MDMA (Ecstasy) precipitation of panic disorder.

The authors describe three patients whose panic disorder began during recreational use of MDMA (Ecstasy) and was subsequently complicated by agoraphobic avoidance that continued autonomously after cessation of the drug. Their panic disorder responded well to serotoninergic antidepressant drugs. Theoretical and practical implications are discussed.

Diurnal rhythms and symptom severity in panic disorder. A preliminary study of 24-hour changes in panic attacks, generalised anxiety, and avoidance behaviour.

Diurnal changes in the frequency of panic attacks and symptoms of generalised anxiety, phobic anxiety and phobic avoidance in 34 panic-disorder patients and 40 normal controls were evaluated. The panic-disorder patients had significant diurnal changes in generalised and phobic anxiety, but not phobic avoidance. Increased severity of symptoms and prominent diurnal changes were most evident in the panic-disorder patients with a history of depression. Although panic attacks were distributed throughout the 24-hour period, patients with a current episode or history of depression tended to have more

Group anxiety management: effectiveness, perceived helpfulness and follow-up.

An evaluation was conducted on out-patient cognitive-behavioural anxiety management groups. Twenty-nine clients assessed before and after the group and at three-month follow-up showed significant improvement on self-report measures. A further follow-up on 21 clients, conducted by an independent assessor at an average of 11 months, showed greater improvement with time. Clients also rated how helpful they had found non-specific therapeutic factors, and specific anxiety management techniques. 'Universality' was the most helpful non-specific factor, and 'the explanation of anxiety' was the most he

Platelet serotonin uptake in panic disorder: comparison with normal controls and the effect of treatment.

Platelet serotonin uptake was measured in 59 patients with panic attacks and compared to 26 controls. Scatchard analysis of the data showed that the maximal rate of uptake (Vmax) of serotonin was significantly higher in patients than controls. The affinity constant (Km) was not different. In a sub-group of 24 patients, benzodiazepine treatment did not alter the kinetics of platelet serotonin uptake. These findings suggest an abnormality in serotonergic function in patients with panic attacks. Insofar as the platelet is a model of central neuronal function they support serotonergic hyperactivit

Patients who complain of social dysfunction as their main problem: II. Comparison with other outpatients seen for behavior therapy.

Forty socially dysfunctional patients (avoidant personality disorder, DSM-III) and 76 other patients also seen for behavioral psychotherapy rated 30 items from the Social Situations Questionnaire on dimensions of distress and frequency of involvement. A discriminant function analysis showed that socially dysfunctional patients formed a distinct group as compared with other patients on ratings of distress. The main characteristics were over initiating social contact and carrying on a conversation with people one does not know. The separation between groups on the rating of frequency of involvem

Cognitive versus behavioral procedures in cognitive-behavior therapy: a critical review of the evidence.

During the past decade there has been an increasing emphasis on cognition in psychology and behavior therapy. This movement has spawned several distinctive cognitive therapies. While those therapies do employ innovative cognitive treatment procedures, they often do so in the context of well-established behavioral treatment procedures which may or may not be acknowledged by the label "cognitive-behavior therapy". This paper addresses the question of whether cognitive therapy is an evolutionary or revolutionary development from behavior therapy and critically evaluates the evidence for the effic

Behavior therapy as a test of psychoanalytic theory.

The author describes 4 patients in whom radically new symptoms appeared after intensive behavior therapy. He suggests that fresh symptoms are most likely to develop in a small minority of patients who are unable to learn more adaptive psychological defense mechanisms during or after behavior therapy and that psychoanalytic theory may help predict the nature of such symptoms.

Lymphocytic Microscopic Colitis Associated with Paroxetine: A Psychiatric Nursing Case Report.

OBJECTIVE: To describe a case of biopsy-confirmed lymphocytic microscopic colitis (MC) temporally associated with paroxetine (Paxil) therapy and to highlight psychiatric nursing implications for monitoring serotonergic adverse effects across the brain-gut interface. METHODS: This case report was developed using clinical history, diagnostic findings, treatment course, and relevant literature on selective serotonin reuptake inhibitors, MC, and serotonergic signaling in gastrointestinal function. RESULTS: A 27-year-old male with generalized anxiety disorder, panic disorder, agoraphobia, and irr…

Positive and negative affect change following psychotherapeutic treatment for anxiety-related disorders: A systematic review and meta-analysis.

BACKGROUND: Anxiety-related disorders feature elevated negative affect (NA), and in some cases, diminished positive affect (PA). It remains unclear how well extant psychotherapies for anxiety-related disorders improve PA versus NA. METHODS: We systematically searched the Cochrane Central Register of Controlled Trials, PubMed, PsychInfo, and Web of Science databases. Records included studies involving (1) patients with a principal or co-principal diagnosis of at least one anxiety-related disorder (i.e., generalized anxiety, social anxiety, panic, agoraphobia, health anxiety, specific phobia, o…

Prevalence of current anxiety disorders in people with bipolar disorder during euthymia: a meta-analysis.

BACKGROUND: Anxiety disorders are highly prevalent in people with bipolar disorder, but it is not clear how many have anxiety disorders even at times when they are free of major mood episodes. We aimed to establish what proportion of euthymic individuals with bipolar disorder meet diagnostic criteria for anxiety disorders. METHOD: We performed a random-effects meta-analysis of prevalence rates of current DSM-III- and DSM-IV-defined anxiety disorders (panic disorder, agoraphobia, social anxiety disorder, generalized anxiety disorder, specific phobia, obsessive-compulsive disorder, post-traumat…

Treatment hurts: Lay theories of graded exposure in the treatment of four anxiety disorders.

OBJECTIVE: This paper concerned the perceived suffering/side effects caused by various well-known treatments for personal problems. It looked at whether people understood whether potentially painful treatments that confront negative aversive affect were effective or not. METHOD: In total, 106 participants completed a long questionnaire assessing the 'psychological pain' ratings of 30 psychotherapy treatments, varying in fear exposure, for four relatively common anxiety disorders: social phobia, agoraphobia, post-traumatic stress disorder, and obsessive compulsive disorder. RESULTS: Factor an…
