Relation between mitral regurgitation and platelet activation.

OBJECTIVES: This study sought to examine the effect of mitral regurgitation (MR) on platelet activation in patients with mitral valve prolapse (MVP) or rheumatic MR. BACKGROUND: MVP and rheumatic MR are associated with an increased incidence of thromboembolic events. Although the underlying causes are not clear, increased platelet activation has been suggested as one of the pathogenic mechanisms. Results of previous studies that have investigated the relation between MVP and platelet activation are controversial. Whether the presence of MR in patients with mitral valve disease is associated w

Left ventricular fibrosis and CMR tissue characterization of papillary muscles in mitral valve prolapse patients.

PURPOSE: Mitral valve prolapse (MVP) is associated with left ventricle (LV) fibrosis, including the papillary muscles (PM), which is in turn linked to malignant arrhythmias. This study aims to evaluate comprehensive tissue characterization of the PM by cardiovascular magnetic resonance (CMR) imaging and its association with LV fibrosis observed by intraoperative biopsies. METHODS: MVP patients with indication for surgery due to severe mitral regurgitation (n = 19) underwent a preoperative CMR with characterization of the PM: dark-appearance on cine, T1 mapping, conventional bright blood (BB)

Normal sympathetic neural activity in patients with mitral valve prolapse.

The hemodynamic and neurohumoral responses to head-up tilt and isometric exercise were studied in 14 mitral valve prolapse (MVP) patients with and 10 MVP patients without complaints and in 16 healthy control subjects. Plasma catecholamines at rest were not different between the three groups and neither were their changes during either test. The hemodynamic changes during head-up tilt were not different between the three groups. The symptomatic MVP patients showed a lower rise in diastolic blood pressure (14.3 +/- 6.4%) than the controls (22.9 +/- 9.6%) (p less than 0.05) during isometric exerc

[Echocardiography in the diagnosis and evaluation of the results of the surgical treatment of interatrial septal defects in adult patients].

Pre- and postoperative echocardiographic findings in 67 patients above 18 years of age with atrial septal defects were compared with hemodynamic data obtained at catheterization of heart cavities. No possible ways of accurate noninvasive assessment of arteriovenous discharge were found. Echocardiographic parameters of the followed-up patients returned to normal within 3 to 4 years after surgical correction of the defect. In some cases where mitral prolapse was found preoperatively, angiographically documented mitral regurgitation was seen at long-term follow-up.

Mitral valve prolapse in the ventriculogram: scintigraphic, electrocardiographic, and hemodynamic abnormalities.

Patients with mitral valve prolapse (MVP) frequently have chest pain, which may be difficult to differentiate from angina pectoris in coronary artery disease (CAD). We performed resting and exercise ECGs, pulmonary arterial pressure measurements, radionuclide ventriculography (99mtechnetium), and perfusion scintigrams (201thallium) in 56 patients with angiographically proven MVP and no CAD. Pathological results were obtained in 31% of exercise ECGs, 33% of pulmonary arterial pressure measurements during exercise, 22% of radionuclide ventriculographies, and in 75% of thallium perfusion scintigr

The role of reduced left ventricular enddiastolic volume in the apparently high prevalence of mitral valve prolapse in atrial septal defect.

Twenty-three patients with ostium-secundum atrial septal defect (ASD) were studied in order to investigate the etiology of angiographic mitral valve prolapse associated with ostium-secundum ASD. 12 patients (52%) had angiograhic MVP. Ventricular volumes, ejection fraction, segmentary contractility and oxygen step-up were analyzed in all patients. Patients with MVP had smaller enddiastolic and stroke volumes (73 +/- 21.8 ml/m2 and 46.6 +/- 18 ml/beat/m2) than patients without MVP (106.6 +/- 22 ml/m2 and 78 +/- 11.3 ml/beat/m2) (P less than 0.01). Oxygen step-up was greater in patients with MVP

[Phase analysis of the normal and postectopic left ventricular contractions in nonejection click-telesystolic apical murmur syndrome].

ECG, PCG, CSG and ACG were synchronously recorded in 25 patient with non-expulsion click syndrome -- telesystolic murmur as well as in 62 cardiac healthy subjects. With the investigation of the systolic and diastolic intervals, a prolongation of the transformation period was established (Q--1 = 72 +/- 12 msec) and a shortening of the isovolumetric relaxation (A2--0 = 63 +/- 10 msec). The ratio PPE/LVET (period prior to expulsion/left venticular expulsion time) is normal in patients with nonexpulsion click -- telesystolic apical murmur but the comparison of PPE/LVET with a normal contraction wi

Biopsy evidence of left ventricular myocardial abnormality in patients with mitral-leaflet prolapse and chest pain.

Left ventricular endomyocardial biopsy specimens were obtained at cardiac catheterisation in 11 patients with mitral-leaflet prolapse, chest pain, and normal coronary arteries. Specimens were examined for birefringence before and after contraction induced by adenosine triphosphate and were also subjected to histochemical analysis. Biopsy specimens from a control group of patients with rigorously defined normal left ventricular function and another control group with poor left ventricular function were also studied. Biopsy specimens from patients with mitral-leaflet prolapse showed a markedly s

Pronounced unexplained preoperative tachycardia heralding serious cardiac events: a series of three cases.

PURPOSE: Pronounced, unexplained preoperative tachycardia can be a formidable challenge for the anesthesiologist. Whereas the relationship between persistent intraoperative tachycardia and perioperative morbidity is indisputable, there is a lack of available data on unexplained preoperative tachycardia. The main objective of this case series is to stimulate research and discussion on this topic, so that guidelines can be developed to aid in management. CLINICAL FEATURES: We present three patients with pronounced (> or = 130 beats x min(-1)) unexplained preoperative tachycardia who suffered ad

Impact of Concomitant Tricuspid Annuloplasty on Tricuspid Regurgitation, Right Ventricular Function, and Pulmonary Artery Hypertension After Repair of Mitral Valve Prolapse.

BACKGROUND: For patients undergoing mitral valve (MV) repair, the indications for and results of concomitant tricuspid annuloplasty remain controversial. OBJECTIVES: This study was designed to compare a strategy of routine concomitant tricuspid annuloplasty for moderate tricuspid regurgitation (TR) or tricuspid annular dilation in patients undergoing degenerative MV surgery. METHODS: Of 645 consecutive patients (mean age 57±13 years) undergoing primary repair of degenerative mitral regurgitation between 2003 and 2011, 419 (65%) underwent concomitant tricuspid annuloplasty for moderate TR and

Surgical management of mitral regurgitation associated with Marfan's syndrome.

BACKGROUND: The surgical treatment of mitral regurgitation associated with Marfan's syndrome remains controversial because of the underlying degenerative process. METHODS: From October 1986 to June 1996, 33 patients with Marfan's syndrome underwent a mitral valve procedure. The mean age was 30 years (range, 2 to 55 years). Mitral regurgitation was caused by annulus dilatation in 2 patients, leaflet prolapse in 30 patients, and restricted leaflet motion in 1 patient. Mitral valve repair was performed in all patients except 1 who had mitral valve replacement. RESULTS: Two patients died in the

Granulicatella adiacens infective endocarditis in pregnancy: diagnostic contribution of metagenomic sequencing-a case report.

BACKGROUND: Granulicatella adiacens is a fastidious Gram-positive coccus and is a rare but recognized cause of infective endocarditis. Infective endocarditis during pregnancy is uncommon but carries substantial maternal and fetal risk. Plasma metagenomic analysis of microbial cell-free DNA has emerged as a complementary diagnostic tool in culture-negative infections. CASE SUMMARY: We describe a 35-year-old pregnant woman with known mitral valve prolapse who presented at 21 weeks of gestation with an acute ischemic stroke. Initial etiological work-up, including transesophageal echocardiography

A case report of delayed left ventricular rupture after mitral transcatheter edge-to-edge repair: clip entrapment in hypercontractile left ventricle.

BACKGROUND: Left ventricular (LV) rupture is an extremely rare but possible complication after mitral transcatheter edge-to-edge repair (M-TEER). We describe a delayed LV rupture after M-TEER that was successfully treated with surgical repair. CASE SUMMARY: An 83-year-old Asian male with congestive heart failure was referred for treatment of severe mitral regurgitation (MR) due to A1/A2 segment prolapse with abnormally hypertrophied anterior papillary muscle. The patient was at high surgical risk, and M-TEER with MitraClip (Abbott Vascular, Minneapolis, MN, USA) was performed. During the proc

Implications of Mitral Annular Disjunction in Patients Undergoing Transcatheter Edge-to-Edge Repair for Degenerative Mitral Regurgitation.

BACKGROUND: Little is known about mitral transcatheter edge-to-edge repair (TEER) in patients with mitral annular disjunction (MAD). OBJECTIVES: The authors sought to explore TEER for degenerative mitral regurgitation (MR) according to MAD status. METHODS: We retrospectively analyzed 271 consecutive patients (median age 82 [Q1-Q3: 75-88] years, 60.9% men) undergoing an isolated, first-ever TEER for whom there were viewable preprocedural echocardiograms. Stratified by MAD status at baseline, the cohort was evaluated for all-cause mortality, heart failure hospitalizations, and mitral reinterve
