-lasting nystagmus group (spontaneous nystagmus < 48 h), mean age and cardiovascular risk were significantly higher; the rates of negative HVIN and paretic HVIN were 41.7% and 58.3%, respectively. In the long-lasting nystagmus group (spontaneous nystagmus > 48 h)

Vestibulo-ocular reflex and the head impulse test.

The authors highlights the importance of the vestibulo-ocular reflex examination through the head impulse test as a diagnostic method for vestibular dysfunction as well as, and primarily, a bedside semiotic resource capable of differentiating between acute peripheral vestibulopathy and a cerebellar or brainstem infarction in emergency rooms.

Hyperventilation-induced nystagmus and vertigo after stereotactic radiotherapy for vestibular schwannoma.

OBJECTIVE: To describe the phenomenon of hyperventilation-induced nystagmus (HVN) after stereotactic radiotherapy for vestibular schwannoma. PATIENTS: We present 2 patients with vestibular schwannoma and no pretreatment vestibular symptoms who both received stereotactic radiotherapy. Within 2 months of completing treatment, both patients developed intense vertigo while exerting themselves. Video-oculography demonstrated an intense nystagmus with the fast phase directed toward the side of the schwannoma in both patients. INTERVENTION: Diagnostic. RESULTS: Patients who have undergone surgical

Evaluation of balance function in peripheral vestibular disorders.

The functional influence upon overall balance function induced by a peripheral vestibular disturbance is evaluated by positioning, rotatory and posturographic testing. The information does not contribute to the diagnostic labelling of a patient (as e.g. Ménière's disease, etc), but it is of primary importance in the determination of a logical and adequate treatment and so for rehabilitation treatment. Some important items are reviewed.

On the search for markers of poor vestibular compensation.

The effectiveness of pursuit gain, cancellation of the vestibulo-ocular reflex, and a clinical oscillopsia test were assessed as vestibular function tests and tests that may allow prediction of which patients would compensate poorly after vestibular surgery. Cancellation of the vestibulo-ocular reflex in 17 patients and 17 control subjects was compared. Pursuit gain for 17 patients was determined for three frequencies at peak velocities of 25 and 50 degrees/sec. The oscillopsia test was administered to seven patients during at least the first 6 postoperative months. We are unable to state that…

Vestibular-evoked myogenic potentials and caloric stimulation in infants with congenital cytomegalovirus infection.

BACKGROUND: The influence of congenital cytomegalovirus infection on cochlear function has been well recognised; however, its impact on the vestibular system in infants has not been examined. The purpose of the present study was to evaluate vestibular function in a group of infants, using caloric stimulation tests and vestibular-evoked myogenic potential measurements. MATERIALS AND METHODS: Vestibular-evoked myogenic potentials and auditory brainstem responses were recorded and caloric stimulation was performed in 66 infants aged three months, comprising 40 healthy controls and 26 infants wit

Clinical consensus on vestibular infant screening.

PURPOSE: To address the current lack of standardized protocols for vestibular infant screening (VIS), this clinical consensus aims to establish a clinical consensus on VIS in infants aged 0-12 months. Given a frequent co-occurrence of vestibular dysfunction in children with sensorineural hearing loss (SNHL) and the significant impact of vestibular dysfunction on early development, early identification and management are critical. METHODS: This clinical consensus was developed by a panel of 28 international experts in pediatric otolaryngology and vestibular disorders through a structured, mult

[Clinical evaluation of otolithic function by the measurement of ocular cyclotorsion and skew deviation].

The ocular tilt reaction (OTR) consists in an association of an eye-head postural reaction, alteration of vertical perception, conjugated eye cyclotorsion, skew deviation and head tilt. We observed this condition following unilateral vestibular neurectomy and labyrinthectomy performed in patients suffering from severe Meniere's disease. All patients described postoperative vertical diplopia and vertical perception tilt. One illustrative case is reported in detail here. Changes in visual vertical were measured using a vertical frame with a pivoting pointer and the Maddox rod calibrated with a s
