Thrombolysis for Wake-Up Stroke Versus Non-Wake-Up Unwitnessed Stroke: EOS Individual Patient Data Meta-Analysis.

BACKGROUND: Stroke with unknown time of onset can be categorized into 2 groups; wake-up stroke (WUS) and unwitnessed stroke with an onset time unavailable for reasons other than wake-up (non-wake-up unwitnessed stroke, non-WUS). We aimed to assess potential differences in the efficacy and safety of intravenous thrombolysis (IVT) between these subgroups. METHODS: Patients with an unknown-onset stroke were evaluated using individual patient-level data of 2 randomized controlled trials (WAKE-UP [Efficacy and Safety of MRI-Based Thrombolysis in Wake-Up Stroke], THAWS [Thrombolysis for Acute Wake-

Role of the locus coeruleus in the emergence of power law wake bouts in a model of the brainstem sleep-wake system through early infancy.

Infant rats randomly cycle between the sleeping and waking states, which are tightly correlated with the activity of mutually inhibitory brainstem sleep and wake populations. Bouts of sleep and wakefulness are random; from P2-P10, sleep and wake bout lengths are exponentially distributed with increasing means, while during P10-P21, the sleep bout distribution remains exponential while the distribution of wake bouts gradually transforms to power law. The locus coeruleus (LC), via an undeciphered interaction with sleep and wake populations, has been shown experimentally to be responsible for the

Hypergraph-Based Audio-Visual Fusion for Obstructive Sleep Apnea Severity Estimation During Wakefulness.

Obstructive sleep apnea (OSA) is associated with psychophysiological impairments, and recent studies have shown the feasibility of using speech and craniofacial images during wakefulness for severity estimation. However, the inherent limitations of unimodal data constrain the performance of current methods. To address this, we proposed a novel hypergraph-based multimodal fusion framework (HMFusion) that integrates psychophysiological information from audio-visual data. Specifically, we employ long short-term memory (LSTM)-based encoders to extract modality-specific temporal dynamics from pre-t

When stress becomes wakefulness: Stress-arousal circuitry and the state control of insomnia.

Insomnia disorder (ID) is the most common sleep disorder and among the most prevalent neuropsychiatric conditions, affecting ∼9-20% of adults with substantial burden. Chronic ID is linked to cardiometabolic and psychiatric morbidity, cognitive impairment, and accelerated neurodegeneration. Although behavioral and pharmacologic therapies help, relapse is common, suggesting a pathophysiology not simply of "too little sleep" but a persistent bias toward wakefulness. Current models converge on ID as a disorder of hyperarousal, yet the circuit, cellular, and synaptic mechanisms by which stress prod

[Glutamatergic neurons in paraventricular nucleus of the thalamus promote wakefulness during propofol anesthesia].

OBJECTIVE: To determine whether the glutamatergic neurons in the paraventricular nucleus of the thalamus (PVT) is involved in the change of consciousness induced by propofol through a combination of behavioral and electroencephalography (EEG) recordings. METHODS: Healthy male VGluT2-IRES-Cre mice aged 8-12 weeks were used in this experiment. (1) The glutamatergic neurons in the PVT was selectively damaged, and its effect on propofol anesthesia induction and recovery times as well as the energy of EEG in different frequency bands were observed. (2) Optogenetics was utilized to selectively acti

Detecting passive and active response in patients with behaviourally diagnosed unresponsive wakefulness syndrome.

The diagnosis of unresponsive wakefulness syndrome depends mostly on the motor response following verbal commands. However, there is a potential for misdiagnosis in patients who understand verbal commands (passive response) but cannot perform voluntary movements (active response). To evaluate passive and active responses in such patients, this study used an approach combining functional magnetic resonance imaging and passive listening tasks to evaluate the level of speech comprehension, with portable brain-computer interface modalities that were applied to elicit an active response to attentio

Vasopressin neurons in the paraventricular hypothalamus promote wakefulness via lateral hypothalamic orexin neurons.

The sleep-wakefulness cycle is regulated by complicated neural networks that include many different populations of neurons throughout the brain. Arginine vasopressin neurons in the paraventricular nucleus of the hypothalamus (PVHAVP) regulate various physiological events and behaviors, such as body-fluid homeostasis, blood pressure, stress response, social interaction, and feeding. Changes in arousal level often accompany these PVHAVP-mediated adaptive responses. However, the contribution of PVHAVP neurons to sleep-wakefulness regulation has remained unknown. Here, we report the involvement of

Melatonin inhibits GABAergic neurons in the hypothalamus consistent with a reduction in wakefulness.

Although melatonin is necessary for circadian regulation of sleep, the mechanisms underlying this effect of melatonin are still unclear. In the present study, we showed that melatonin suppressed the activity of GABAergic neurons in the lateral hypothalamus, which has been reported to play a crucial role in maintaining wakefulness. The inhibitory effect of the melatonin was mediated by activation of melatonin 1 receptors and depended on the inhibition of hyperpolarization-activated cyclic nucleotide-gated (HCN) ion channels. At behavioral levels, infusion of melatonin into the lateral hypothala

Social Experience Is Sufficient to Modulate Sleep Need of Drosophila without Increasing Wakefulness.

Organisms quickly learn about their surroundings and display synaptic plasticity which is thought to be critical for their survival. For example, fruit flies Drosophila melanogaster exposed to highly enriched social environment are found to show increased synaptic connections and a corresponding increase in sleep. Here we asked if social environment comprising a pair of same-sex individuals could enhance sleep in the participating individuals. To study this, we maintained individuals of D. melanogaster in same-sex pairs for a period of 1 to 4 days, and after separation, monitored sleep of the

Increased Reward-Related Behaviors during Sleep and Wakefulness in Sleepwalking and Idiopathic Nightmares.

BACKGROUND: We previously suggested that abnormal sleep behaviors, i.e., as found in parasomnias, may often be the expression of increased activity of the reward system during sleep. Because nightmares and sleepwalking predominate during REM and NREM sleep respectively, we tested here whether exploratory excitability, a waking personality trait reflecting high activity within the mesolimbic dopaminergic (ML-DA) system, may be associated with specific changes in REM and NREM sleep patterns in these two sleep disorders. METHODS: Twenty-four unmedicated patients with parasomnia (12 with chronic

Reflection of tolerance to alcohol in the structure of the sleep wakefulness cycle.

The purpose of the present work was to study the effects of tolerance to ethanol on the sleep-wakefulness cycle (SWC) structure. Experiments were carried out in chronic conditions on the adult cats (n=5). The following methods were used: the stereotaxic - for implanting electrodes; polysomnographic - for EEG registration of the SWC structure. Alcoholisation (0.2-2.5 g/kg 25% ethanol solution) was conducted by intraperitoneal injections that lasted for two weeks. The obtained results were processed statistically and significance of the changes was determined by the Student t-test. Low single do

Orexin gene therapy restores the timing and maintenance of wakefulness in narcoleptic mice.

STUDY OBJECTIVES: Narcolepsy is caused by selective loss of the orexin/hypocretin-producing neurons of the hypothalamus. For patients with narcolepsy, chronic sleepiness is often the most disabling symptom, but current therapies rarely normalize alertness and do not address the underlying orexin deficiency. We hypothesized that the sleepiness of narcolepsy would substantially improve if orexin signaling were restored in specific brain regions at appropriate times of day. DESIGN: We used gene therapy to restore orexin signaling in a mouse model of narcolepsy. In these Atx mice, expression of a

Wakefulness in young and elderly subjects driving at night in a car simulator.

Young drivers are over-represented in nighttime traffic accidents and several studies have suggested that many accidents are associated with elevated sleepiness levels. It has been suggested that there may be a connection between lowered wake capacity and functional sensory motor skills on the one hand and sleep deprivation at the circadian low in young drivers on the other. Performance during a 45/min evening and night drive among young (n=10, age range 18-24 years) and elderly (n=10, age range 55-64 years) subjects was studied using a moving base driving simulator. EEG was measured continuou

[The role of opioid system in the regulation of the sleep-wakefulness cycle].

In chronic experiments influence of single administration of various doses of morphine (0.5, 2.0 and 3.0 mg/kg) on general behavior, structural organization of the sleep-waking cycle and some EEG and somato-vegetative parameters was studied in the cats. Intraperitoneal administration of morphine elicits numerous autonomic disorders, psychic derangement and complete deterioration of the sleep-waking cycle structure. The result certify that single administration of morphine induces activation of the opiate receptors in various structures of the brain, which entails deviations of behavioral and E

Regional brain glucose metabolism during morning and evening wakefulness in humans: preliminary findings.

STUDY OBJECTIVES: The mechanisms that maintain wakefulness across the day, in the face in increasing sleep drive, are largely unknown. The goal of this pilot study was to examine regional relative brain glucose metabolism during morning and evening wakefulness in healthy humans. DESIGN: [18F]-fluorodeoxyglucose positron emission tomography scans were conducted during quiet wakefulness in the morning and in the evening. Statistical parametric mapping was used to compare relative regional glucose metabolism during the 2 scans. Subjects also completed subjective ratings of alertness. SETTING: U

Effects of three hypnotics on the sleep-wakefulness cycle in sleep-disturbed rats.

RATIONALE: New sleep disturbance model in rats is useful for estimating the characteristics of some hypnotics. OBJECTIVES: The present study was undertaken to investigate the utility of a sleep disturbance model by placing rats on a grid suspended over water using three kinds of hypnotics, that is, short-acting benzodiazepine (triazolam), intermediate-acting benzodiazepine (flunitrazepam) and long-acting barbiturate (phenobarbital). METHODS: Electrodes for measurement of EEG and EMG were implanted into the frontal cortex and the dorsal neck muscle of rats. EEG and EMG were recorded with an e

Elicited pontogeniculooccipital waves and phasic suppression of diaphragm activity in sleep and wakefulness.

Fractionations are 20- to 100-ms pauses in diaphragm activity that occur spontaneously during rapid-eye-movement (REM) sleep, sometimes in association with pontogeniculooccipital (PGO) waves. Auditory stimuli can elicit fractionations or PGO waves during REM sleep, non-REM (NREM) sleep, and waking; however, their interrelationship has not been investigated. To determine whether the two phenomena are produced by a common phasic-event generator in REM sleep, we examined PGO waves and fractionations that were elicited by auditory stimuli (tones) presented to freely behaving cats across states.
