Comparison of the demographic and wound characteristics of non-suicidal and suicidal self-wrist cutting injuries.

Patients who commit self-wrist cutting injuries (SWCIs) are a heterogeneous group composed of patients with non-suicidal self-injury (NSSI) and suicide attempt (SA). The purpose of this study was to compare the demographic features and wound characteristics of patients with NSSI and SA.A retrospective review of 300 patients who visited the emergency department (ED) for treatment of SWCIs between January 2011 and December 2015 was performed. Data collected from the electronic medical records included age, sex, the reason for SWCIs, presence of suicidal ideation, concomitant intoxication with al

Management and functional outcomes of combined injuries of flexor tendons, nerves, and vessels at the wrist.

A retrospective review of 42 patients with spaghetti wrist lacerations operated on by the author between June 1997 and May 2005 was completed. A total of 31 males and 11 females, average age of 17.1 years (range, 2-40 years), sustained spaghetti wrist injuries. The most frequent mechanisms of injury were accidental glass lacerations (55%), knife wounds (24%), and electrical saw injuries (11%). An average of 9.16 structures was injured, including 6.95 tendons, 1.4 nerves, and 0.8 arteries. The most frequently injured structures were median nerve (83%), flexor digitorum superficialis 2-4 tendons…

Transposed replantation of fingers at forearm bones in severe segmental injuries across the hand and wrist.

There are situations in which amputated hands or fingers cannot be replanted directly back to their original positions. When there is severe segmental injury across the hand and wrist but one or several fingers are still healthy, the fingers can be selected to be replanted at the forearm bones to restore pinch function. This is different from the toe to antebrachial stump transplantation presented by Dr. Vilkki. From the results in four patients, the following conclusions are drawn: (1) When the fingers are replanted at the forearm bone(s), only pinch function can be obtained. There is no oppo

Diagnostic accuracy of standard practice wrist MRI for scapholunate ligament injuries: a retrospective arthroscopic correlation study.

INTRODUCTION: The scapholunate ligament is the most injured intercarpal ligament. Untreated tears can lead to carpal instability and subsequent radiocarpal and midcarpal osteoarthritis. While wrist arthroscopy remains the gold standard, the diagnostic performance of standard practice preoperative MRI in clinical practice warrants further investigation. METHODS: We retrospectively analysed patients who underwent preoperative wrist MRI followed by wrist arthroscopy between 2013 and 2021. MRI findings regarding scapholunate ligament integrity were compared with arthroscopic findings serving as r

Wrist and hand degloved injuries salvaged with reverse radial forearm flap in the CoVid-19 era.

Reverse radial forearm flap is a quick and reliable flap, providing a well-vascularized tissue for coverage of hand defects. It is based on the reverse flow of the radial artery and the only drawback is the proximal ligation of the vessel. Two cases with complex dorsal hand defects were referred with delay to our Hand Unit, due to the CoVid-19 pandemic restrictions. In both cases deep structures of the wrist and fingers were exposed and the wounds were infected. Surgical debridement, skin coverage and reconstruction were decided in a staged approach: The reverse radial forearm flap was first p

Forensic evaluation of alleged wrist restraint/handcuff injuries in survivors of torture utilizing the Istanbul Protocol.

Despite being a common form of abuse, there is a paucity of literature describing shackling and wrist restraint injuries among survivors of torture. Forensic evaluation of alleged wrist restraint/handcuff injuries in survivors of torture presents challenges to the evaluator, especially if the injuries are remote and do not leave lasting marks nor neurologic deficits. Thorough history-taking and physical examination are critical to effective forensic documentation. Guidance is provided in The Manual on Effective Investigation and Documentation of Torture and Other Cruel, Inhuman or Degrading Tr

The value of preliminary clinical evaluation for decision making in injuries of the hand and wrist.

INTRODUCTION: Recent research suggests that up to 20% of minor trauma patients admitted to the emergency department (ED) will suffer from non-specific chronic conditions over the subsequent several months. Thus, the present study assessed the correlates of symptoms that persisted at 4 months after an ED visit and, in particular, evaluated the associations between these symptoms and self-reported stress levels at ED admission and discharge. METHOD: This study was a prospective observational investigation conducted in the ED of Bordeaux University Hospital that included patients admitted for mi

Overuse wrist injuries in young athletes: What do sports physicians consider important signals and functional limitations?

This study's objective was to collect items from experienced sports physicians, relating to the presence and severity of overuse wrist injuries in young athletes, for developing a measurement instrument for signals of overuse wrist injury. Seven Dutch elite sports physicians involved in guidance and treatment of young athletes in wrist-loading sports (gymnastics, tennis, judo, field hockey, volleyball and rowing) participated in a focus group. They discussed signals and limitations related to overuse wrist injuries in young athletes. Data were coded and categorised into signals and limitations…

Diagnosis, Treatment, and Return to Play for Four Common Sports Injuries of the Hand and Wrist.

Hand and wrist injuries in the high-level athlete are challenging because they may be underestimated by the patient, family, and team, and return to play may be longer than desired. The needs of the player and the team must be balanced with the long-term functional ramifications of the injury. Four common soft-tissue sports injuries are flexor digitorum profundus avulsion, flexor pulley rupture, extensor carpi ulnaris dislocation, and thumb metacarpophalangeal joint ulnar collateral ligament injury. For each of these injuries, the assessment, treatment, and considerations for return to play sh…

Real-time digital contrast enhancement and magnification in the assessment of scaphoid and other wrist injuries.

A study on the value of a commercially available desk-top digital magnifier-contrast enhancer (DETECT System) was made in a series of 550 patients presenting with an acute wrist injury. Four radiologists, of varying experience, independently reviewed the radiographs on a conventional lightbox and later with the digitizer. In the scaphoid series (350 cases), the performance of the two more experienced radiologists was marginally better with the digitizer, whereas the less experienced radiologists performed slightly worse. Overall the digitizer improved the confidence of the radiologists in diag

Review article: Best practice management of closed hand and wrist injuries in the emergency department (part 5 of the musculoskeletal injuries rapid review series).

Acute hand and wrist injuries are a common presentation to the ED and are associated with large individual and societal costs. Appropriate management of these injuries in the ED is crucial given that optimal hand function is essential for daily activities and quality of life. This rapid review investigated best practice for the assessment and management of common closed hand and wrist injuries in the ED. Databases were searched in 2017, including PubMed, CINAHL, EMBASE, TRIP and the grey literature, including relevant organisational websites. Primary studies, systematic reviews and guidelines …

MRI Diagnosis of Gymnast's Wrist (Distal Radial Physeal Stress Injury).

Gymnast's wrist is an uncommon stress‑related injury of the distal radial physis caused by repetitive axial loading in skeletally immature athletes. We report the case of a 16‑year‑old elite gymnast presenting with progressive wrist pain without acute trauma. Magnetic resonance imaging demonstrated focal widening of the volar distal radial physis with adjacent metaphyseal and epiphyseal bone marrow edema, consistent with a stress‑related physeal injury. Conservative management with activity modification and physical therapy resulted in symptom resolution. Teaching point: In young athletes with

Staged Reconstruction for Dorsal Hand and Wrist Shear Injury: A Case Report.

INTRODUCTION: Dorsal hand shear injuries present complex challenges for surgeons in addressing soft tissue coverage, extensor tendon injuries, and bone loss. This case study examines the inherent challenges associated with the treatment of a severe injury to the dorsum of the hand, as well as the selected treatment approach. CASE REPORT: A 41-year-old male experienced a severe injury to the dorsum of his left hand following a road traffic accident. The injury involved skin loss from the wrist to the metacarpophalangeal joints, complete loss of the extensor digitorum communis tendons and exten

A rare case of bilateral complex wrist injury in a professional motocross rider.

We describe the treatment of a 29 year-old professional motocross rider, who sustained a perilunate dislocation in his right wrist and a distal radius fracture and dorsal dislocation of his left wrist during a race. Both wrists were treated acutely during a single operating session. Surgery consisted in open reduction, k-wire fixation and mini-anchor repair of the scapho-lunate and luno-triquetrium ligaments on the right wrist, while closed reduction and percutaneous k-wire fixation was used in the left wrist. Follow-up at 6 months has shown satisfying radiological and functional outcomes in b

A rare wrist injury: simultaneous fractures of the hamate body and scaphoid waist.

Hamate fractures, which are classified as fractures of the body or the hook, are rare, representing only 2% to 4% of carpal fractures. Hamate body fractures can occur as isolated injuries, but in a high-energy trauma, associated carpal bone fractures or carpal disruption can be seen. Hamate fractures can be easily underdiagnosed. Pain in the ulnar side of the palm and swelling of the wrist after a wrist injury should alert the physician. Anteroposterior and lateral radiographs of the wrist may not always be adequate for the diagnosis. Computed tomography should be used to explain the pain in t
