The smallest de novo deletion of 20q11.21-q11.23 in a girl with feeding problems, retinal dysplasia, and skeletal abnormalities.

We report on a de novo interstitial deletion of 20q11.21-q11.23 in a 2-year-old girl with a set of dysmorphic features, cleft palate, heart defect, severe feeding problems, failure to thrive, developmental delay, preaxial polydactyly (right thumb), and retinal dysplasia. Interstitial microdeletions of the long arm of chromosome 20 are rare. Exclusively rare are proximal microdeletions involving 20q11-q12 region. Our patient is the fourth described so far and has the smallest deleted region 20q11.21-q11.23 of 5.7 Mb. The defined clinical phenotype of our patient is very similar to previously pu

A Chinese patient with KBG syndrome and a 9q31.2-33.1 microdeletion.

KBG syndrome is characterized by postnatal short stature, macrodontia, facial and hand anomalies, delayed bone age and intellectual disability. KBG syndrome is an infrequently reported autosomal dominant condition caused by a mutation or haploinsufficiency of ANKRD11 at 16q24.3. We report on a patient, who showed many manifestations of KBG syndrome and was found to harbor a de novo ANKRD11 mutation, c.362T > A (p.Met121Lys). As the patient showed additional characteristics not occurring in KBG syndrome, a CGH array was performed which showed a de novo microdeletion of 9q31.2-q33.1. The majorit

Dyssegmental dysplasia with bone entrapment and visceral ramifications.

A stillborn neonate had the typical clinical and radiologic manifestations of dyssegmental dysplasia, an autosomal recessive genetic disorder. The diagnosis was supported by histologic demonstration of the characteristic ballooned chondrocytes in the growth plate and spaces filled with mucoid material in the resting cartilage. Autopsy revealed entrapment of the femoral heads by bone overgrowth in the acetabulum. This notable feature, which has apparently not previously been documented, explained the articular rigidity, which is a significant clinical manifestation. Horizontal grooves on the an

[Sacrum histoplasmosis 10 years after NHL of the sacrum: a case report].

The location of bone histoplasmosis is rare. It is a parasitic infection caused by Histoplasma capsulatum, affecting especially the immunocompromised. Diagnosis is difficult, requiring the use of mycological and histopathological studies. Through this observation, we report a rare sequence of the development of sacrum histoplasmosis in a patient treated 10 years ago for a malignant non-Hodgkin lymphoma of the sacrum with complete remission. The clinical and radiological investigations suggested a lymphomatous relapse but the pathological study showed histoplasmosis. Antifungal treatment was st

[Hip joint development after treatment the developmental dislocation with overhead traction].

Despite the fact that ultrasound of children's hips is widely used for screening, late diagnosed cases of developmental dislocation of the hip are still a common problem in the orthopaedic practice. The aim of the study is to review final clinical and radiological outcomes of treatment of DDH with overhead traction and closed reduction after skeletal growth. Clinical records and radiograms of 107 hips (81 children) were retrospectively reviewed. All of them were treated according to the same program: overhead traction (about 2 weeks), followed by closed reduction, modified Lorenz cast (2 month

Novel dental anomalies associated with congenital contractural arachnodactyly: a case report.

Congenital contractural arachnodactyly (CCA) is an inherited disorder of connective tissue similar to Marfan's syndrome. The craniofacial and oral features of a young girl with CCA are described. The patient has the typical features of CCA as well as some additional dental anomalies which have not previously been reported with this syndrome. These include banded pitted enamel hypoplasia and hypomineralization, long, spindly tapered roots, and pulp canal obliteration with multiple pulp stones. Dentists must be aware of the clinical features of a patient's syndrome to determine whether there are

Antley-Bixler syndrome, description of two new cases and review of the literature.

OBJECTIVE: Antley-Bixler syndrome (ABS) is a rare disorder characterized by multiple malformations of cartilage and bone including multisynostotic osteodysgenesis, midface hypoplasia, choanal atresia or stenosis, femoral bowing, neonatal fractures and multiple joint contractures and, occasionally, urogenital, gastrointestinal or cardiac defects. Since the first report in 1975, at least 34 cases (including this report) have been described. We present 2 cases of congenital hydrocephalus, suggesting the cause of craniosynostosis and midface hypoplasia is associated with ABS. CLINICAL PRESENTATIO

Acromicric dysplasia: long term outcome and evidence of autosomal dominant inheritance.

Acromicric dysplasia is a rare bone dysplasia characterised by short stature, short hands and feet, normal intelligence, mild facial dysmorphism, and characteristic x ray abnormalities of the hands. Only a very small number of children with this condition have been reported so far. Here we report on a series of 22 patients including 10 boys and 12 girls with acromicric dysplasia. Length was normal at birth and height fell progressively off the centiles postnatally. The mean adult height was 130 cm (133 cm in males, 129 cm in females). The hands, feet, and limbs were short and OFC was normal. I

Evaluation of five different incisions for correction of radial dysplasia.

The complications of the five different incisions used for centralization from 1970 to 1996 were evaluated. In five cases access was by an S- shaped incision, in one with a transposition flap. Sixteen centralizations were done using a radial Z-plasty in combination with an ulnar excision and four by using two opposite Z- plasties, preserving wrist mobility as much as possible. In 1995 and 1996 a bilobed flap was used in seven cases. No complications occurred with the primary procedures prior to 1995. In all seven cases where the bilobed flap was used venous congestion was seen. In four out of

Second trimester oligohydramnios, a predictor of poor fetal outcome.

Twelve cases of severe second trimester oligohydramnios are reported. The outcome of these pregnancies was uniformly poor, with no survivors in the present series. Four patients had therapeutic abortions, one woman had spontaneous labor at 22 weeks' gestation, and seven patients continued to viability. Of these, five patients had severe renal abnormalities incompatible with life. Two infants died at, or shortly after, birth from severe intrauterine growth retardation (IUGR), one of which had a triploid karyotype. Review of the literature shows a similar poor outcome for pregnancies with severe

Patterns of osteoarthritis of the hip.

The division of osteoarthritis into primary and secondary varieties implies that these are aetiologically distinct entities, the former being due to some intrinsic defect of cartilage and the latter resulting from previous articular damage. This traditional concept is questioned and the hypothesis is advanced that osteoarthritis is always secondary to some underlying abnormality of the joint. A detailed clinical, radiographic and morbid anatomical study of 327 cases of osteoarthritis of the hip is presented. In all but twenty-seven some predisposing abnormality of the joint was diagnosed: 107

Unexpected high frequency of skeletal dysplasia in idiopathic short stature and small for gestational age patients.

OBJECTIVE: To assess the prevalence of skeletal dysplasias (SDs) in patients with idiopathic short stature (ISS) or small for gestational age (SGA) status. SETTING: Rare Endocrine/Growth Diseases Center in Paris, France. DESIGN: A prospective study on consecutive patients with ISS and SGA enrolled from 2004 to 2009. METHOD: We used a standardized workup to classify patients into well-established diagnostic categories. Of 713 patients with ISS (n=417) or SGA status (n=296), 50.9% underwent a skeletal survey. We chose patients labeled normal or with a prepubertal slowdown of growth as a compa

Characterization of familial short stature with or without skeletal dysplasia in a Pakistani cohort reveals founder mutations as well as novel variants.

Skeletal dysplasias are a diverse group of genetically inherited bone disorders which may manifest with or without short stature. Extreme short stature can also result from different growth hormone deficiencies. Our research aimed to identify the genetic causes of recessively inherited short stature, with or without skeletal abnormalities in Pakistani families. We recruited participants from thirty-eight pedigrees with a history of these disorders. Radiological examinations were performed for a few patients. Some samples were first screened for variants in GHRHR and RMRP by Sanger sequencing.

FBN1 TB5 domain variants in acromelic dysplasia: multisystem manifestations, genotype-phenotype correlations, and partial responses to growth hormone therapy.

BACKGROUND: Acromelic dysplasias, including acromicric dysplasia (AD) and geleophysic dysplasia type 2 (GD2), are ultrarare disorders caused by FBN1 variants in the transforming growth factor-β-binding protein-like domain 5 (TB5). These conditions are characterized by severe short stature and variable multisystem involvement, but genotype-phenotype correlations and treatment responses remain incompletely defined. OBJECTIVE: To characterize the clinical spectrum, genotype-phenotype correlations, and growth outcomes in a Chinese cohort of patients with FBN1-related AD/GD2. METHODS: We conducte

Management of Sleep-Disordered Breathing in Children With Craniofacial Abnormalities and Skeletal Dysplasia.

INTRODUCTION: Anatomic malformations associated with craniofacial abnormalities (CFAs) and skeletal dysplasias (SDs) contribute to airway obstruction during sleep in children, which may be treated surgically, with positive airway pressure (PAP) therapy, or with dental appliances. This study examines the management of OSA in children with CFAs and/or SDs, including the prevalence of residual OSA postsurgery and PAP therapy compliance. METHODS: This was a retrospective cohort study of children with CFAs and/or SD. Children were included if they underwent polysomnography (PSG) at the Queensland

3D Knee Trochlear Morphology Assessment by Magnetic Resonance Imaging in Patients With Normal and Dysplastic Trochleae.

BACKGROUND: Trochlear dysplasia is recognized as a significant risk factor for patellar instability, but current imaging modalities fail to allow full visualization and classification of the complex 3-dimensional (3D) anatomy of the trochlea. The purpose of this study was to elucidate primary differences in trochlear morphology between patients with and without patellar instability by utilizing 3D magnetic resonance imaging (MRI) reconstructions of the trochlea. METHODS: An institutional review board-approved retrospective review included 24 patients with a diagnosis of patellar instability a
