Vulvar mucinous adenocarcinoma with neuroendocrine differentiation: A case report and review of the literature.

BACKGROUND: There are limited cases in literature of patients with mucinous adenocarcinoma of the vulva with neuroendocrine differentiation have. With this new case, we aim to provide an overview of the existing literature and present a tool with relevant markers for the pathologist in the differential diagnosis. CASE DESCRIPTION: A 92-year-old multiparous, Caucasian woman presented with a 8 cm spherical tumor of the left major labium. Since the initial punch biopsy was not conclusive, a local resection was performed. Histopathological examination showed mucus production, large pools of mucin

Mucinous adenocarcinomas of the vulva.

An 80-year-old nullipara had a 2.0-cm cystic tumor of the right labium majus. Histologic diagnosis was mucinous eccrine carcinoma. Seventy-five percent of these rare skin adnexal tumors arise on the face, eyelid, or scalp; but none has been reported on the vulva. Indolent localized growth is usual with regional nodal spread in 11% and distant metastases in 3%. A 67-year-old multipara had a 1.2-cm polypoidal nodule of the posterior fourchette. Histologically, a colonic type mucinous carcinoma was arising within a villous adenoma. Mucicarmine and CEA stains were positive. Extensive workup failed

Melanoma of the vulva.

Twenty-three cases of malignant melanoma of the vulva recorded at Roswell Park Memorial Institute over a 36-year period have been reviewed. The findings confirm those of earlier series that these lesions tend to occur in postmenopausal Caucasians and most commonly originate on the labia minora and clitoris. Twenty of 23 patients were treated with therapy more conservative than radical vulvectomy and regional node dissection, with a resultant 25% 5-year survival and 10% 10-year survival among eligible patients. The fact that 12 of 14 recurrences surgical removal of these structures; No patient

Treatment of recurrent inguinal lymphocele by lymphatic leakage mapping and subsequent ligation of lymphatic vessel endings: a case report.

BACKGROUND: Different approaches have been attempted in both prophylaxis and treatment of recurrent inguinal lymphoceles; however, to date none have been consistently effective. We hereby report our preliminary experience with mapping of the lymphatic leakage followed by ligation of these mapped vessels for resolution of a recurrent inguinal lymphocele. CASE: A 73-year-old woman underwent an anterior modified radical vulvectomy with bilateral inguinofemoral lymph node dissection due to squamous cell carcinoma of the vulva. Postoperatively she presented with a recurrent inguinal lymphocele unr

The Roswell Park Cancer Institute experience with extramammary Paget's disease.

Extramammary Paget's disease (EMPD) is a rare intraepithelial neoplasm. Common sites of occurrence include the vulva, perianal region, perineum and scrotum. Despite frequent recurrences, surgery is the standard treatment. This study examines the recurrence rate for EMPD treated by conventional surgical management. Alternative and multimodal therapeutic approaches are reviewed. This retrospective analysis included all 30 patients treated for EMPD at Roswell Park Cancer Institute (RPCI) between 1970 and 1998. Following conventional surgical treatment, 44% of our patients developed recurrence. Vu

Endoscopic near infrared and indocyanine green to verify the viability of the subcutaneous flap for vulvar cancer.

INTRODUCTION: Vulvar cancer often requires radical vulvectomy with subsequent vulvar flap. Approximately in 20-60% of cases, there are post-operative complications ranging from infection to flap necrosis that often require reoperation. Several methods have been described to verify the vitality of the flap, but these are often expensive and require specific machinery that is not generally present in a gynecological clinic. In this case report, we present a viability verification of VY fasciocutaneous advancement flap for vulvar reconstruction by Endoscopic Near-Infrared and Indocyanine Green.

A vulvar mass as the first presentation in colorectal carcinoma: An unusual site of metastasis masquerading a primary cancer.

OBJECTIVE: To demonstrate a case with a vulvar metastasis masquerading a primary vulvar malignancy. The clinical and histological features, mechanism, and impact to the prognosis are discussed. CASE REPORT: A 58-year-old woman presented to gynecologist for abnormal vaginal discharge. A vulvar nodule was noticed during physical examination. Biopsy showed adenocarcinoma (ADC) and she was referred for further survey under the impression of Bartholin duct ADC. Later she was further found to also have a colorectal tumor with liver metastasis and subsequently received surgery under the suspicion of

Basal cell carcinoma of the vulva: a case report and systematic review of the literature.

The vulva is an unusual site for basal cell carcinoma (BCC). Vulvar BCC accounts for <1% of all BCCs and <5% of all vulvar malignancies. We report the case of an 83 year-old woman who presented with a 2-month history of a tender labial growth, with histopathology confirming nodular BCC. We conducted a systematic literature review of the characteristics of reported cases of vulvar BCCs. A comprehensive systematic review of articles indexed for MEDLINE and Embase yielded 96 reports describing 437 patients with 446 BCCs of the vulva. The mean age at presentation was 70 (range 20-100). Most women

Successful multidisciplinary treatment of the rare pathology of vulvar Crohn's disease.

A woman in her 50s with a medical history of rheumatoid arthritis (RA) and hypothyroidism presented with 3 months of vaginal pain and swelling. Physical examination showed vulvar oedema and erythema, ulcerations of the inferior vulva that extended to the buttocks and introitus with erythema and superficial erosions. MR pelvis showed a fibroid in the uterus but otherwise was unremarkable. Biopsies of the vagina showed extensive acute and chronic inflammation, necrosis and granulomas without concern for malignancy, which was consistent with a diagnosis of cutaneous Crohn's disease. MR enterograp

Cloacogenic Adenocarcinoma of the Vulva: A Case Report and Review of the Literature.

Primary adenocarcinoma of the vulva, unrelated to the native glands of perineum is an extremely rare neoplasm. Despite awareness of this lesion for over 40 years, the origin is not beyond speculation. The most reasonable hypothesis is based on the remnants of cloacal differentiation during early days of life. Here we report the case of a 60-year-old patient with a vulvar mass, who underwent partial vulvectomy and bilateral regional lymph node dissection. The tumor was composed of papillary and complex glandular structures and exhibited diffuse positivity for cytokeratin 20 and polyclonal CEA,

Vulvar carcinoma in pregnant women aged less than 40 years: case report.

BACKGROUND: Invasive squamous cell carcinoma of the vulva is primarily a disease of postmenopausal women and thus is rarely associated with pregnancy. CASE: We have reported on a young woman under 40 years old with vulvar carcinoma, which occurred during the pregnancy but optimal treatment was delayed to the postpartum period. This 37-year-old woman was diagnosed with 3x3 cm vulvar lesion, 2 weeks after cesarean section, subsequent biopsy revealed squamous cell carcinoma. She had a history of an ulcer on her left labia minor at the third month of the pregnancy. She was treated by a modified r

Primary extraskeletal myxoid chondrosarcoma of the vulva.

Primary extraskeletal myxoid chondrosarcoma (EMC) of the vulva is extremely rare. There is little available information about the biological behavior and treatment strategy for primary EMC of the vulva. We report a rare case of primary EMC of the vulva treated surgically. A 24-year-old Japanese woman had demonstrated a small and elastic mass of the vulva and underwent enucleation of the mass at a previous hospital, but a definitive histopathological diagnosis was not obtained. Therefore, the patient was referred to our hospital for further evaluation and treatment. We histopathologically diagn

Laparoscopically assisted vulvocolpohysterectomy and abdominoperineal resection with sigmoid vaginal replacement.

BACKGROUND: In this report, we discuss the feasibility of laparoscopy for the resection of recurrent invasive vulvar cancer involving the vagina and anus (stage IVA), requiring radical surgical treatment. METHODS: Successive steps of surgery are discussed: laparoscopic mobilization of the uterus, colon, and rectum for a posterior compartment exenteration (radical vulvectomy, colpohysterectomy, and abdominoperineal resection) and primary neovaginal reconstruction with sigmoid vaginal replacement. RESULTS: Duration of surgery was 240 minutes. There was no postoperative complication except for

Rapid groin recurrence of a vulvar carcinoma with invasion of the femoral vessels and the importance of the initial groin dissection in the staging surgery.

BACKGROUND: Carcinoma of the vulva accounts for 3% to 5% of all gynaecological malignancies. The disease spreads from the vulva through embolization to the locoregional lymphatic station, the inguinofemoral nodes. The single most important prognostic factor is the presence or absence of nodal disease. CASE REPORT: A 56-year old woman was admitted with a large, centrally necrotic left groin recurrence of a vulvar carcinoma. One year previously, she had undergone a radical vulvectomy without groin dissection for a pT2 vulvar carcinoma. In our institution, the tumour mass was excised. The femora

[Vulvar carcinoma. Diagnosis and therapy].

Vulvar cancer is a rare entity. It appears mostly in older women aged 70-79 years with a slow tendency to younger age. More than 90% of the tumors show a squamous differentiation. The correspondent preneoplasia is VIN 3. This lesion occurs in women mostly younger than 35 years. Experts assume vulvar cancer to appear in two different types:HPV-induced type in younger women and non-HPV-dependent type in older women. The preneoplasia VIN 3 already should be treated by resection or destruction. Invasive carcinomas stage I or II can be treated by wide local excision. The inguinofemoral lymph nodes

Lichen sclerosus.

Lichen sclerosus is a skin condition affecting all areas of the body at all ages and in both sexes. It appears to be a histologic and clinical entity, although in very many respects it is close to lichen planus and morphea. Its cause and pathogenesis are not established, but a link with autoimmune disease is recognized. Its predilection for the anogenital area in women has led to an unfortunate dichotomy between dermatologists and gynecologists. This was a potent source of confusion for many years but has now been resolved with a the recent formulation of a classification acceptable to the ISS
