The aim of this study was to examine children with myelomeningocele (spina bifida) who were investigated according to the level of neurosegmental lesion and to determine the number who walked and their average age at commencement/cessation of walking. The data have been collected since 1978 at the Royal Children's Hospital, Melbourne during annual evaluations of patients with spina bifida and high-level paralysis. After screening for established criteria and assigning to a neurosegmental lesion level, 173 children were examined for their ability to walk, with or without aids, in a community se…

Eighty children with idiopathic toe walking (ITW) were examined 2-8 years (average, 34 months) after initial presentation to compare outcomes [passive ankle dorsiflexion (DF) and parental satisfaction] after three treatment modalities: observation, cast/brace treatment, or surgical triceps surae lengthening. Overall, 32% had a family history of ITW, 28% were born prematurely, and 16% had psychomotor delay. Forty-eight untreated patients showed little change in passive ankle DF at final follow-up, and only 25% of parents were satisfied with the child's gait. Compared with untreated children, ca…

1. The functions of ipsilateral cutaneous reflexes were studied with short trains of stimuli presented pseudorandomly to the sural nerve during human walking. Electromyograms (EMG) of lower (tibialis anterior (TA), soleus, lateral (LG) and medial (MG) gastrocnemius) and upper leg (vastus lateralis and biceps femoris) muscles were recorded, together with ankle, knee and hip joint angles. Net reflex EMG responses were quantified in each of the sixteen parts of the step cycle. The kinematic measurements included ankle eversion- inversion, and ankle, knee and hip flexion-extension. 2. The function…

In artificial laboratory situations where subjects undergo repetitive self-generated or externally imposed head rotations, visual-vestibular … lly enhanced VOR (VVOR) in lit conditions, during three natural activities: 1) standing quietly; 2) walking in place; 3) running in place. These data were compared with similar measurements during repetitive voluntary head oscillations at 0.8 Hz in pitch or yaw. During VVOR, subjects viewed a target placed 6 to 10 m away and remembered this target during …

Habitual toe-walking has been presented as a prolongation of a normal stage of development that requires conservative treatment to prevent or ameliorate associated gait abnormalities such as tripping and falling. An approach to the evaluation of a child with toe-walking should include (1) medical history (prenatal, intrapartum, and postnatal), (2) gait evaluation, (3) musculoskeletal examination, and (4) neurologic examination. Pathologic entities producing toe-walking have been explored in order to differentiate those conditions from idiopathic (habitual) toe-walking. The most common etiologi…

The device described indicates accurately the amount of weight borne on an affected leg. This device consists of two major elements: a weight measuring system incorporated in the walking cane's body, and a feedback mechanism situated in the cane's handle. By means of vibration or lights the patient and the therapist can determine whether or not the affected leg is bearing the prescribed weight. Thirty patients (10 with fractures, 10 with vascular disorders, and 10 below-knee amputees) and 10 healthy people participated in the study. The study concluded that young patients used the feedback inf…

A total of 127 children with profound mental retardation were reviewed to determine the age at which walking began. 53 per cent of the children walked at a median age of 30 months. Further sub-classification revealed that both the number of ambulatory children and their age at walking varied with degree of neurological handicap, children with additional neurological problems having a higher median age for beginning to walk. The minimal cognitive level required for walking remains unknown, but is probably less than generally accepted. In this profoundly retarded population, the existence of neu…

To examine further the relation between type of exercise, workload, ventilation, and exercise-induced asthma, we compared treadmill walking with treadmill running and treadmill running with isocapnic hyperventilation in separate studies in children and adolescents. Inspired air conditions were identical during each pair of tests. Walking and running with similar minute ventilation and oxygen consumption were followed by similar falls in peak expiratory flow rate as were running and hyperventilation with similar minute ventilation and end-tidal carbon dioxide tension. This study supports the co…

BACKGROUND: Mild cognitive impairment (MCI) can be an early sign of Alzheimer's disease and other types of dementia detectable through gait analysis. Curve walking, which demands greater cognitive and motor skills, may be more sensitive in MCI detection than straight walking. However, few studies have compared gait performance in older adults with and without MCI in these conditions.
OBJECTIVE: To compare the capability of curve and straight walking tests for the detection of MCI among older adults.
METHODS: We employed a Kinect v.2 camera to record the gait of 55 older adults (30 healthy co…

QUESTIONS: Does walking training combined with transcranial direct current stimulation (tDCS) improve walking (ie, speed, cadence and step length) and reduce falls and freezing, compared with no/sham intervention, in people with Parkinson's disease? Is walking training combined with tDCS superior to walking training alone? Are any benefits carried over to social participation and/or maintained beyond the intervention period?
DESIGN: A systematic review with meta-analyses of randomised clinical trials.
PARTICIPANTS: Ambulatory adults with a clinical diagnosis of Parkinson's disease.

OBJECTIVE: To determine the degree to which self-selected walking speed (SSWS), maximal walking speed (MWS), and walking speed reserve (WSR) are associated with fall status among community-dwelling older adults.
DESIGN: WS and 1-year falls history data were collected on 217 community-dwelling older adults (median age = 82, range 65-93 years) at a local outpatient PT clinic and local retirement communities and senior centers. WSR was calculated as a difference (WSRdiff = MWS - SSWS) and ratio (WSRratio = MWS/SSWS).
RESULTS: SSWS (P < 0.001), MWS (P < 0.001), and WSRdiff (P < 0.01) were associ…

[Purpose] The aim of this study was to determine the threshold for classifying walking independence in stroke patients with and without cognitive disorders. [Subjects] The subjects were 130 patients with initial stroke hemiplegia. [Methods] The following factors were analyzed for associations with walking independence: Brunnstrom stage, one-leg standing time on the paralytic side, one-leg standing time on the non-paralytic side, and 10-m walking speed. We classified the patients with Mini-Mental State Examination (MMSE) scores ≥24 points into the high-score group and those with MMSE scores of …

BACKGROUND: The Romberg test, with the subject standing and with eyes closed, gives diagnostic arguments for a proprioceptive disorder. Closing the eyes is also used in balance rehabilitation as a main way to stimulate neural plasticity with proprioceptive, vestibular and even cerebellar disorders. Nevertheless, standing and walking with eyes closed or with eyes open in the dark are certainly 2 different tasks. We aimed to compare walking with eyes open, closed and wearing black or white goggles in healthy subjects.
METHODS: A total of 50 healthy participants were randomly divided into 2 prot…

INTRODUCTION: Poor cardiopulmonary fitness is an important risk factor for postoperative complications, yet a feasible, objective and prognostically accurate method to assess preoperative fitness has not been established. The 6 min walk test (6MWT) is a simple, inexpensive and widely applicable measure that shows promise for predicting postoperative risk. However, robust data are lacking on whether the 6MWT accurately predicts complications, provides incremental prognostic value beyond routinely collected clinical factors or outperforms simpler alternatives such as questionnaires, cardiac biom

BACKGROUND: The energetic cost of walking increases with age and is linked to physical function impairment, but its relation to cognitive impairment is unknown.
METHODS: A total of 687 initially cognitively normal older adults (mean age 74.0 ± 7.2 years, 52% women) underwent repeated walking energy expenditure assessments (V̇O2) and adjudicated cognitive diagnoses over 7.6 ± 3.8 years. We examined (1) trajectories in the energetic cost of walking prior to any clinical diagnosis of cognitive impairment, comparing adults who later developed cognitive impairment versus those who did not, and (2)…

Humans typically choose to walk at a self-selected stride frequency that minimizes their metabolic cost. However, when environmental constraints are present (e.g., icy sidewalk), they will change their stride frequency to accommodate. This study provides a comprehensive understanding of the muscle-tendon dynamics when humans walk with different stride frequencies, offering valuable insights into the biomechanics of walking. The study aimed to quantify the effect of stride frequency on the muscle-tendon forces, powers, and induced accelerations on the center of mass. Data was collected with eig…

Obstructive Sleep Apnea (OSA) is the most common sleep- related breathing disorder. In recent years, evidence have shown that patients with OSA may have this disorder for different reasons, with different symptoms and comorbidities. Therefore, treatment should be individualized [1]. This has led to a growing interest in the characterization of the disease into phenotypes. OSA in elderly patients is often a challenge for clinician in terms of diagnosis, as symptoms may be masked, but also for treatment in terms of efficacy and adherence. However, aging is a pathophysiological factor that predis
