Application of various compositions of thoracodorsal perforator flap for craniofacial contour deformities.

BACKGROUND: Craniofacial contour defects are challenging to restore because they may involve multiple tissues and span several aesthetic sub … microsomia, infection, post excision of venous malformation, lipodystrophy, craniectomy for chronic frontal sinusitis and infected pneumocephalus. They were reconstructed with thoracodorsal perforator flaps bearing various components, that is, adiposal, adipofascial, dermoadiposal, adipomyofascial and osteomuscular elements. Half of the flaps were in chimaeric form …

When Appearances Deceive: A Silent Somatotroph Macroadenoma Presenting With Acromegaloid Features and Paradoxically Low Insulin-Like Growth Factor-1.

Silent somatotroph adenomas are uncommon pituitary neuroendocrine tumours (PitNETs) of the PIT1 lineage that express growth hormone (GH) immunohistochemically but without biochemical evidence of acromegaly. We report the case of a 32-year-old male patient referred with recurrent epistaxis, headache, right-sided diplopia, and third cranial nerve palsy. Clinical examination revealed acromegaloid features including frontal bossing, spade-like hands, and coarse facies, which were strongly suggestive of longstanding GH excess. However, preoperative insulin-like growth factor-1 (IGF-1) was paradoxic

Unusual metastasis of breast cancer to the paranasal sinuses: a case report.

BACKGROUND: Breast cancer is the most common malignancy in women, with invasive ductal carcinoma being the predominant subtype. Although metastases commonly affect the bones, liver, lungs, and brain, involvement of the paranasal sinuses is exceedingly rare and poses significant diagnostic challenges. CASE PRESENTATION: A 70-year-old postmenopausal woman from Pakistan with a history of left-sided mastectomy and chemotherapy for invasive ductal carcinoma presented with an altered sensorium, aphasia, and reduced oral intake. Imaging revealed an aggressive, destructive lesion in the left frontal

The frontal sinus of the adult human cranium from Border Cave, KwaZulu-Natal, South Africa: Does it provide a contextual clue?

Human specimens from Border Cave thought to derive from its Middle Stone Age (MSA) deposits have been widely discussed in relation to Late Pleistocene evolution in southern Africa. Among the most prominent is the adult cranium BC 1. However, there are questions about the context and geochronological age of a number of these remains, including BC 1, which was recovered ex situ from a sediment dump created by miners digging for guano. The cranium has not been directly dated. Previous morphological analyses have clearly demonstrated its complete anatomical modernity, but its possible affinities t

Calvarial Vault Remodeling Technique for Lambdoid Craniosynostosis.

Premature fusion of the lambdoid suture is the most uncommon single-suture synostosis. It presents with a classic "windswept" appearance, a trapezoid-shaped head, and significant skull asymmetry notable for an ipsilateral mastoid bulge and contralateral frontal bossing. Because of the rarity of lambdoid synostosis, little is known about optimal techniques for its treatment. In particular, the proximity of the lambdoid suture to critical intracranial structures such as the superior sagittal and transverse sinuses represents a potential for significant intraoperative bleeding. Prior work has sho

Pediatric Facial Fractures: An Assessment of Airway Management.

Pediatric facial fractures present unique and challenging management considerations, especially with regards to airway management. Anatomical differences in children increase both airway resistance and the difficulty of intubation. A surgical airway may be required if intubation is unable to be performed. The purpose of this study was to examine a single center's experience with pediatric facial fractures to determine the frequency of advanced airway use, as well as the risk factors that may predispose a patient to requiring an advanced airway. A retrospective review of all facial fractures at

Anatomic relationship of the first olfactory neuron and trochlea: cadaveric study with surgical implications.

BACKGROUND: Iatrogenic injury to the trochlea is a potential complication of the Draf III or endoscopic modified Lothrop procedure; however, the location of the trochlea from within the sinus cavity has yet to be clearly characterized. We performed the first cadaveric study assessing the position of the trochlea in relation to the first olfactory neuron, a commonly identified landmark during the Draf III procedure. METHODS: Thirteen external dissections of the trochlea were performed on 7 disarticulated cadaveric heads via an extended Lynch type incision. An endoscopic Draf III procedure was

Removal of dental implant displaced into maxillary sinus by combination of endoscopically assisted and bone repositioning techniques: a case report.

BACKGROUND: Accidental displacement of a dental implant into the maxillary sinus is an infrequent although not uncommon complication encountered in dental clinical practice, with the main cause thought to be inadequate bone height in the posterior maxilla. We report a case of migration of a dental implant into the maxillary sinus, and discuss the benefits of its removal by a combination of endoscopically assisted and bone repositioning techniques. CASE PRESENTATION: A 35-year-old Japanese man with a partially edentulous maxilla underwent implant placement at a private clinic. Three months lat

Therapy options and long-term results of sinonasal malignancies.

OBJECTIVES: Nasal and paranasal sinus malignancies are rare. The most common lesions are located in the nasal cavity and the maxillary sinus, although they also occur in the ethmoid, sphenoid and frontal sinuses. Treatment often combines surgery, radiotherapy and chemotherapy. Endoscopic surgical approaches are increasingly used in order to reduce the morbidity associated with standard open resection. The aim of our study was to analyse the long-term treatment results of sinonasal malignancies (SNM), with a special focus on surgical approaches. MATERIALS AND METHODS: A retrospective review of

Utility of a three-dimensional endoscopic system in skull base surgery.

We evaluated the utility of a three-dimensional (3-D) endoscopic system for skull base surgery. We performed a retrospective case series in a tertiary care medical center. Thirty-six patients underwent skull base (nonpituitary) resections via 3-D endoscopic system. Fifteen patients (42%) were operated for excision of malignant tumors, 19 (53%) for excision of benign lesions, and 3 (8.3%) for skull base reconstruction. The tumors involved the cribriform plate (n = 13), sphenoid sinus and planum (n = 17), clivus (n = 7), and sella (n = 7). Complete tumor resection was achieved in 31 patients and

Fortuitously discovered persistent left superior vena cava in young competitive athletes. Clinical implications of sports physicians.

This report describes two athletes with persistent left superior vena cava (PLSVC) accidentally identified during preparticipation medical evaluation. The clinical implications of PLSVC for sports physicians are also discussed. A 16-year-old male ice hockey player and an 18-year-old male high-level field hockey player visited our institute for medical evaluation prior to participating in competition. Neither complained of palpitation, faintness or syncope, which would have suggested a possible cardiac rhythm disturbance, or had been informed of any abnormalities in previous physical examinatio

Fungal infections.

Fungal infections have increased in frequency in the last decades because of the growing number of immunocompromised patients who survive longer periods of time than in the past, the widespread use of immunosuppressive drugs, a large aging population with increased numbers of malignancies, and the spread of AIDS. Although fungi are present everywhere, some mycoses predominate in the tropics, not only in view of warm temperature and humid climate, which favor their growth, but also because of inadequate hygienic and working conditions brought about by poverty. Mycotic diseases in the brain are

Magnetic resonance angiography of supratentorial tumours: comparison with selective digital subtraction angiography.

We compared magnetic resonance angiography (MRA) and intra-arterial digital subtraction angiography (IADSA) in the study of brain tumours and assessed the utility of gadolinium-enhanced MRA. We studied 17 patients with supratentorial brain tumors. The entire brain was imaged with multiple overlapping thin volume acquisitions. After IV injection of gadolinium-DTPA, a single thick-slab MRA acquisition was performed. Standard three-dimensional (3D-TOF) acquisitions (in six patients) and 3D-TOF with magnetization transfer prepulse and tilted optimisation nonsaturing radiofrequency excitation pulse

Use of bone for obliteration of the nasofrontal duct with the osteoplastic flap: a cat model.

The management of the frontal sinus remains controversial for traumatic and inflammatory disease. The osteoplastic flap with fat obliteration has become the treatment standard, although late failures with infection and mucocele formation may arise. Theories for mucocele formation include regrowth of residual mucosa from the sinus and ingrowth of mucosa from a nonobliterated nasofrontal duct. A study to evaluate the means of nasofrontal duct obliteration was developed. Three cat groups were evaluated at 1, 2, and 3 months. Group 1 had the duct and sinus obliterated with fat. Group 2 had the duc

NARES: a model of inflammation caused by activated eosinophils?

Twenty patients were selected on the basis of perennial rhinitis, the absence of allergy and with an eosinophil count higher than 20% of total leucocytes in nasal secretions (NARES). Nasal endoscopy with biopsies from the middle turbinate and sinus CT were performed. Biopsies were processed for histological examination and for immunofluorescence. The clinical progress during treatment was scrutinized. An acute congestive aspect of the nasal mucosa was noted in 4 cases, and micropolyposis in 9 cases. Sinus CT showed opacity of the ethmoidal cells in 87% of cases (maxillary sinuses: 75%; frontal
