A Practical Guide to Becoming a Clinical Problem Solving Discussant.

Clinical problem solving (CPS) conferences provide a real-time view into diagnostic reasoning, yet faculty who serve as discussants often do so without formal guidance. This article offers a practical framework for clinicians preparing to take on the CPS discussant role, emphasizing that success depends less on reaching the correct diagnosis and more on structured reasoning. Preparatory steps include strengthening clinical analytical skills, observing experienced discussants, and practicing in low-stakes settings that foster comfort with speaking and thinking aloud. During the CPS session, dis

Standardizing EEG preprocessing for cross-site integration - the CLEAN pipeline.

Electroencephalography (EEG) is a powerful tool for investigating neural processes underlying cognition and neuropsychiatric disorders. Yet, variability in EEG preprocessing strategies restricts reproducibility and data integration across study sites and laboratories, particularly in larger research consortia. This paper introduces the CLEAN-EEG preprocessing pipeline, designed to standardize data processing and documentation across multiple sites. The CLEAN pipeline is implemented in MATLAB using EEGLAB. It comprises three modular, script-based stages: main preprocessing (including down-sampl

Naturalistic actions modulate Working Memory prioritization in immersive virtual reality.

Human cognition is inherently action-oriented, enabling flexible interaction with dynamic environments. Working Memory (WM) links perception and action by maintaining task-relevant representations. Although evidence indicates that action planning can influence what information is maintained in WM, traditional laboratory paradigms often rely on simplified response demands that only partially capture real-world perception-action coupling. Here, we investigated whether the nature of actions modulate the prioritization of sensory representations in WM. Using a virtual reality task, participants re

Virtual reality transition to underground: human responses across sequential spatial transitions.

Given the growing demand for the active utilization of underground spaces, understanding their effects on humans is crucial. However, constructing physical underground spaces for research is often costly and inflexible. In response, virtual reality (VR) provides a flexible and cost-effective alternative for simulating such environments and evaluating human responses. Yet, accurately replicating the experience of being underground in VR remains challenging. This study proposes a novel framework-Virtual Reality Transition to Underground (VRTU)-designed to stimulate the experience of descending f

Adaptive cascading artificial intelligence for Alzheimer's disease assessment: a clinically oriented narrative review and implementation framework.

Artificial intelligence (AI) has achieved remarkable success in the diagnosis of Alzheimer's disease (AD) in the literature, where many of the models use multi-modal methods including neuroimaging, cerebrospinal fluid, genetics, and cognitive assessment. But clinical adoption of these systems is still limited since most systems are developed in an idealized setting, as cost-effective and specialized diagnostic studies are not universally accessible. We discuss the translation of benchmark performance of AI to real-world dementia care pathways. A practical framework that would be useful for sca

Working memory load-dependent modulation of neural activity predicts response to cognitive behavioral therapy in obsessive-compulsive disorder.

Cognitive behavioral therapy (CBT) is an effective treatment for obsessive-compulsive disorder (OCD). However, CBT does not lead to a satisfying symptom reduction in a considerable number of patients with OCD. The identification of variables that predict insufficient treatment response could improve efficient treatment selection and inform the development of specific augmentative treatments. In the current study, we tested whether prediction of treatment response can be improved by including neurobiological markers during working memory (WM) performance. Forty-four patients with a primary OCD

Seasonal biological social and cognitive mechanisms of mental health among Chinese adolescents.

This study develops a "Bio-Social-Cognitive Dynamic Interaction Model" to explore seasonal mental health risks in Chinese adolescents. Using data from cross-sectional surveys (N = 6,121), longitudinal tracking (N = 1,000), and RCTs (N = 200), we assess mental health via PHQ-9, GAD-7, and SADQ scales. We analyze multidimensional indicators (melatonin, vitamin D, academic stress) via SEM and ARIMA, considering school stage and gender. Results show winter has the highest depression and anxiety scores (PHQ-9: M = 14.5 ± 4.8), linked to vitamin D deficiency (β=-0.25, p < 0.001) and social stress. S

State of the Science: The Unified Protocol for Transdiagnostic Treatment of Emotional Disorders.

Before the development of the Unified Protocol for the Transdiagnostic Treatment of Emotional Disorders (UP), evidence-based treatment options for commonly co-occurring anxiety, mood, and related disorders consisted of numerous single-disorder protocols that shared many similarities, reflecting the overlap among these disorders themselves. The UP distilled common elements of cognitive behavioral protocols into a unified intervention directly targeting core temperamental features underlying these disorders of emotion, namely neuroticism and associated emotion dysregulation. The UP has since bec

Discriminant Subspace Low-Rank Representation Algorithm for Electroencephalography-Based Alzheimer's Disease Recognition.

Alzheimer's disease (AD) is a chronic progressive neurodegenerative disease that often occurs in the elderly. Electroencephalography (EEG) signals have a strong correlation with neuropsychological test results and brain structural changes. It has become an effective aid in the early diagnosis of AD by exploiting abnormal brain activity. Because the original EEG has the characteristics of weak amplitude, strong background noise and randomness, the research on intelligent AD recognition based on machine learning is still in the exploratory stage. This paper proposes the discriminant subspace low

Honeycomb: a template for reproducible psychophysiological tasks for clinic, laboratory, and home use.

OBJECTIVE: To improve the ability of psychiatry researchers to build, deploy, maintain, reproduce, and share their own psychophysiological tasks. Psychophysiological tasks are a useful tool for studying human behavior driven by mental processes such as cognitive control, reward evaluation, and learning. Neural mechanisms during behavioral tasks are often studied via simultaneous electrophysiological recordings. Popular online platforms such as Amazon Mechanical Turk (MTurk) and Prolific enable deployment of tasks to numerous participants simultaneously. However, there is currently no task-crea

The Zebrafish Tol2 System: A Modular and Flexible Gateway-Based Transgenesis Approach.

Fetal alcohol spectrum disorders (FASD) are characterized by a highly variable set of structural defects and cognitive impairments that arise due to prenatal ethanol exposure. Due to the complex pathology of FASD, animal models have proven critical to our current understanding of ethanol-induced developmental defects. Zebrafish have proven to be a powerful model to examine ethanol-induced developmental defects due to the high degree of conservation of both genetics and development between zebrafish and humans. As a model system, zebrafish possess many attributes that make them ideal for develo

What good are positive emotions for treatment? Trait positive emotionality predicts response to Cognitive Behavioral Therapy for anxiety.

OBJECTIVE: Cognitive behavioral therapy (CBT) is empirically supported for the treatment of anxiety disorders; however, not all individuals achieve recovery following CBT. Positive emotions serve a number of functions that theoretically should facilitate response to CBT - they promote flexible patterns of information processing and assimilation of new information, encourage approach-oriented behavior, and speed physiological recovery from negative emotions. We conducted a secondary analysis of an existing clinical trial dataset to test the a priori hypothesis that individual differences in tra

Stress Induces a Shift Towards Striatum-Dependent Stimulus-Response Learning via the Mineralocorticoid Receptor.

Stress is assumed to cause a shift from flexible 'cognitive' memory to more rigid 'habit' memory. In the spatial memory domain, stress impairs place learning depending on the hippocampus whereas stimulus-response learning based on the striatum appears to be improved. While the neural basis of this shift is still unclear, previous evidence in rodents points towards cortisol interacting with the mineralocorticoid receptor (MR) to affect amygdala functioning. The amygdala is in turn assumed to orchestrate the stress-induced shift in memory processing. However, an integrative study testing these m

An fMRI study of risk-taking following wins and losses: implications for the gambler's fallacy.

Human decision-making involving independent events is often biased and affected by prior outcomes. Using a controlled task that allows us to manipulate prior outcomes, the present study examined the effect of prior outcomes on subsequent decisions in a group of young adults. We found that participants were more risk-seeking after losing a gamble (riskloss) than after winning a gamble (riskwin), a pattern resembling the gambler's fallacy. Functional MRI data revealed that decisions after riskloss were associated with increased activation in the frontoparietal network, but decreased activation i

Retrospective study of the use of hydromorphone in palliative care patients with normal and abnormal urea and creatinine.

An uncontrolled retrospective study was conducted looking at the use of oral controlled-release hydromorphone in palliative care patients. Over a 2-year period 55 patients were switched to hydromorphone therapy, and the efficacy and outcomes were assessed. Urea and electrolyte measurements were also recorded at the time of opioid switch and renal impairment defined as urea > 10.5 mmol/l and/or creatinine > or = 101 mmol/l. This group of 29 patients with abnormal urea and/or creatinine (Group 1) was compared with the remaining 26 patients (Group 2) who had normal urea and creatinine. The major

Technical note: equine skeletal preservation techniques to enhance teaching effectiveness.

When the decision was made to euthanatize an acutely laminitic Thoroughbred broodmare, graduate students from the Department of Animal Sciences and Industry reconstructed the skeleton for use as a teaching tool. The reproductive and gastrointestinal tracts were removed and preserved in formalin. The hide, muscle, tendons, ligaments, and organs were removed, and the bones were boiled in water for > or = 48 h to remove all remaining tissue. After boiling, the bones were soaked in gasoline to remove fat from the marrow cavities and then soaked in a bleach/detergent mixture as a final cleaning ste

[Expert consensus on late stage of critical care management].

We wished to establish an expert consensus on late stage of critical care (CC) management. The panel comprised 13 experts in CC medicine. Each statement was assessed based on the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) principle. Then, the Delphi method was adopted by 17 experts to reassess the following 28 statements. (1) ESCAPE has evolved from a strategy of delirium management to a strategy of late stage of CC management. (2) The new version of ESCAPE is a strategy for optimizing treatment and comprehensive care of critically ill patients (CIPs) after the
