Identification of intrinsic capacity impairment: diagnostic performance of integrated care for older people (ICOPE) screening tools in Zimbabwe.

INTRODUCTION: The World Health Organization (WHO) Integrated Care for Older People (ICOPE) framework proposes screening (Step-1) and subsequent in-depth assessment (Step-2) to inform personalised interventions to promote intrinsic capacity (IC) in older adults. We aimed to assess the diagnostic performance of screening approaches against in-depth assessments in Zimbabwe. METHODS: This cross-sectional study recruited older adults age ≥65 years in urban Zimbabwe. For both Steps 1 and 2, IC was assessed using WHO ICOPE-proposed and/or alternative assessments for seven domains: cognition, locomot

Vision symptoms after concussion: Review of diagnosis, pathophysiology, and treatment.

Vision symptoms are frequent after concussion and can often persist and be disabling. Most studies on concussion with persisting concussion symptoms (PCS) provide limited attention to vision symptoms. Vision symptoms occur in 69-82% of patients with PCS and commonly include photophobia, blurred vision, and computer screen intolerance (CSI) - a syndrome induced by viewing electronic screens including computer, cell phone, tablet, or TV screens. We discuss the vision symptoms that may arise and persist after a concussion. Unfortunately, the pathophysiology of many of these symptoms is unknown, a

The management of presbyopia with contact lenses: a review.

Although presbyopes represent a large and growing fraction of the population, only 2% of them have tried bifocal contact lens correction. Bifocal contact lenses have in the past been notorious for the chair time they required and for low success rates. But now the picture is beginning to change. The growing number of new and innovative lens designs is expanding the fitter's options and greatly increasing the chances of success. Success, however, depends on the fitter being able to select an appropriate lens for each patient. This paper reviews contact lens options available for presbyopic pati

Optical quality of the eye in subjects with normal and excellent visual acuity.

PURPOSE: To study the relationship between visual acuity (VA) and the eye's optical quality in subjects with normal and excellent spatial vision. VA ranged from decimal values of 1.0 (20/20) to 2.0 (20/10) when defocus and astigmatism were carefully corrected. METHODS: In 60 eyes of young subjects, visual and optical performance with the natural pupil were measured. A forced-choice procedure was used to measure tumbling-E high-contrast VA (HCVA) and low-contrast VA (LCVA). Wavefront aberration (WA) was measured using a Hartmann-Shack sensor. The associated point-spread function (PSF) and modu

Emerging trends in ocular telemedicine: the diabetic retinopathy model.

Diabetes mellitus is a leading cause of vision loss in industrialized countries. Diabetic retinopathy has features which make it ideal for disease management by telemedicine. The American Telemedicine Association (ATA) has recently established consensus recommendations for ocular telemedicine for diabetic retinopathy, in cooperation with the US National Institute of Standards and Technology. The guiding principle is that it would be inappropriate to use telemedicine to provide anything less than the accepted standard of clinical care. The ATA practice recommendations delineate performance stan

Acute visual change.

Changes in vision or loss of vision are common complaints for patients presenting to the emergency department. Such complaints may represent a simple problem related to recent trauma, early evidence of a systemic disease, or may be a vision-threatening lesion. A logical and organized approach to the history and the physical examination of the patient with eye complaints is key to the diagnosis and treatment of the more significant causes of these complaints. This article reviews an organized approach to the patient with complaint of visual loss and considers the wide differential diagnosis. It
