OBJECTIVE: To test the efficacy of diethylcarbamazine DEC single dose regimen of 6 mg/kg body weight (bw) on a sample of Wuchereria bancrofti microfilaria (mf) carriers in Matara.
DESIGN: 6 mg/kg bw DEC dose in 50 mg tablets given under direct observation to the subjects at 23.00 hours after pre-treatment blood collection for mf counts. Post-treatment mf counts were at 1, 2 weeks and 1, 3, 6, 12 months.

Three types of in vitro released excretory, secretory and metabolic antigens of Wuchereria bancrofti third larval stage (L3ESM) are evaluated in ELISA test to detect infected individuals in the endemic area. A total of 104 reference sera are used to predict the sensitivity of these antigens. None of L3 ESM antigens, although homologous in nature, did not identify correctly the categorised reference sera. This study clearly indicated a need for defined antigens to detect W. bancrofti infection early in the endemic residents.

In the fall of 1988, 14 Tahitian Wuchereria bancrofti carriers were treated by a single diethylcarbamazine (DEC) 3 mg/kg dose. Determination of blood microfilarial (mf) density was carried out on days 0, 7, 14, 30, 90 and 180 using the membrane filtration technique. The spatial variation of Wuchereria bancrofti and Plasmodium falciparum infection densities was measured in a rural area of Papua New Guinea where they share anopheline vectors. The spatial correlation of W. bancrofti was found to reduce by half over an estimated distance of 1.7 km, much smaller than the 50 km grid used by the World Health Organization rapid mapping method. For P. falciparum, negligible spatial correlation was found. After mass treatment with anti-filarial drugs, there was negligible correlation between the changes in the densities of the two parasites.

BACKGROUND: Lymphatic filariasis (LF) is a neglected tropical disease in some parts of India and results in chronic lymphedema, hydrocele and acute inflammatory episodes. Even outreach to communities often yields undiagnosed cases of chronic cases needing confirmatory testing and tied-in care. This case illustrates the confluence between chronic infectious and metabolic diseases in rural India, and the requirement of comprehensive primary care.

We studied the epidemiology and clinical presentation of lymphedema in three previously LF-endemic health districts of Mali after at least five consecutive rounds of mass drug administration (MDA) with albendazole and ivermectin.

In order to evaluate transmission risk an assay is needed that can specifically detect infective L3 stage parasites.
