Hyperpigmentation of the distal phalanx of both hands and feet is usually a prominent feature of dark-skinned newborns, but recently it has been described also in fair-skinned ones as a coincidental finding and classified as a transient benign disorder of infancy. We report our experience, having observed a group of at-term newborns and a group of premature newborns. The group of premature newborns did not show any hyperpigmentation. On the contrary, 7 out of 40 fair-skinned at-term newborns had hyperpigmentation in the distal phalanx of the fingers which was not related to any dermatologic di

Neutrophilic dermatosis of the dorsal hands (NDDH) is a rare and recently described disorder regarded as a subset of neutrophilic dermatosis, similar to superficial pyoderma gangrenosum and pustular vasculitis. Many currently consider it to be a localized variant of Sweet Syndrome. We describe the case of a 63-year-old male patient with NDDH associated with hypopharyngeal carcinoma. A biopsy demonstrated changes consistent with those seen in Sweet Syndrome, with leukocytoclasia but no signs of true vasculitis. Treatment with oral prednisolone resulted in complete resolution of the lesions in 1

BACKGROUND: Palmar pits are intriguing and are found in association with diverse dermatologic conditions. Some are innocuous, whereas others have sinister implications.
OBJECTIVE: Six cases of asymptomatic palmar pits are described, and the literature is reviewed in brief.
METHOD AND RESULTS: Of the six patients, five were male and one was female, with a mean age of 63.5 years. In all six patients, the palmar pits were noticed incidentally during a cutaneous examination for some other unrelated dermatoses, except in one in whom the pits were the reason for visiting a dermatologist. The numbe

Healthcare workers (HCW) have an increased risk of suffering from occupational skin diseases (OSDs). Therefore, we established special prevention and skin protection courses for HCW. Medical aims are to optimize diagnostic procedures and individual therapy, educational aims are to improve individual skin protection/skin care habits of the patient and also to influence the participants' attitudes towards health. Participants are patients that have been suspected to suffer from an OSD and are insured with the German Accident Prevention Insurance Association for Health Care Workers (BGW). Teachin

Irritant contact dermatitis (ICD) is often chronic; its aetiology frequently being related to occupational exposure. Management of ICD involves persistent reduction in exposure to skin irritants such as water, detergents and prolonged occlusion by gloves. The aim of this study was to determine the severity of hand dermatitis 5 years after initial diagnosis and to find out what factors were related to this outcome. A questionnaire survey was carried out on severity of hand dermatitis, exposure to skin irritants and preventive measures, 5 years after initial ICD diagnosis. Of a cohort of 201 pat

A self-completion questionnaire sent to 2600 Nottinghamshire members of the Graphical Paper and Media Union elicited a 62% response. Forty one per cent of respondents reported suffering a skin complaint at some time and 11% had a current skin problem on the hand. This paper reports the validation stage of the study. Samples of 45 'cases' of self-reported dermatitis and 60 'controls', who reported they had never suffered a skin complaint, were clinically examined. All 45 self-reported cases were clinically confirmed as dermatitis. Occupationally related irritant contact dermatitis (ICD) was dia

OBJECTIVE: Dyshidrotic hand eczema is a therapeutic challenge. A prospective pilot study was performed with left-right comparison in order to investigate whether chemical de-innervation of sudoriferic nerves would be superior to standard therapy with topical corticosteroids.
BACKGROUND: Botulinum toxin A (BTXA) is a potent inhibitor of acetylcholine release, that induces eccrine sweat production and release. Inhibition of sweating by other measures such as tap water iontophoresis has been shown to be beneficial in dyshidrotic hand eczema.
METHODS: Eight adult patients suffering from dyshidro

Porphyria cutanea tarda (PCT) is frequently described among patients with chronic renal failure undergoing hemodialysis. One patient who was receiving peritoneal dialysis and had PCT develop has been described in the literature. However, it was later determined that the patient's PCT was related to the hepatotoxic drug she was receiving rather than her peritoneal dialysis. Our patient is the first reported case, to our knowledge, of a patient with end-stage renal disease with negative hepatitis serology who was not receiving a hepatotoxic drug, or on hemodialysis, who had PCT develop while rec

We describe a patient with sporadic dystrophic epidermolysis bullosa associated with well-documented atopic dermatitis. We discuss this case in relation to a newly described clinical subtype of epidermolysis bullosa known as epidermolysis bullosa pruriginosa, a dystrophic variant associated with prominent pruritus. The relations of this case of sporadic dystrophic epidermolysis bullosa with other dominantly inherited forms of dystrophic epidermolysis bullosa such as the Pasini variant, the pretibial variant, and Bart's syndrome are also discussed. The role of atopic dermatitis in exacerbating

OBJECTIVE: To determine frequency and prevalence of finger pad tophaceous deposits in patients with chronic tophaceous gout.
METHODS: We examined 36 consecutive patients with chronic tophaceous gout for intracutaneous tophaceous urate deposits in the fingers pads. Subjects were patients of outpatient clinics at 2 large university hospitals, Emory and the University of Pennsylvania Affiliated Hospital Systems.
RESULTS: Finger pad tophaceous deposits were found in 30.5%, which was surprising, given how few cases are reported.
CONCLUSION: Finger pad tophi may be more common than generally beli

Allergy to oil of turpentine has diminished largely due to the use of cheaper substitutes in many occupations. However, 2 particular areas still reliant on real oil of turpentine are those of the perfume industry and ceramic decoration. We report 24 cases of hand dermatitis in pottery workers involved in ceramic decoration, paintresses, liners, gilders, enamellers and a fine china painter, seen in a 6-month period following a change from Portuguese to Indonesian turpentine, of whom 14 were sensitive to Indonesian turpentine, 8 to alpha-pinene, 4 to delta-3-carene and 2 positive to turpentine p

Acrylates have a broad area of application in various products including glues, sealants and adhesives. Whereas anaerobic acrylic sealants are well-known sensitizers, acrylate glues that cure in air have only seldom been reported as allergens. Here a patient sensitized to such a glue, and developing hand dermatitis that spread to the lower arms, chest, neck and face, is presented. Her glue was analyzed by gas chromatography/mass spectrometry (GC/MS) and contained 24.6% 2-hydroxyethyl methacrylate (2-HEMA) and 0.4% ethylene glycol dimethacrylate (EGDMA). These 2 acrylate compounds, as well as h

The clinical courses of Borrelia burgdorferi infections may show such variety, that diagnostic problems arise. On the other hand, early antibiotic therapy is necessary in order to prevent late-term complications. In this article, we present an exceptional case of a long-term documentation of Lyme-disease which was not diagnosed until an acrodermatitis chronica atrophicans had developed. Whether the manifestation of bilateral carpal tunnel syndrome is connected with Lyme borreliosis is yet to be clarified.

Water content of the epidermis was measured before and after a 3-h , 4-day, and 10-day treatment using 10% salicylic acid or 10% carbamide ointment. The measurement was performed using the Corneometer CM 420 from the firm "Schwarzhaupt Medizintechnik GmbH". Salicylic acid ointment was applied on one and carbamide ointment on the other back of the hand. Neither salicylic acid nor carbamide seem to influence significantly the water content of the healthy epidermis. In two patients with more severe hyperkeratosis carbamide increased the water content more than salicylic acid did.
